Thus, in patients with EAH, electrocardiographic (ECG) criteria of left ventricular hypertrophy (LVH) can occur without changing the mass of the left ventricular myocardium and its geometric characteristics. At the same time, they have independent diagnostic and prognostic value.</p><p><strong>Purpose of the study: </strong>Evaluation of the effect of manual-physical effects according to the method of A.Yu. Shishonin on the amplitude manifestations of electrophysiological myocardial remodeling in patients with EAH.</p><p><strong>Material and methods: </strong>We examined 148 patients with EAH with high normal blood pressure (BP), or arterial hypertension of the 1st degree, who were divided into the main and control groups. The main group included 114 patients (65 women and 49 men; mean age 61.8±5.2 years) with clinical manifestations of cervical osteochondrosis, who were treated with EAG using the method of A.Yu. Shishonin, including step-by-step manual-physical correction with the formation of a stable muscle corset in the cervical spine. The control group consisted of 34 patients (18 men and 16 women; mean age 61.6±4.0 years) who did not undergo treatment. To prove the effectiveness of the technique in reducing blood pressure and affecting the ECG pattern, instrumental diagnostic techniques for assessing blood pressure using the Korotkov auscultatory method and recording ECG criteria in 12 leads, respectively, were used.</p><p><strong>Results: </strong>All amplitude ECG criteria had significantly lower values after EAG correction using the method of A.Yu. When assessing the dynamics of the number of patients with amplitude criteria for LVH, a reliable decrease in this indicator was shown during therapy using the method of A.Yu. Shishonin: from 25.69 to 9.03% according to the Cornell criterion and from 26.39 to 8.33% according to the Sokolov-Lyon index. Considering that all patients did not have morphological signs of target organ damage, including criteria for concentric myocardial hypertrophy, the obtained results can be explained by effects at the tissue and cellular level.</p><p><strong>Conclusions: </strong>In the process of manual-physical correction using the method of A.Yu. Shishonin, are used in representatives of both sexes.As a result, it is of interest to assess gender differences that can determine the features of the course, manifestations of pathology and response to the methods of treatment used.</p><p><strong>Objective: </strong>Study of changes in blood flow parameters in the vertebrobasilar basin under the influence of manual-physical correction according to the author's method of A.Yu. Shishonin.</p><p><strong>Material and methods: </strong>A total of 112 patients suffering from EAG were examined, including 49 men (aged 63.3±9.2 years) and 63 women (aged 62.1±8.3 years) with signs of clinically expressed degenerative-dystrophic processes in the cervical spine and arterial pressure (AP) corresponding to high normal AP (prehypertension), or EAG of the 1st degree. Methods of examination and treatment. All patients received the procedure of manual-physical correction according to the method of A.Yu. Shishonin. {"title":"[Prognostic model of a patient with sepsis readiness to early mobilization with assistance].","authors":"I A Ruslyakova, E Z Shamsutdinova, K I Shirokov, N E Ivanova","doi":"10.17116/kurort202510202126","DOIUrl":"10.17116/kurort202510202126","url":null,"abstract":"<p><p>Management of the early rehabilitation (passive, assisted, active) risks will allow to minimize complications and increase safety of patients with sepsis.</p><p><strong>Objective: </strong>To develop a prognostic model of patient with sepsis readiness to early mobilization with assistance.</p><p><strong>Material and methods: </strong>A single-center retrospective cohort observational study with analysis of 780 electronic medical records of patients with sepsis from the «Intensive therapy of patients with severe community-acquired pneumonia» base of the North-Western State Medical University named after I. I. Mechnikov in the period from 2013 to 2022 was carried out. Statistical analysis by binary logistic regression method and ROC-analysis were performed using Jamovi 2.3.21 and IBM SPSS Statistics 26 softwares package.</p><p><strong>Results: </strong>The study includes the data of 519 patients with sepsis. Patients are divided into 2 study groups according to the score on the Intensive Care Unit Mobility Scale (IMS): IMS 0/1 (281 patients) and IMS ≥2 (238 patients). The predictors of IMS ≥2 for patients with sepsis have been determined: score on the APACHE II assessment scale of patient's state severity ≤20.0 (adjusted odds ratio (AOR) 6.38; 95% CI 3.01-13.5; <i>p</i><0.001), score on the CCI Charlson comorbidity index ≤3.0 (AOR 3.84; 95% CI 1.72-8.58; <i>p</i>=0.001), score on the mNUTRIC critical state risk scale ≤5.0 (AOR 2.81; 95% CI 1.33-5.96; <i>p</i>=0.007), PO<sub>2</sub>/FiO<sub>2</sub> index >119.0 mm Hg (AOR 104; 95% CI 29.59-365.6; <i>p</i><0.001), albumin level >25.0 g/l (AOR 2.37; 95% CI 1.11-5.06; <i>p</i>=0.025). The prognostic model that considers 82.3% of factors determining the dispersion of IMS ≥2 probability has been developed. Area under ROC-curve amounted to 0.967±0.007 (95% CI 0.952-0.982). Sensitivity and specificity of IMS ≥2 prognostic model in patients with sepsis at 65% threshold were 92.0 and 92.6%, respectively, and accuracy - 92.3%.</p><p><strong>Conclusion: </strong>The prognostic model of patient with sepsis readiness to early mobilization with assistance has been developed based on IMS ≥2 independent predictors. {"title":"[Possibilities of vibroacoustic therapy application in patients with chronic bronchopulmonary pathology].","authors":"A M Schikota, I V Pogonchenkova, E A Turova, I M Rud', M A Rassulova","doi":"10.17116/kurort202510201112","DOIUrl":"10.17116/kurort202510201112","url":null,"abstract":"<p><p>Chronic bronchopulmonary diseases are a frequent concomitant pathology which potentially affects the clinical and functional status of patients of different profiles undergoing rehabilitation in hospital, that should be taken into account in planning treatment.</p><p><strong>Objective: </strong>To study the influence of vibroacoustic pulmonary therapy (VAPT) on clinical status and spirometry values of patients undergoing in-patient rehabilitation and with comorbid bronchopulmonary pathology.</p><p><strong>Material and methods: </strong>We examined 60 patients (18 men (30%) and 42 women (70%)) aged between 18 and 75 years (median age 62 [56; 68] years) with chronic lung diseases (chronic bronchitis, chronic obstructive pulmonary disease), undergoing an in-patient rehabilitation for disorders of the central nervous and musculoskeletal systems. The study group consisted of 30 patients who had a 5-day course of VAPT in the individual medical rehabilitation program; the control group included 30 patients who did not underwent VAPT. Qualitative and quantitative characteristics of respiratory symptoms and parameters of spirometry before and after VAPT course were studied.</p><p><strong>Results: </strong>In this study 37 (62%) of subjects underwent rehabilitation regarding pathology of the central nervous system, 23 (38%) - regarding musculoskeletal pathology, 40 (67%) had concomitant cardiovascular diseases among all patients. Complaints of dyspnea were presented by 96.7% of patients in the study group and 90% of patients in the control group, mean value of dyspnea intensity on the mMRC scale amounted to 2.1±0.89 and 2.1±0.8 scores, respectively. Cough occurred in 83% of patients in the study group and 80% of patients in the control group, mean value on the 6-score daily and night cough scale was 2.0±1.3 and 1.9±0.84 scores, respectively. There has been a decrease in the number of patients with dyspnea and cough to 83.3 and 66.7%, respectively, a decrease in the dyspnea degree on the mMRC scale by 19% (<i>p</i><0.05) and the cough severity by 22.3% (<i>p</i>≤0.001) in the study group after VAPT. An increase of the peak expiratory flow rate by 5% (<i>p</i>≤0.05) has been revealed according to spirometry data. No significant improvement of the studied indicators has been achieved in the control group of patients. Genetic polymorphism has an impact on morbidity, mortality and the reserves of the body's recovery in communicable diseases.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Objective: &lt;/strong&gt;To study the possibilities of rehabilitation of patients with post-COVID syndrome (PCS) in the conditions of a sanatorium-resort organization depending on the genetic polymorphism of the &lt;i&gt;CD-14&lt;/i&gt; monocytes receptor gene.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Material and methods: &lt;/strong&gt;The study included 97 patients with COVID-19 past history and PCS diagnosis, who underwent rehabilitation in the Department of Pulmonology of the I.M. Sechenov Research Institute of Physical Methods of Treatment, Medical Climatology and Rehabilitation (Yalta). Comprehensive clinical, laboratory and functional examinations of patients were performed. Questionnaires for assessment of dyspnea, psychological state and quality of life were additionally used. Determination of genotype variants was done using sets of the «Lytech» company (Russia) according to the manufacturer's instructions. The detection of amplification products for C(159)T polymorphism of the &lt;i&gt;SD-14&lt;/i&gt; gene was performed by means of horizontal electrophoresis in 3% agarose gel. Patients received comprehensive rehabilitation including: climatotherapy, physical therapy, massage, aromatherapy, respiratory therapy and pharmaceutical treatments as medically indicated. The treatment course was 21 days. Evaluation of the efficacy of therapy was carried out on the basis of studying the dynamics of the investigational indicators before and after the rehabilitation course.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Patients were divided into 3 groups according to the data of performed analysis: with variants of homozygous distribution of CC alleles - 30 (30.9%) patients and TT - 22 (22.7%) patients, as well as with heterozygous variant of CT - 45 (46.4%) patients. It has been established that the most pronounced respiratory symptoms, degree of dyspnea, low grade by questionnaires of dyspnea, low value of the indicators of pulmonary function tests, laboratory abnormalities, as well as more significant reduction of life quality, development of anxiety, low assessment of the level of one's own health are characteristic for CC genotype. Patients with this genotype's variant were more likely to have obesity and dyslipidemia, which exacerbated respiratory symptoms. The heterozygous CT variant of genotype, in which more indicators had a significant positive dynamics during rehabilitation, proved to be more sensitive to rehabilitative exposure.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Conclusion: &lt;/strong&gt;The state of PCS depending on the genotypes of the &lt;i&gt;SD-14&lt;/i&gt; gene was studied. The most pronounced symptoms were revealed in CC159 genotype. Balneotherapeutic methods occupy one of the first places in terms of the effectiveness of treatment of many diseases, that is especially relevant for the population living in harsh natural climatic conditions of the North.</p><p><strong>Objective: </strong>To assess the physiological action of natural mineral waters of the «Seregovo» sanatorium of the Komi Republic.</p><p><strong>Material and methods: </strong>An analysis of publications from databases of electronic sources (WOS, Scopus, PubMed, eLibrary) over the last 20 years, in which the results of mineral waters application are presented, was performed. The results of the study of composition and quality of mineral waters of the «Seregovo» sanatorium of the Komi Republic are presented (conclusion of the National Medical Research Center for Rehabilitation and Balneology (NMRC RB) and materials obtained by Institute of Geology of the Komi Scientific Center of the Ural branch of the Russian Academy of Sciences).</p><p><strong>Results: </strong>Distribution and diversity of mineral waters of the territory of the «Seregovo» sanatorium of the Komi Republic are presented. Mineral water from the 1K well refers to the subcategory of brines (over 35.0 g/dm3 but not more than 150.0 g/dm3), is of sodium-chloride composition (Cl<sup>-</sup> 100, Na<sup>+</sup>+K<sup>+</sup>79 mg eq.%), with total mineralization of M100.9 g/dm3, with pH equal 6.22, with conditioned content of bromine and boron according to the balneological conclusion №7-1372 from 24.05.2022 by the NMRC RB of the Ministry of Health of Russia. Physiological and therapeutic effects of sodium chloride mineral waters have been shown according to the literature.</p><p><strong>Conclusion: </strong>The diversity of natural mineral waters and dynamic change of their composition over time require periodic controlled study of their impact on human body. Shishonin)].","authors":"A Yu Shishonin, V I Pavlov","doi":"10.17116/kurort202510201137","DOIUrl":"10.17116/kurort202510201137","url":null,"abstract":"<p><p>Recently, there is increasing evidence of the relationship between degenerative-dystrophic processes of vertebral structures and cardiovascular pathology, including the development of essential arterial hypertension (EAH).In this regard, one of the methods of EAH therapy being developed is step-by-step manual-physical correction with the formation of a stable muscular corset in the cervical spine (method of A.Yu. Shishonin), to prove the effectiveness of which, an assessment of changes in blood flow in the vessels of the vertebral arteriesusing ultrasound was used.</p><p><strong>Objective: </strong>To study dynamics of blood flow in the vessels of the vertebral arteriesunder the influence of the author's method of manual-physical correction of A.Yu.Shishonin.</p><p><strong>Material and methods: </strong>144 patients with EAH and significant manifestations of cervical osteochondrosis, including varying degrees of vertebrobasilar blood flow disorderswere examined. The main group included 112 patients (mean age 63.0±5.1 years) with EAH,who were treated using the method of A.Yu. Shishonin. The control group included 32 patients (mean age 52.6±4.9 years) with EAH, who did not receive treatment.Blood flow studies in the cervical vertebral arteries were conducted using the method of ultrasound Dopplerography of the brachiocephalic arteries.</p><p><strong>Results: </strong>During therapy, all patients in the study group, on average, at least, showed an improvement in blood flow in the cervical segments of the vertebral arteries, accompanied by normalization of blood pressure.In patients in the control group, no significant changes occurred.The dynamics of the severity of changes in specific indicators of vertebrobasilar blood flow indicated, first of all, a decrease in peripheral resistance (ED initially 1.2±0.5 cm/s vs ED post-therapeutic 16.6±6.4 cm/s; TAMEAN initially 1.8±0.8 cm/s vs TAMEAN post-therapeutic 15.2±4.0cm/s). (Literature review)].","authors":"E F Legkaya, L S Khodasevich, A V Polyakova","doi":"10.17116/kurort202510202161","DOIUrl":"https://doi.org/10.17116/kurort202510202161","url":null,"abstract":"<p><p>The use of information and communication technologies (ICT) in comprehensive rehabilitation and habilitation of patients with autistic spectrum disorder (ASD) in current conditions seems to be actual, however it is not sufficiently illustrated in the literature to date.</p><p><strong>Objective: </strong>To analyze present scientific data on the comprehensive rehabilitation and habilitation of children with ASD by restoring lost or forming absent skills and abilities by adapted physical education using current ICT.</p><p><strong>Material and methods: </strong>A review of publications on cognitive, motor and social disorders' correction in children with ASD using ICT presented in the RSCI, PubMed and Google Scholar databases was carried out. The search was performed by the following keywords: autistic spectrum disorder, rehabilitation and habilitation, adapted physical education, information and communication technologies.</p><p><strong>Results: </strong>ICT in the form of technical teaching aids are used, including remotely, at classes of adapted physical education in patients with ASD to develop gross and fine motor skills, improve movements' coordination, enrich motor experience and increase physical activity, and from the side of higher mental functions - to develop memory, attention, speech and to improve perceptive processes. Arthrogenic muscle inhibition, immediately following injury in some cases, leads to quadriceps muscle hypotrophy and weakness, that further embarrasses the athlete's good-quality rehabilitation.</p><p><strong>Objective: </strong>To assess clinical and functional effectiveness of passive electrical myostimulation (EMS) as a muscle preoperative preparation to autotransplantation of knee anterior cruciate ligament.</p><p><strong>Material and methods: </strong>The study involved 47 male athletes with ACL injury. Summarized surface electromyography (EMG) was performed in all participants at admission, as a result of which 28 patients with reduced EMG parameters were selected. Among them, 14 patients received a course of passive EMS of quadriceps on physiotherapeutic <b>INTELECT Advanced</b> (Chattanooga (DJO), USA) hardware. All patients got passive EMS for 2 weeks after surgery and started active muscle training since 3rd week.
