Shishonin].","authors":"A Yu Shishonin, V I Pavlov","doi":"10.17116/kurort202510202121","DOIUrl":"https://doi.org/10.17116/kurort202510202121","url":null,"abstract":"<p><p>It is known that essential arterial hypertension (EAH) leads to structural-geometric and electrophysiological remodeling and these processes, following in parallel, do not always coincide. Thus, in patients with EAH, electrocardiographic (ECG) criteria of left ventricular hypertrophy (LVH) can occur without changing the mass of the left ventricular myocardium and its geometric characteristics. At the same time, they have independent diagnostic and prognostic value.</p><p><strong>Purpose of the study: </strong>Evaluation of the effect of manual-physical effects according to the method of A.Yu. Shishonin on the amplitude manifestations of electrophysiological myocardial remodeling in patients with EAH.</p><p><strong>Material and methods: </strong>We examined 148 patients with EAH with high normal blood pressure (BP), or arterial hypertension of the 1st degree, who were divided into the main and control groups. The main group included 114 patients (65 women and 49 men; mean age 61.8±5.2 years) with clinical manifestations of cervical osteochondrosis, who were treated with EAG using the method of A.Yu. Shishonin, including step-by-step manual-physical correction with the formation of a stable muscle corset in the cervical spine. The control group consisted of 34 patients (18 men and 16 women; mean age 61.6±4.0 years) who did not undergo treatment. To prove the effectiveness of the technique in reducing blood pressure and affecting the ECG pattern, instrumental diagnostic techniques for assessing blood pressure using the Korotkov auscultatory method and recording ECG criteria in 12 leads, respectively, were used.</p><p><strong>Results: </strong>All amplitude ECG criteria had significantly lower values after EAG correction using the method of A.Yu. When assessing the dynamics of the number of patients with amplitude criteria for LVH, a reliable decrease in this indicator was shown during therapy using the method of A.Yu. Shishonin: from 25.69 to 9.03% according to the Cornell criterion and from 26.39 to 8.33% according to the Sokolov-Lyon index. Considering that all patients did not have morphological signs of target organ damage, including criteria for concentric myocardial hypertrophy, the obtained results can be explained by effects at the tissue and cellular level.</p><p><strong>Conclusions: </strong>In the process of manual-physical correction using the method of A.Yu. The development of comprehensive rehabilitation for the people with disabilities, is at the intersection of social and medical fields, and it is classically adopting innovative methods of healthcare, AI technologies in particular. However, the use of such technologies for various targeted rehabilitation groups (TRGs) people with disabilities, taking into account the specifics of possible rehabilitation measures and services, has not been sufficiently studied for today. The aim of the work is to identify the existing, most widespread and successfully applied AI technologies by analyzing foreign publications on the use of AI technologies in the comprehensive rehabilitation of the people with disabilities. The paper analyzed foreign studies described in systematic reviews and randomized controlled trials (RCTs) in the PubMed international medical database over the past 5 years (from January 2019 to May 2024). AI technologies were considered from the point of view of application in the areas of complex rehabilitation for different TRGs. The concept of TRGs is new and has no analogues abroad, however, its use in the analysis allows to systematize information about AI technologies in terms of applied tasks of rehabilitation of the disabled. The largest number of studies on the use of AI technologies in rehabilitation relate to the restoration of motor functions of people with disabilities. An adequate correction is required in the case of vascular endothelial dysfunction in order to provide the control over possible complications after the surgery, that determines the study's relevance.</p><p><strong>Objective: </strong>To develop and scientifically substantiate non-pharmacological rehabilitation treatment technology of orthognathic patients after the surgery for the purposes of endothelial dysfunction inactivation and the risks of inflammatory odontogenic complications decrease by inclusion of low-intensity pulsed laser radiation of different wavelength (635 and 904 nm) in the basic pharmacological therapy program (anti-inflammatory, symptomatic drugs).</p><p><strong>Material and methods: </strong>A number of patients with orthognathic pathology equal 121 divided into 4 groups were under observation. Extraction and implantation (K04.5; K06.0) with exposure by low-intensity pulsed laser radiation of different wavelength (635 and 904 nm, pulse - 100 s, PFD - 25 mW/cm<sup>2</sup> on vestibular and oral surface in the projection of surgery area and epidermally in the projection of wound) immediately after the surgery were performed in the 1st group (31 patients); in the 2nd group (30 patients) - implantation due to anodontia (K00.0) with subsequent exposure by low-intensity pulsed laser radiation of 904 nm wavelength (pulse - 100 s, PFD - 25 mW/cm<sup>2</sup> epidermally in the projection of wound).</p><p><p>in the 3rd group (30 patients) - implantation (K05.3) with subsequent exposure by low-intensity pulsed laser radiation of 635 nm wavelength (pulse - 100 s, PFD - 25 mW/cm<sup>2</sup> on vestibular and oral surface in the projection of surgery area; in the 4th group (30 patients, control) - extraction and implantation (K04.5, K05.3) without laser exposure. {"title":"[Key components of the rehabilitation process of participants in a special military operation. Due to specific functional impairments and challenges associated with transitioning from a military to civilian environment, the rehabilitation of combat veterans has distinctive features.</p><p><strong>Objective: </strong>To identify the essential components of the rehabilitation process for individuals who have participated in a special military operation.</p><p><strong>Material and methods: </strong>A literature review was conducted using the eLIBRARY.RU database, following the PRISMA flowchart for systematic reviews and meta-analyses.</p><p><strong>Results: </strong>The categories and characteristics identified in the analytical review allow us to identify the main components of the rehabilitation of participants in a special military operation. These include the goal of rehabilitation for military personnel, the specific functions of specialists, psychological orientation of the process, interdisciplinary approach, versatility, and multidimensional, barrier-free practices. From the initial stages of medical rehabilitation, it is important to focus on an integrated approach that involves various specialists.</p><p><strong>Conclusion: </strong>The analysis allows us to understand the specific aspects of the rehabilitation of combatants and identify its main components that contribute to the understanding of this phenomenon. Studies indicate that patients who have undergone NCI are at risk of developing thrombotic disorders and problems with the cardiovascular system due to the presence of a low-grade inflammatory process (LGI). In this regard, there is a need to develop new methods and tools, including non-pharmacological approaches, to combat LGI.</p><p><strong>Objective: </strong>To evaluate the effect of grape polyphenols and magnetic therapy on the state of the LGI in patients with asthma who have undergone COVID-19.</p><p><strong>Material and methods: </strong>The study included patients with a verified diagnosis of asthma, as well as a history of NCI and with manifestations of postcovid syndrome living in the Republic of Crimea. Patients (<i>n</i>=50) according to indications, namely the presence of manifestations of postcovid syndrome, were referred for sanatorium treatment to the Department of Pulmonology of the Sechenov Institute in Yalta. The intervention group (<i>n</i>=20), in addition to standard sanatorium treatment, received a concentrate of polyphenols of Fenokor grapes produced by Resfood LLC, as well as a course of low-intensity magnetic therapy (MT) using a low-frequency magnetic therapy unit with frequency control of UMTvp-MADIN Lux (Magnetoturbotron), Russia. The second group was a control group, and consisted of 30 patients who were comparable in gender and age to the experimental group. ELISA kits manufactured by Cloud Clone Corp. were used to determine the level of C-reactive protein (CRP), zonulin, lipopolysaccharide (LPS), sCD14, LPS-binding protein, and bactericidal protein that increases bacterial permeability (BPI). (Wuhan, Hubei, China).</p><p><strong>Results: </strong>In patients of group 1, after a course of sanatorium treatment with the use of polyphenol concentrate and MT, a significant decrease in the level of CRP was revealed (<i>p</i><0.011). In both groups, a statistically significant increase in the concentration of BPI, sCD14 and zonulin was registered (<i>p</i><0.05). There were no significant changes in the level of CRP in group 2 (<i>p</i>>0.05).</p><p><strong>Conclusions: </strong>The use of grape polyphenol concentrates in combination with MT helps to reduce the level of C-reactive protein and increase the activity of components of LPS-binding systems in humans during the recovery period after COVID-19. {"title":"[Results of manual therapy in degenerative spine diseases with and without radiculopathy. We have studied the materials of clinical studies, meta-analyses and systematic reviews devoted to the evaluation of the results of manual therapy in patients with degenerative spine diseases, including those who underwent surgical treatment. The analysis (38 sources were analyzed, including 15 meta-analyses and 21 systematic reviews) revealed the ability of manual therapy to influence the level of acute and chronic pain (local and projection), duration of pain-free period, functional status of patients with degenerative spine diseases, including those with radiculopathy and after surgical treatment. There is medium-quality evidence (9 studies, 1176 patients) of a positive effect of manipulation and mobilization on vertebral pain levels and spinal motor activity in patients with chronic low back pain - manipulative therapy and mobilization compared with exercise or physical therapy statistically significantly reduced pain (SMD=-0.28, 95% CI -0.47 to -0.09, <i>p</i>=0.004; I 2 =57%). It was also observed that the effect was stronger at 3 months after intervention (370 patients SMD=-0.68, 95% CI -1.14 to -0.23; I2= 73.7%) and 6 months of follow-up (3 studies, 223 patients) (SMD=-0.72, 95% CI -0.99 to -0.45; I2= 0%). Medium-quality evidence suggests that myofascial release is more effective in controlling pain than control treatments (LFC, simulated osteopathic manipulative treatment, physiotherapy) (ES: -0.69; 95% CI: -1.05, -0.33; P=0.0002), even at follow-up after 12 weeks (ES: -0.73; 95% CI: -1.09, -0.37; P<0.0001). There is also medium-quality evidence (47 RCTs, 2678 individuals) of the efficacy of manual therapy for cervical and lumbar radiculopathy. In particular, mobilization of the nervous system resulted in a significant reduction in pain intensity (Hedges' g=-1.097, 95% CI=-1.482 to -0.712, <i>p</i><0.001, I2=85.338%). {"title":"[Prognostic model of a patient with sepsis readiness to early mobilization with assistance].","authors":"I A Ruslyakova, E Z Shamsutdinova, K I Shirokov, N E Ivanova","doi":"10.17116/kurort202510202126","DOIUrl":"10.17116/kurort202510202126","url":null,"abstract":"<p><p>Management of the early rehabilitation (passive, assisted, active) risks will allow to minimize complications and increase safety of patients with sepsis.</p><p><strong>Objective: </strong>To develop a prognostic model of patient with sepsis readiness to early mobilization with assistance.</p><p><strong>Material and methods: </strong>A single-center retrospective cohort observational study with analysis of 780 electronic medical records of patients with sepsis from the «Intensive therapy of patients with severe community-acquired pneumonia» base of the North-Western State Medical University named after I. I. Mechnikov in the period from 2013 to 2022 was carried out. Statistical analysis by binary logistic regression method and ROC-analysis were performed using Jamovi 2.3.21 and IBM SPSS Statistics 26 softwares package.</p><p><strong>Results: </strong>The study includes the data of 519 patients with sepsis. Patients are divided into 2 study groups according to the score on the Intensive Care Unit Mobility Scale (IMS): IMS 0/1 (281 patients) and IMS ≥2 (238 patients). The predictors of IMS ≥2 for patients with sepsis have been determined: score on the APACHE II assessment scale of patient's state severity ≤20.0 (adjusted odds ratio (AOR) 6.38; 95% CI 3.01-13.5; <i>p</i><0.001), score on the CCI Charlson comorbidity index ≤3.0 (AOR 3.84; 95% CI 1.72-8.58; <i>p</i>=0.001), score on the mNUTRIC critical state risk scale ≤5.0 (AOR 2.81; 95% CI 1.33-5.96; <i>p</i>=0.007), PO<sub>2</sub>/FiO<sub>2</sub> index >119.0 mm Hg (AOR 104; 95% CI 29.59-365.6; <i>p</i><0.001), albumin level >25.0 g/l (AOR 2.37; 95% CI 1.11-5.06; <i>p</i>=0.025). The prognostic model that considers 82.3% of factors determining the dispersion of IMS ≥2 probability has been developed. Area under ROC-curve amounted to 0.967±0.007 (95% CI 0.952-0.982). Sensitivity and specificity of IMS ≥2 prognostic model in patients with sepsis at 65% threshold were 92.0 and 92.6%, respectively, and accuracy - 92.3%.</p><p><strong>Conclusion: </strong>The prognostic model of patient with sepsis readiness to early mobilization with assistance has been developed based on IMS ≥2 independent predictors. (Literature review)].","authors":"E A Tenyaeva, E A Turova, V A Badtieva, I G Babeyan, N A Kostina","doi":"10.17116/kurort202510203162","DOIUrl":"https://doi.org/10.17116/kurort202510203162","url":null,"abstract":"<p><p>Amenorrhea of female athletes is a part of complex multifactorial syndrome - triad of sportswoman, components of which are eating disorders, menstrual disorders and low bone density, which are one of the manifestations of relative energy deficiency in sport (RED-S) syndrome. There is a lack of studies on this pathology In Russian literature, that suggests a low level of awareness among female athletes and coaches about the importance of correcting this pathology and its possible complications.</p><p><strong>Objective: </strong>To analyze the studies on the RED-S problem in sportswomen.</p><p><strong>Material and methods: </strong>A review of studies in the Pubmed and RSCI databases on the problem of triad of female athletes and athletic amenorrhea was carried out. We analyzed 52 sources of literature devoted to own studies and consensus agreements on the RED-S problem.</p><p><strong>Results: </strong>Correction of the sportswomen triad in its different manifestations should be performed by a multidisciplinary team, including gynecologist, endocrinologist, coach, dietician or nutritionist. The essential components of RED-S treatment are nutrition transition towards increased caloric value, correction of eating disorders, reduction of physical exertion, development of individual training programs, selection of certain exercises to increase muscle and bone mass and, in some cases, drug therapy strictly if medically required and with mandatory consideration of current standards of the anti-doping regulations.</p><p><strong>Conclusion: </strong>Timely diagnosis and willingness of female athlete and coach to comply with all medical recommendations lead to a favorable prognosis: it is possible to normalize mental health, improve endocrine profile and metabolism. {"title":"[Gender differences in vertebrobasilar hemodynamics in response to manual-physical impact in the treatment of essential arterial hypertension using the method of A.Yu. Shishonin].","authors":"A Yu Shishonin, V I Pavlov","doi":"10.17116/kurort20251020215","DOIUrl":"https://doi.org/10.17116/kurort20251020215","url":null,"abstract":"<p><p>Therapy methods for essential arterial hypertension (EAH) associated with the impact on the cervical spine and, in particular, step-by-step manual-physical correction with the formation of a stable muscular corset in the cervical spine according to the method of A.Yu. Shishonin, are used in representatives of both sexes.As a result, it is of interest to assess gender differences that can determine the features of the course, manifestations of pathology and response to the methods of treatment used.</p><p><strong>Objective: </strong>Study of changes in blood flow parameters in the vertebrobasilar basin under the influence of manual-physical correction according to the author's method of A.Yu. Shishonin.</p><p><strong>Material and methods: </strong>A total of 112 patients suffering from EAG were examined, including 49 men (aged 63.3±9.2 years) and 63 women (aged 62.1±8.3 years) with signs of clinically expressed degenerative-dystrophic processes in the cervical spine and arterial pressure (AP) corresponding to high normal AP (prehypertension), or EAG of the 1st degree. Methods of examination and treatment. All patients received the procedure of manual-physical correction according to the method of A.Yu. Shishonin. Timely correction of the developing pathological gait stereotype is an important task for a physician of physical and rehabilitation medicine. Identification of the central defect in the biomechanics of gait and targeted impact on it using various rehabilitation methods makes it possible to restore the gait function in a patient with post-stroke hemiparesis more effectively.</p><p><strong>Objective: </strong>To evaluate the effects of rehabilitation using author's technique of shifting the center of gravity of the foot on gait biomechanics in patients with post-stroke hemiparesis.</p><p><strong>Material and methods: </strong>This article studies a variant of rehabilitation effect on the pathological gait stereotype caused by the violation of the forward outreach of the paretic leg using author's technique of shifting the center of gravity of the foot developed on the basis of the Department of Nervous Diseases of the Prof. V.F. Voino-Yasenetsky Krasnoyarsk State Medical University. A pilot study was carried out with participation of 15 patients undergoing rehabilitation at the 2nd stage in the medical rehabilitation departments No. 2 and 3 of the Federal Siberian Research Clinical Centre under FMBA of Russia (FSRCC FMBA of Russia) in Krasnoyarsk. The patients underwent gait training sessions using technique of shifting the center of gravity of the foot according to the scheme of 10 daily sessions with the duration of 20-40 minutes per day. The function of gait was assessed using the ichnography method.</p><p><strong>Results: </strong>A comparison of gait function before and after rehabilitation was conducted using the Wilcoxon p-test. The median of the step length of the paretic leg before sessions was 34.31 cm, after sessions 44.84 cm (<i>p</i><0.01), the median of the step asymmetry, that is, the difference in the step length of the paretic and intact leg, before sessions, was 11.77 cm, after sessions 6.35 cm (<i>p</i><0.01).
