{"title":"[The use of oscillatory respiratory therapy with positive expiratory pressure (PEP-therapy) to restore the functional state of the lungs in patients after cardiac surgery].","authors":"A A Eremenko, T P Zyulyaeva, A P Alferova, D V Fomina, M S Grekova, O O Grin, S S Dmitrieva, A V Molochkov, A P Gens, K V Kotenko","doi":"10.17116/kurort202310006121","DOIUrl":"10.17116/kurort202310006121","url":null,"abstract":"<p><p>Postoperative pulmonary complications in cardiac surgery patients occur in 10-35% of cases, depending on differences in their definition, patient characteristics and type of surgical intervention, most of them are associated with ineffective coughing and evacuation of bronchial secretions.</p><p><strong>Objective: </strong>To determine the effectiveness of stimulating the evacuation of bronchial secretions with the help of oscillating PEP therapy carried out during the first three days.</p><p><strong>Material and methods: </strong>A randomized prospective study of 60 adult patients after elective cardiac surgery was performed (Clinical Trials.gov. protocol number NCT05159401). Oscillatory PEP-therapy was performed in 30 patients using Acapella DHGreen device (SmithMedicalASD, USA) 10-12 hours after tracheal extubation 3 times a day for 3 days after surgery. The control group (30 patients). The inclusion criteria: age over 18 years, spontaneous breathing after tracheal extubation, clear consciousness and productive contact with the patient, the ability to maintain adequate gas exchange on the low-flow oxygen inhalation, adequate analgesia (<2 points of VAS). Exclusion criteria: the need for re-intubation and mechanical ventilation, non-invasive mask ventilation, high-flow oxygen therapy, acute cerebrovascular accident, ongoing bleeding, cardiac insufficiency (inotropic index >10), shocks syndrome of various etiologies, the use of any extracorporeal support, any neuromuscular disorders, pneumothorax, hydro-or hemothorax. Before each session and 20 minutes after its end, when breathing air, blood oxygen saturation was recorded using a pulse oximeter (SpO<sub>2</sub>), the maximum inspiratory capacity (MIC) was measured using a Coach-2 incentive spirometer from SmithsMedical and spirometry with a portable ultrasonic spirometer Spiro Scout (Schiller, Switzerland). For the purposes of this work, the total index of the spirometry maximum inspiratory capacity (SMIC) was used - the sum of the respiratory volume and the reserve volume of inspiration in ml.</p><p><strong>Results: </strong>Difficulties in evacuation of sputum were noted in 90% of patients. Three-day sessions of oscillating PEP- therapy are accompanied by a significant improvement in the passage of sputum, as evidenced by a 3-fold increase in the number of patients with productive cough. The increase in MIC in the main group was 46.9% and 21.3%, respectively (<i>p</i>=0.042), and the number of patients with values greater than MICo. 1500 ml increased from 23.3% to 7.6% (<i>p</i><0.001). The effectiveness of oscillatory PEP-therapy is confirmed by a 7-fold decrease in the frequency of radiological changes in the lungs at the end of sessions (<i>p</i><0.001), while in the control group the frequency of their occurrence practically did not change and remained at a high level. {"title":"[Alopecia and COVID-19: possible etiopathogenetic variants and therapeutic approach].","authors":"I A Gritskova, I G Ponomarenko, I V Cherkashina","doi":"10.17116/kurort202310005156","DOIUrl":"10.17116/kurort202310005156","url":null,"abstract":"<p><p>Sudden hair thinning, phantom trichalgia in the early and late rehabilitation period after novel coronavirus infection (COVID-19) are the most common complaints of patients, that can be considered by both dermatocosmetologist and medical rehabilitation specialist. A telogen hair loss was found in 19.8% of patients, whereby 27.3% of patients suffering from hair loss during disease and 72.7% - at 3rd-6th month after recovery. Most commonly, hair loss is non-structural and associated with an abnormal ovulatory cycle shift and diffuse asynchronous loss of hair follicles in telogen phase, as well as with an increase of total predisposed to loss hair follicles number. Nevertheless, the analysis of clinical observations of patients with post-COVID hair loss has shown that this disorder is registered not only in telogen phase. There is a rapid disease progression up to the final stages in the presence of verified androgenetic alopecia diagnosis. The cases of alopecia areata and cicatricial alopecia, associated with previous COVID-19, also were registered. Androgenetic alopecia is the first (30.7%) and diffuse alopecia is the second (19.8%) by degree of incidence. The relapses or much less frequently the onsets of alopecia areata and the unexplained pronounced pain at the hair roots in parietal region (7.8%) are in the third place. Robotic systems are new devices which are becoming increasingly popular as a part of the treatment of cerebral palsy. They have become a good addition to comprehensive rehabilitation therapy, including conducted at the sanatorium-resort stage. The most common complication of a stroke is a violation of the motor function of the limbs, which significantly worsens the quality of life and the level of self-care and independence of patients. Restoring the function of the upper limb is one of the priority tasks of rehabilitation after a stroke. A large number of factors, such as the location and size of the primary brain lesion, the presence of complications in the form of spasticity, impaired skin and proprioceptive sensitivity, and comorbidities, determine the patient's rehabilitation potential and the prognosis of ongoing rehabilitation measures. Of particular note are the timing of the start of rehabilitation measures, the duration and regularity of the treatment methods. A number of authors propose scales for assessing the rehabilitation prognosis, as well as algorithms for compiling rehabilitation programs for restoring the function of the upper limb. A fairly large number of rehabilitation methods and their combinations have been proposed, including special methods of kinesitherapy, robotic mechanotherapy with biofeedback, the use of physiotherapeutic factors, manual and reflex effects, as well as ready-made programs that include sequential and combined use of various methods. Dozens of studies have been devoted to comparative analysis and evaluation of the effectiveness of these methods. {"title":"[Robotic mechanotherapy in a complex of procedures for the functional state recovery of the neuromuscular system and peripheral hemodynamics in track-and-field athletes].","authors":"A Sh Abutalimov,&nbsp;Yu V Koryagina,&nbsp;S M Abutalimova,&nbsp;G N Ter-Akopov,&nbsp;S V Nopin","doi":"10.17116/kurort202310003139","DOIUrl":"https://doi.org/10.17116/kurort202310003139","url":null,"abstract":"<p><p>The background of the study is caused by the need to develop methods of drug-free neuromuscular system and peripheral hemodynamics recovery of athletes to optimize their functioning after intense physical activity in current conditions of high sporting competition.</p><p><strong>Objective: </strong>To develop a program of comprehensive recovery of neuromuscular apparatus and hemodynamics of the lower limbs of track-and-field athletes during intense physical activity, including mechanotherapy on the robotic biomechanical complex with biological feedback and to evaluate its efficacy, comparing with a standard recovery program.</p><p><strong>Material and methods: </strong>The study involved 23 track-and-field athletes (the mean age 24.6±3.8 years) with qualification «master of sports» and «master of sports of international class». The athletes were randomly divided into two groups (study and control). In the study group athletes received hydro-, presso- and magnetotherapy procedures, as well as mechanotherapy procedures on a robotic biomechanical complex with biological feedback. The athletes from control group received only traditional rehabilitation with hydro-, press- and magnetotherapy. The examination of the functional state of the neuromuscular apparatus and peripheral hemodynamics was performed with the use of stimulation electroneuromyography, robotic dynamometry and rheovasography.</p><p><strong>Results: </strong>When registering a motor response from a foot extensor digitorum brevis, innervated by the deep fibular nerve, after conduction of procedures set, a decrease in the residual latency parameters of study group's athletes was found. The dynamometric investigation showed a decrease in the fatigue coefficient of knee joint's flexors and extensors, as well as an increase in the strength of knee joint's extensors in the athletes from the study group. During the rheovasography the decrease of the rheographic index in the «foot» and «lower leg» segments in the study group was revealed. The decrease of the geographic index in the «lower leg» segment and normalization of the rheographic waves distribution time in the «foot» segment in control group were identified.</p><p><strong>Conclusion: </strong>The study results showed the efficacy of both the standard recovery program for athletes and the program, supplemented by mechanotherapy. Antenatal ultrasound can detect obstructive uropathy in infants. It is important to conduct diagnostics not only to identify the obstruction level in urinary system, but to assess renal function, renal blood flow and urination. Children are given conservative and surgical treatment methods to restore urodynamics, prevent infectious complications, improve renal blood flow. Currently, there are no principles, approaches and technologies for medical rehabilitation of patients with obstructive uropathy, therefore, the use of selective chromotherapy, which has an activating effect on regional circulation, and sound stimulation improving muscles tone of pelvis and ureters, is pathogenetically reasonable and promising.</p><p><strong>Objective: </strong>To develop technologies of physiotherapy application (sound stimulation, selective chromotherapy) for inclusion in comprehensive medical rehabilitation of children with megaloureter.</p><p><strong>Material and methods: </strong>Clinical observations and special examinations have been performed in 90 children with megaloureter aged from 1 to 10 years. The patients were divided into 2 groups: 30 children (study group) received sound stimulation combined with selective chromotherapy; 30 children (the 1st comparison group) - sound stimulation; 30 children (the 2nd comparison group) - selective chromotherapy (blue spectrum). General clinical methods, ultrasound of kidneys and bladder with Doppler monitoring of intrarenal blood flow, functional methods of bladder examination were used.</p><p><strong>Results: </strong>The positive effects of separate and combined application of sound stimulation and selective chromotherapy on clinical and laboratory indicators, urodynamics of urinary tract and renal blood flow in children with megaloureter after surgery have been revealed. The studies were conducted in Kislovodsk in the middle altitude (1240 meters) on the Maloe Sedlo mountain, in the conditions of training camps. We studied the parameters of heart rate variability and functional state of cardiovascular and respiratory systems. Hyperoxic inhalations were performed with the JAY-10 oxygen concentrator (LONGFIAN SCITECH CO., LTD., China). The oxygen concentration in the mixture was 93±3%, capacity - 5 l/min, exposure time - 20 min.</p><p><strong>Results: </strong>Athletes specializing in sports with predominantly anaerobic lactate and mixed mechanism of energy supply, respectively experiencing hypoxia in the process of their athletic activity during training can experience hypoxia at rest as well. The state of hypoxia during exercise in the middle altitude in such athletes is shown in heart rate variability indices (high values of heart rate and stress index, low power of high frequency waves), low saturation, increased oxygen uptake from the blood microcirculation system. Cognitive and autonomic interactions formed with the help of biofeedback training, changing the function of autonomic and visceral systems, qualitatively improve the efficiency of any activity, including athletic activity, the nature of which is predetermined by the type of sports.</p><p><strong>Objective: </strong>To identify effects of the beta-stimulating training in recovery of adaptive reserves in athletes of different sports.</p><p><strong>Material and methods: </strong>The study included young male athletes aged 18-22 years (<i>n</i>=1020), distributed according to the type of sports: group 1 - cyclic sports (<i>n</i>=387); group 2 - speed-strength sports (<i>n</i>=255); group 3 - single combat (<i>n</i>=31); group 4 - team sports (<i>n</i>=173); group 5 - complex-coordination sports (<i>n</i>=174). The main objective of the beta-stimulating training was to select a strategy to raise the beta rhythm level while muscles were relaxed. For each group, we identified training efficiency periods according to the lowest value of the attention index (θ/β, c.u.), then we examined distribution of brain rhythms (α, %; β, %; θ, %) according to EEG. The level of adaptive reserves was identified according to the heart rate variability (HRV) spectral analysis («VNS-Spektr», Neurosoft, Ivanovo).</p><p><strong>Results: </strong>We identified following periods of the beta-stimulating training efficiency according to the attention index (θ/β, c.u.): group 1 - 10<sup>th</sup> session (game variant); group 2 - 5<sup>th</sup> session (graphic and game variants); group 3 - 1<sup>st</sup> and 10<sup>th</sup> sessions (game variant), 5<sup>th</sup> session (graphic variant); group 4 (game variant) - 1<sup>st</sup>, 5<sup>th</sup> and 10<sup>th</sup> sessions; group 5 (game variant) - 5<sup>th</sup> and 10<sup>th</sup> sessions. The efficiency periods in the observed groups were accompanied by the following distribution of rhythms in the EEG structure: β-rhythm - 45-60%; θ-rhythm - 20-35%, α-rhythm - 19-25%. During these periods, wave structure of the heart rhythm also changed: the activity of VLF waves increased (%); the proportion of HF waves decreased in the range of 23.3-31.59% in the groups 1, 2 and 3; the share of HF waves increased in the range of 32.3-39.8% in the group 4 at the 1<sup>st</sup> session and in the group 5 at the 10<sup>th</sup> session. The values of total power (TP, ms<sup>2</sup>) reduce at the 10<sup>th</sup> session in the groups 1, 3 and 4 and at the 5<sup>th</sup> session in the group 5. This review represents current approaches to the diagnosis of sarcopenia with reference to recent European and Asian consensus. They include rules for the evaluation of the main muscle strength and function tests (hand dynamometry, sit-to-stand test, 6-minute walk test, physical performance battery tests, etc.), physical and instrumental methods of muscle mass analysis (densitometry, bioimpedance analysis, magnetic resonance imaging). Furthermore, the pathogenetic relationship between the lack of physical activity and muscle dysfunction in elderly people is broached, including in particular the role of myostatin, interleukin-6, somatotropin and insulin resistance.
