Nous aborderons ici la voie antérieure en prenant garde à éviter le paquet vasculonerveux circonflexe, ainsi que la voie latérale moins utilisée mais très utile dans certains cas. La ponction échoguidée de hanche est très utile pour le diagnostic d’épanchement (mécanique ou inflammatoire), l’infiltration de corticoïde ou d’autres produits peut permettre de soulager le patient mais avec de faibles preuves scientifiques.</div></div><div><div>Ultrasound-guided hip punctures and injections are now common practice in rheumatology. We will discuss here the anterior path, taking care to avoid the lateral circumflex femoral artery and nerve, as well as the lateral path which is less used but very useful in certain cases. {"title":"Rôle possible de la carence en glial derived growth factor (GDNF) dans la pathogénie des syndromes douloureux régionaux complexes","authors":"Jean-Marie Berthelot , Paul Arnolfo , Julien Nizard , Benoît Le Goff","doi":"10.1016/j.rhum.2024.05.006","DOIUrl":"10.1016/j.rhum.2024.05.006","url":null,"abstract":"<div><div>Une carence en <em>glial derived nerve factor</em> (GDNF) pourrait être, avec une sensibilité accrue au <em>calcitonin gene related peptide</em> (CGRP) libéré par des fibres peptidergiques stressées par la mort des plus fragiles autres petites fibres nociceptives (IB4), et la pré-activation de cellules gliales dans la moelle, un co-facteur majeur de la pathogénie des <em>complex regional pain syndrome</em> (CRPS). Un défaut de sécrétion de GDNF (en particulier par les cellules de Schwann/Remak autour des petites fibres nociceptives) pourrait en effet : faciliter l’apoptose des fibres IB4 (isolectine B4) ; retarder leur repousse ; contribuer à la sidération des fibres sympathiques adrénergiques notée à la phase chaude des CRPS ; cadrer avec l’absence usuelle de développement de CRPS lors des paralysies périphériques, durant lesquelles du GDNF est libéré en quantité ; contribuer à expliquer pourquoi seulement certains traumas articulaires sont suivis de CRPS ; faciliter une surréaction des cellules satellites, et leur synthèse de NGF, suite à la mort de fibres IB4 ; renforcer l’activation des cellules gliales dans la moelle épinière ; favoriser le <em>sprouting</em> dans la moelle épinière de dendrites des nerfs périphériques, dont peptidergiques ; participer à l’inflammation préalable de la moelle épinière, en facilitant la migration d’antigènes à partir de la muqueuse intestinale ; favoriser des troubles proprioceptifs du membre touché ; majorer la perte osseuse dans l’os sous-chondral ; contribuer à la dépilation. Comme stases et dépressions, deux co-facteurs favorisants des CRPS, sont associés à des taux bas de GDNF, lequel n’a pu être retrouvé dans la pièce d’exérèse d’un index de CRPS, un manque en GDNF paraît contribuer aux CRPS.</div></div><div><div>Besides preactivation of spinal cord glial cells, and greater sensitivity to the calcitonin gene related peptide (CGRP) released in excess by small peptidergic nerves (following the release of NGF and IL-1β by satellite cells reacting to the extensive death of the fragile IB4 nerves), glial derived nerve factor (GDNF) deficiency might contribute to complex regional pain syndrome (CRPS).
