Electrogastrography and capsule endoscopy are contributing in the diagnosis of Gastric diseases. On their individual functioning, as a diagnostic tool, they are not capable of producing appropriate results such as the position of tumor in the GI tract. The recording of electrical rhythmic activity can mimic the disability of gastric cells which can’t be diagnosed by endoscopic procedures. Integrating the gastric rhythmic information and capsule endoscopy can lead to an ultimate diagnosis of gastric diseases. The structural information which is captured by a capsule consisting of a camera, LEDs and transmitter provides a visual diagnosis of the gastric disorders/infection. The functional information is obtained by recording the electrical rhythmic activity of the gastric cell via surface electrodes. This paper reports a case of a 32 years old type 1 diabetic female patient with Gastroparesis due to the disease. This patient had a severe abdominal pain and vomiting for 3 weeks. She had already diagnosed with diabetic gastroparesis and usually took metoclopramide and omeprazole. Approximately 100 cases of double cecal appendix have been described so far. Till now there is no report of duplex appendix serving as a lead point in a patient with intussusceptions. This case report presents the first case of such presentation for the first time. This 6 month old female child presented with vomiting and diarrhea of week duration. She was tachycaridic, tachypniec and febrile. On abdominal examination she had distended and tender abdomen with hyper tympanic sound on percussion. She was irritable and had a sunken eye ball. She had leaukocytosis (20.8 × 103), hypokalemia (3.3 mmol/l) and hyponatremia (125 mmol/l). Abdominal ultrasounds were suggestive of intussusception. After stabilizing the patient we operated her for possible pre-operative diagnosis of SBO secondary to Intussusception. Intra-operatively it was an Ileocolocolic intussusception with duplex appendix as a leading point. We did resection and anastomosis. Post operatively she was transfused with two units of blood (20 ml/kg), given IV antibiotics, analgesics and followed with vital sign sheet (Table 1). She started PO feeding after 36 hours post operation and discharged on the 7th day improved. The case is extremely important for the study of this possible anatomical variation and its association with intussusceptions. It also serves as an input for future studies in the area. Moreover, its association with the presence of other anatomical variations such as intestinal, genitourinary and bone is important. Wen","doi":"10.4172/2161-069X.1000536","DOIUrl":"https://doi.org/10.4172/2161-069X.1000536","url":null,"abstract":"Objective: The aim of this study was to report and analyze outcomes associated twenty-eight patients’ in-hospital evaluation of the computed tomography (CT) and magnetic resonance imaging (MRI) features of solitary fibrous tumors (SFTs) in various anatomic regions.Methods: A number of twenty-eight patients whose clinical data, imaging finding, pathological and immune histochemical findings and with proven SFTs were retrospectively reviewed of the first affiliated hospital of Anhui medical university of January 2012 to 2015 were identified and included for analysis. Twenty-one patients (17 with contrast) had undergone CT scan while 7 (4 with contrast) had undergone MRI. Image characteristics and pattern of enhancement on CT and MRI were analyzed and compared with histopathological findings and multivariate analysis.Results: Among 28 patients, 12 SFTs occurred in the thoracic cavity, 4 in the mediastinum, 3 in the retroperitoneum, 2 in the meanings while a single SFT was found in other 7 organs. They appeared as a solitary mass with an oval or irregular shape, well-defined or ill-defined margin. The size ranged from 1.5 ×1.0 cm2 to 23.0 × 16.5 cm2. Plain CT revealed homogeneous iso-density in 10 and heterogeneous iso-density in 11 cases. Out of the 17 contrast enhanced cases, 11 cases showed slight enhancement and 6 showed marked heterogeneous enhancements during the arterial phase (AP). Progressive and prolonged enhancement was observed in the dynamic contrasted scan. Most of tumors (14/17) exhibited serpentine vessel sign in AP. Hypo- or iso-intensity on T1WI and hyper-or isointensity onT2WI were revealed on MRI. Six cases (6/7) showed the flow voids sign on T2WI.Conclusion: SFTs typically present as large, well-defined, heterogeneous, progressively enhancing mass in pleural and extra-pleural regions. The etiology and pathogenesis of rectal SCC is poorly understood. Given the rare occurrence of this type of malignancy, there are a number of diagnostic and therapeutic dilemmas. We are reporting a case of a 56-year-old female, who presented with abdominal pain and found to have rectal mass which was subsequently biopsied under endoscopic ultrasound guidance. Histopathology showed primary submucosal squamous cell carcinoma. She was treated with radiotherapy and systemic chemotherapy. Additionally, removal of \u0000 large stones may be challenging when using EBD alone. \u0000 Patients and methods: 50 patients with calculary obstructive jaundice was enrolled in our randomized \u0000 comparative study, ES was done for have of them and EBD for the rest. During ERCP, stone removal was declared \u0000 as complete if the final cholangiogram showed no residual stones. Clinical evaluation for post ERCP pancreatitis \u0000 was performed on the following day by symptoms and serum amylase. \u0000 Results: There is no statistical significant difference between the two groups, as regard, procedure duration, \u0000 cannulation trials and time. Success rate was 88% and 80 after ES and EBD respectively. Significant higher rates of \u0000 endoscopic bleeding were detected with ES. Apart from significant higher rates of post-ERCP bleeding after ES, no \u0000 difference was detected between the 2 groups at regard post-ERCP complications. \u0000 Conclusion: The efficacy of EBD is similar to ES regarding, removal of common bile duct stones, and it can be \u0000 safely applied particularly in patients with systemic coagulopathy as it carries a lower rate of bleeding. Castell","doi":"10.4172/2161-069X.1000530","DOIUrl":"https://doi.org/10.4172/2161-069X.1000530","url":null,"abstract":"Alginates have been shown not only to be beneficial in their relief of symptomatic post-prandial gastroesophageal reflux, but there is also evidence that they may inhibit reflux, including bile acids and pepsin, and in turn attenuate and even prevent the activation and up-regulation of molecules associated with the development of metaplasia and cancer of the esophagus. Here we present the case of a patient with Barrett's esophagus with high-grade dysplasia (HGD) which was initially unresponsive to endoscopic radiofrequency ablation therapy and cryotherapy for highgrade dysplasia. Following initiation of therapy with an alginate solution in addition to twice daily proton pump inhibitor (PPI) therapy, the patient showed a favorable response and eventual complete eradication of his dysplastic Barrett's. The patient's course is suggestive that the alginate solution, through reflux inhibition, prevented persistent esophageal cell changes which occur secondary to gastric and bile acid exposure, thus allowing development of neosquamous epithelium. Kajiyama","doi":"10.4172/2161-069X.1000531","DOIUrl":"https://doi.org/10.4172/2161-069X.1000531","url":null,"abstract":"Objective: We evaluated various cytokines in patients with esophageal cancers to elucidate the relationship between the Th1/Th2 balance and the patients’ prognosis. \u0000Materials and Methods: Various serum cytokines of preoperative 98 patients, who subsequently underwent radical esophagectomy between September 2005 and December 2006, were quantitatively measured by Cytometric Bead Array (CBA) system. The cytokines include Th1/Th2 balance related 14 cytokines-IL-1α, IL-1β, IL-2, IL-4, IL-5, IL-6, IL-7, IL-8, IL-10, IL-12p70, IL-13, IL-17A, INFγ and IgE. Multivariable analysis was performed using the Cox proportional hazard model. The survival rates based on CRP values were analyzed by the Kaplan-Meier method. Eight clinical pathological factors of the tumors, which are regarded as relevant prognostic factors: diameter, histological type, invasion type, vascular invasion, venous invasion, depth of the invation (T factor), regional lymph nodal status (N factor) and intramural metastasis (IM) and the 14 cytokines were evaluated by the multivariable analysis. \u0000Results: The multivariable analysis showed that T factors (p=0.001), N factors (p=0.002), IL-1β (p=0.003) and IL-6 (p=0.021) were significantly related to the worse prognosis. The two cytokines are known as inflammatory cytokines and they theoretically induce the production of acute inflammatory proteins, including C-reactive protein (CRP). We hereby evaluated the preoperative serum CRP values of the same patients. By means of ROC analysis (Youden index), the cut off value of the serum CRP was determined as 0.25 mg/dl and the survival rates were analyzed by the Kaplan-Meier method. He was followed up under the tentative diagnosis of Brunner gland (BG) hyperplasia (BGH) by biopsy. The tumor was found to have turned inverted dome-shaped with a central depression occupying almost all the top of it 2 years later. The disrupted surface was uneven, more reddened and lobulated by groove-like excavations, in and around which the mucosal pattern was obscured and abnormal vessels were observed. The biopsied specimen was interpreted as BG adenoma (BGA). The tumor was endoscopically resected because of a risk of complicating malignancy, judged to be completely excised, and definitely diagnosed as BGA containing no malignancy by the intensive examinations.
