Screening and Treatment for Helicobacter pylori Infection in Teenagers inJapan 日本青少年幽门螺杆菌感染的筛查和治疗

Suga","doi":"10.4172/2161-069X.1000454","DOIUrl":"https://doi.org/10.4172/2161-069X.1000454","url":null,"abstract":"To elucidate the prevalence and effect of H. pylori infection in Japanese teenagers, we underwent an examination and treatment of it in one high school health screening between 2007 and 2015. The study subjects were students ages 16 to 17. Students who tested positive on this screening using urine-based rapid test kits (RUPIRAN®) examination visited Shinshu University Hospital and underwent esophagogastroduodenoscopy (EGD) and were taken biopsy samples to determine their H. pylori status using culture and histology. Cure of H. pylori infections was determined by urea breath test. For 9 years, 4,297 of 4,312 students (99.7%) received a screening examination for H. pylori infection. One hundred and sixty-two of 4,297 students (3.8%) were positive for H. pylori. Ninety-three of these 162 with H. pylori-positive students visited our hospital, and 91 underwent EGD and 78 (85.7%) were confirmed to be H. pylori infected. The most common endoscopic findings for H. pylori infection were nodular gastritis (80.8%) and closed-type atrophic gastritis (61.5%). A scar from duodenal ulcer was recognized in 6 of them (7.7%), and intestinal metaplasia was histologically present in two. All 78 students with H. pylori infection and their parents agreed to receive eradication therapy using regimens according to the susceptibility of H. pylori. Finally, all except 3 were successfully cured of H. pylori infection. Remaining 3 students have not yet been assessed the decision of H. pylori infection. If this procedure were to be introduced into nationwide health screening at Japanese high schools, we calculated that the cost of the prevention of a gastric cancer would be 495,958 yen (4,508.71dollars) for each person. We here report a 63 years-old male with multiple gastric and concomitant other organ metastasis of RCC 5 years after left radical nephrectomy. He presented with iron deficiency anemia without any gastrointestinal symptom. Radyological imagings showed multipl polypoid masses in the stomach and metastatic tumors in pancreas, lung and bilateral adrenal glands. Esophagogastroduodenoscopy showed multiple gastric “volcano-like” polypoid masses in the stomach. Material and method: Upper gastrointestinal system endoscopy and gastric emptying scintigraphy were performed on the same day in 51 volunteer type 2 Diabetes Mellitus patients. 4-hour gastric emptying scintigraphy with 500 mCi Technetium (Tc) 99 m diethylene triamine pentaacetic acid (DTPA) in semisolid foods for the determination of gastroparesis was performed in the patients. Upper gastrointestinal system endoscopy was compared between patients with and without gastroparesis by gastric emptying scintigraphy. Results: The mean ages of patients with diabetic gastroparesis (20 Female (F)/10 Male (M)) and without (12F/9M) diabetic gastroparesis were 57.04 ± 9.41 and 53.35 ± 6.98 years, respectively. There was no statistically significant difference between groups in terms of age and gender distribution (p=0.192, p=0.489, respectively). There were no significant differences with respect to age, gender, body mass index (BMI), fasting blood glucose (FBG) and Hb A1c between groups (p=0.192, p=0.489, p=0.529, p=0.653, and p=0.707, respectively). Erosive (E) findings were more frequent in patients with (22 E/8 Nonerosive (NE)) than without (1 E/20 NE) diabetic gastroparesis, and the difference was statistically significant (p<0.05). Conclusion: Erosive findings were significantly more frequent in patients with than in those without diabetic gastroparesis. Methods: Rectal cancers were grouped according to Lauren’s classification (UICC I-IV stages); overall survival (OS) and progression free survival (PFS). 25 patients were in UICC III/IV stages, while 19 patients were in UICC I/II stages. VEGF-A/B/C/D, PDGF-A, EGF, IGF-1/2, and corresponding receptor subtypes 1/2 mRNA enrichment was determined using quantitative real-time PCR. Data were correlated to clinical outcome. Results: The obtained results show that VEGF-B and IGF-2 mRNA were significantly higher expressed in rectal adenocarcinoma than adjacent normal tissues, while other GF genes were not significantly affected. IGF-2 levels were further upregulated in advanced stages UICC III/IV where patients received radiotherapy. Patients with good response to neoadjuvant radiotherapy (as determined by the histological Dworak regression score) had significant better overall survival. Expression of microRNA miR-483 (a known microRNA molecule previously associated with IGF-2 expression) was evaluated by TaqMAN PCR, but no significant correlation to IGF-2 levels was detected. Conclusions: IGF-2 appears as a biomarker of rectal adenocarcinoma, but does not serve as predictive parameter for tumor progression or survival. The results encourage further development of biomarkers targeting IGF-2 molecules in blood or urine. We analyzed the change in the prevalence of colorectal tumors in elderly patients (80 years or older) over a decade. Method: A total of 10,516 patients who were admitted to our hospital for colonoscopic examinations from 2009 to 2011 (recent period) were analyzed and compared with 8,767 historical cases from 1999 to 2001 (prior period). The proportion of patients who were elderly, reason for colonoscopy, completion rate and detection of tumors were analyzed in each period. Results: The rate of elderly patients significantly increased from 2.86% in the prior period to 5.40% in the recent period (p<0.001). As to the reasons for colonoscopy, a double-contrast barium enema X-ray study suggestive of colorectal carcinoma significantly decreased from 6.4% to 1.1% (p<0.001) and screening studies in the elderly significantly increased from 12.7% to 25.2% (p<0.001). The completion rate in the elderly improved (prior period 87.2%, recent period 93.59%, p=0.005). The detection of colorectal tumors was significantly higher in elderly (47.5%) than in non-elderly patients (37.3%, p<0.001) and the proportion of elderly with colorectal adenoma significantly increased from 2.95% in the prior period to 6.54% in the recent period (p<0.001). However, the detection rate of tumors in the elderly significantly decreased over a decade (prior period 54.2%, recent period 47.5%, p<0.001). Conclusions: The proportion of the elderly population is increasing with increasing frequency of colonoscopic examinations performed. During our review we found 22 cases that are reported in the last decade. The difficulty to differentiate GE from Emphysematous gastritis – which entails a more malignant course, poses a challenge to the clinicians in management. To our knowledge this is the first case of gastric emphysema presenting with chest pain and vomiting. We present a case of a diabetic woman whom presented with chest pain found vomiting and admitted initially as a cardiac case. Later on her admission she was found to have gastric emphysema. Our aim to compare the estimated viable HCC after locoregional therapy (LRT) by CT imaging and prior to liver transplant to the histopathological assessment of viable HCC in the hepatic explant. Methods: We prospectively evaluated 44 patients with HCC who underwent both LRT and liver transplantation at London health science center. Using mRECIST criteria, the response to LRT was assessed by two blinded radiologists and the percentage of necrosis was reported separately for the reference CT (rCT) done after the last LRT and prior to liver transplantation. The report obtained from each radiologist was combined then was compared to the findings of an expert pathologist reporting on viable tumour present and tumour necrosis in the hepatic explants. Both parties were blinded to prevent bias in the results. Results: A total of forty-one transplant recipients fulfilled the inclusion criteria for the study. At time of listing 100% were within total volume criteria, 86% within UCSF, and 68% within Milan. The average time from the last reference CT scan to liver transplant was 57.7 days; the average time from last LRT to reference CT was 72.5 days. Thirty-four recipients (83%) had accurate assessment for necrosis (mRECIST) within 20% comparing rCT to explant (i.e. concordant). Ninteen, 19 (46%) of the 41 predicted 100% concordance. Only 7/41 (17%) had a poor concordance (>50%) between histology and reference CT images. positive correlation was detected with the correlation coefficient is calculated as 0.5723. Our study demonstrated CT-pathologic correlation in predicting number and size of tumors with Correlation coefficient 0.64 and 0.31, respectively. However, there was poor correlation in predicting total volume with correlation coefficient is calculated as 0.014. Conclusion: Dynamic CT is an accurate tool to evaluate the tumour response prior to liver transplantation and the likelihood of underestimating the tumour burden is low. Liver biopsy remains the gold standard for diagnosis of NASH. A scoring system predictive of NASH based on these biomarkers would prove indispensable for avoiding the invasive liver biopsy. The estimated prevalence of pediatric NAFLD is 2.6%-9.6%. Studies estimate that the prevalence of NAFLD among overweight or obese children and adolescents is 24%-77%. Serum biomarkers to diagnose NASH may be divided into four categories: Markers of Inflammation, oxidative stress, apoptosis and fibrosis. TNF-α mRNA cutoff value of 100 ng/mL predicted NASH with a sensitivity of 66.7%, specificity 74.1%, Serum adiponectin, levels are lower in patients with NASH but no definite level for accurate prediction has been determined. Serum CK-18 is an independent predictor of NASH. A cut off value of 380 U/l provided a specificity of 94% and sensitivity of 91%. Ox-LDL and TBARS in serum were higher in biopsy-proven NASH patients. Apart from these a panel comprising of hyaluronic acid (HA), amino-terminal propeptide of type III collagen (PIIINP), and tissue inhibitor of metalloproteinase 1 (TIMP-1) is under evaluation as a predictor of fibrosis. Liver biopsy, considered as the gold standard to diagnose NASH is invasive and costly. Some of the Noninvasive Biomarkers have an established role in the diagnosis of NASH whereas the role of some needs further evaluation.
