Immunosuppressive treatment is essential throughout the entire functioning period of the transplanted kidney to prevent graft rejection. Aphthous stomatitis with painful ulcerations is a common complication among kidney transplant recipients. A rare cause of this condition is the adverse effect of mycophenolate mofetil, which is part of the most common stable triple combination of immunosuppressive drugs. This article presents a case of multiple painful oral ulcerations induced by mycophenolate mofetil in a 4-year-old boy following a related donor kidney transplant. However, with the wide availability of over-the-counter medications, self-treatment is becoming increasingly relevant. This review article summarizes the most frequent vaginal complaints - vaginal candidiasis, bacterial vaginosis, aerobic vaginitis, mucosal dryness, and genital herpes - and outlines treatment options available within the scope of pharmaceutical care. It highlights the pharmacist's role in educating patients, recommending appropriate therapies, and identifying cases that require medical evaluation. This is beneficial for the consumer, as their antioxidant activity is known to reduce the risk of neurodegenerative, cardiovascular and oncological diseases. This activity is related to the ability to scavenge free radicals and depends on their structure, especially on the number and positions of hydroxyl groups, as well as the character of substitutions on aromatic rings. Their other significant effects are antiviral, antibacterial, anti-inflammatory and antifungal. Due to the complex of many beneficial effects, they are investigated in more detail preclinical studies (in vitro, ex vivo and in vivo). It can currently only be administered in specialized treatment centers in the Czech Republic, which must be able to ensure the collection of T lymphocytes, their sending for preparation, the subsequent administration of CAR-T preparations, as well as the recognition, monitoring and management of possible adverse effects of the treatment (1). The aim of this article is to highlight the role of hospital pharmacists, who are key to the correct handling of these highly specialized preparations. Due to the difficulty of handling and the high price of these preparations, it is necessary to have all steps standardized and documented. Hospital pharmacists are guarantors of the correct handling of all medicinal products in hospitals and play an important role in the management of CAR-T therapies. They actively participates not only in providing CAR-T preparations for medical facilities, but also in solving irregularities and defects connected with their application. {"title":"Adherence to the Pharmacist's Protocol of sore throat symptomatic treatment in Ukraine and problems of harmonization with international guidelines.","authors":"Viktoriia Propisnova, Svitlana Misiurova","doi":"10.36290/csf.2025.002","DOIUrl":"https://doi.org/10.36290/csf.2025.002","url":null,"abstract":"<p><strong>Background: </strong>Good pharmaceutical counselling in the sore throat symptomatic treatment is an important component of the health care first line, timely detection of infectious diseases, provision of qualified assistance, prevention of complications, and correct use of medications.</p><p><strong>Methods: </strong>The study included a self-assessment of adherence of Ukrainian pharmacists to the Pharmacist Protocol through a survey, evaluation of the level of awareness of Ukrainian pharmacists with international standards for the symptomatic treatment of sore throat, assessing prospects and finding ways to harmonize the Ukrainian standard with international ones.  Results: A high level of commitment of pharmacists to the proper detection of the patient's problem, high fluctuations in adherence to the detection of specific \"alarm\" symptoms, an average level of proper information about the medications, a low level of awareness of international approaches to the sore throat symptomatic treatment with nonsteroidal anti-inflammatory drugs were established. The first case describes the long-term treatment of avian mycobacteriosis (MAC - Mycobacterium avium complex) in a patient with cystic fibrosis. It details the patient's history of multi-drug resistant MAC infection, numerous antituberculosis regimens, and significant drug intolerances, emphasizing the need for careful consideration of drug interactions, particularly with cystic fibrosis transmembrane conductance regulator (CFTR) modulators. The second case focuses on the management of a drug interaction between rifampicin and levothyroxine. It describes a patient with hypothyroidism whose levothyroxine the-rapy was destabilized by the co-administration of rifampicin, a potent enzyme inducer. This case highlights the critical need for proactive management and careful monitoring of thyroid function in patients receiving both levothyroxine and rifampicin. The third case explores a potential intoxication by menthol and eucalyptus in a patient attempting to quit smoking, who consumed excessive amounts of menthol-eucalyptus lozenges. This case illustrates how seemingly harmless over-the-counter products can lead to adverse effects when consumed in large quantities, underscoring the importance of a detailed dietary history. It is a serious disease diagnosed most often in young adulthood and, without treatment, is the most common cause of non-traumatic disability at this age. Repellents are an effective protection. The aim of this article is to introduce you to the substances most often used in repellents, to warn you about their correct use and to create an overview of the preparations used in the Czech Republic.
