Pandemia COVID-19 znacząco wpłynęła na rozwój cyfryzacji sektora ochrony zdrowia i implementację rozwiązań z zakresu e-zdrowia w Polsce. Celem pracy była analiza rozwiązań z zakresu e-zdrowia wdrożonych w sektorze ochrony zdrowia w kraju. W niniejszym opracowaniu skupiono się na ogólnopolskich rozwiązaniach systemowych. Metody przeglądu. Zebrano i przeanalizowano dane dotyczące rozwiązań z zakresu e-zdrowia wdrożonych w polskim systemie ochrony zdrowia w okresie od 1 stycznia 2004 do 31 marca 2022 roku. Dane uzyskano na podstawie analizy aktów prawnych (ustaw i rozporządzeń) oraz informacji publikowanych przez Ministerstwo Zdrowia, Narodowy Fundusz Zdrowia oraz Centrum e-Zdrowia. Opis stanu wiedzy. Zidentyfikowano 7 systemowych rozwiązań z zakresu e-zdrowia, wdrożonych na poziomie ogólnokrajowym, którymi są: Internetowe Konto Pacjenta (IKP), elektroniczna recepta (e-recepta), zwolnienie (e-zwolnienie), skierowanie (e-skierowanie), teleporady, informator o terminach leczenia (e-kolejka), elektroniczna dokumentacja medyczna (EDM). Zaobserwowano 5 obszarów dotyczących tych rozwiązań, którymi są: kluczowe funkcjonalności danej e-usługi; dotychczasowe funkcjonowanie usługi przed wprowadzeniem danego rozwiązania informatycznego; rozwiązania legislacyjne i informatyczne umożliwiające wdrożenie danej e-usługi; zakres informacji zawartych w danej e-usłudze; wpływ danej e-usługi na organizację systemu ochrony zdrowia z perspektywy pacjenta i świadczeniodawcy. Podsumowanie. E-recepta oraz e-skierowanie stanowią jedne z najczęściej świadczonych usług z zakresu e-zdrowia w Polsce. Pomimo znaczącego rozwoju usług e-zdrowia w trakcie pandemii, nadal istnieje wiele barier, które opóźniają rozwój usług cyfrowych w ochronie zdrowia, m.in. dług technologiczny, niskie nakłady na ochronę zdrowia oraz niski poziom kompetencji cyfrowych wśród starszych pracowników medycznych.","PeriodicalId":387674,"journal":{"name":"Medycyna Ogólna i Nauki o Zdrowiu","volume":"92 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130052083","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Zespół obturacyjnych bezdechów podczas snu (OBPS) jest spotykany u coraz większej liczby pacjentów. Jego negatywny wpływ na układ sercowo-naczyniowy oraz nieprzestrzeganie przez pacjentów zaleceń leczniczych zmuszają do badań nad nowoczesnymi rodzajami terapii, także tej farmakologicznej. Celem niniejszego przeglądu piśmiennictwa jest przedstawienie możliwości stosowania substancji z grupy kannabinoidów – kannabidiolu (CBD), delta 9-tetrahydrokannabinolu (THC) oraz ich syntetycznych analogów w leczeniu zespołu OBPS. Metody przeglądu. Użycie elektronicznej bazy danych Pubmed oraz rejestrów badań klinicznych pozwoliło znaleźć przeglądy piśmiennictwa, badania przedkliniczne oraz kliniczne dotyczące farmakoterapii zespołu OBPS substancjami z grupy kannabinoidów. Użyto słów kluczowych „OSAS pharmacotherapy”, „cannabinoid”, „sleep apnea” oraz ich kombinacji. Wybrane prace oryginalne zostały opublikowane w latach 2010–2021 oraz dotyczyły wpływu syntetycznego odpowiednika THC, dronabinolu, na ilościowe i jakościowe parametry snu. Opis stanu wiedzy. Badania nad kannabinoidami w leczeniu zespołu OBPS, mimo małej ilości danych, dają obiecujące wyniki, które wykazują ich skuteczność. Syntetyczny analog THC, dronabinol, jest lekiem zmniejszającym liczbę bezdechów podczas snu, poprawiającym jego jakość oraz zwiększającym udział snu wolnofalowego. Podczas badań został uznany za lek bezpieczny i dobrze tolerowany. Podsumowanie. Mimo iż aktualne wyniki badań dowodzą skuteczności leków z grupy kannabinoidów w leczeniu zespołu OBPS, konieczne jest przeprowadzenie badań wieloośrodkowych, aby określić ich profil bezpieczeństwa, długoterminowe skutki ich stosowania, odpowiednie dawkowanie oraz ustalić, pacjenci o którym fenotypie mogą zyskać najwięcej ze stosowania takiej terapii.","PeriodicalId":387674,"journal":{"name":"Medycyna Ogólna i Nauki o Zdrowiu","volume":"89 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-07","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116666046","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Scientific studies report that the risk of symptoms of postpartum depression (PPD) can significantly reduce breastfeeding time. It has not yet been shown whether there is a difference in cortisol levels in breast milk and serum cortisol levels in women at risk of PPD but without symptoms. The aim of the study was assessment of the levels of cortisol in breast milk and levels of serum cortisol in women at risk of PPD four weeks after birth. Materials and method. The study included 75 women who were recruited at a University Hospital and via social media. The proper study was conducted in the fourth week after delivery. The research tool used was The Edinburgh Postnatal Depression Scale (EPDS). Assessment of cortisol levels in breast milk was performed with the enzyme-linked immunosorbent assay CORTISOL saliva ELISA DiaMetra. Blood analysis was conducted in an ALAB Laboratory, one of a nationwide network of specialis laboratories. Results. The prevalence of the risk of PPD symptoms in the study sample was estimated at 28% in EPDS. The risk of PPD symptoms does not differentiate between cortisol levels in breast milk and serum cortisol levels (p>0.05). A correlation was shown between the level of cortisol in breast milk and in the blood serum of the study sample (p<0.03). Conclusions. The study indicates that the risk of PPD symptoms does not differentiate between serum cortisol levels and cortisol levels in breast milk. The level of cortisol in breast milk reflects the level of cortisol in the blood serum of the subjects.","PeriodicalId":387674,"journal":{"name":"Medycyna Ogólna i Nauki o Zdrowiu","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-05-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131203926","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Early detection of trunk rotation abnormalities in schoolchildren should be a basis for the prevention of three-plane spinal deformities. Scoliometric evaluation enables reliable, reproducible selection of schoolchildren requiring extended orthopaedic diagnostics. The accepted SOSORT recommendations for the interpretation of trunk rotation angle values need to be popularized mainly in preventive health care teams. The aim of the study was to evaluate trunk rotation angles and total trunk rotation in schoolchildren aged 8–12 years in two body positions. Materials and method. The study group consisted of 72 schoolchildren randomly recruited for the project. Angle of Trunk Rotation (ATR) was measured using Bunnell scoliometer at three levels of the spine, in two positions: standing and sitting. Based on the obtained values the Sum of Trunk Rotation (SR) was calculated. Results. Statistically significant differences in ATR at the level of Th5-TH12 were observed between standing and sitting position, and between the values of global trunk rotation. From the total number of the schoolchildren in the study, 14 (4 girls and 10 boys) were selected for performing further diagnostics. Conclusions. The study carried out in a small group of schoolchildren attending one of primary schools in Warsaw shows the importance of early screening for AIS. Measurement of ATR using an inexpensive instrument such as a scoliometer enables an objective assessment of postural immaturity and referral for further orthopaedic evaluation. However, a prerequisite for proper, reproducible assessment is the widespread education of school screening teams.","PeriodicalId":387674,"journal":{"name":"Medycyna Ogólna i Nauki o Zdrowiu","volume":"17 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-04-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114908553","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
