{"title":"[Animal study results of a novel designed transcatheter mitral valve replacement system].","authors":"D Zhu, S Z Wang, J B Gao, Z L Luo, K Yang, C M Xie, P X Kong, S Y Feng, H Jiang, X B Pan","doi":"10.3760/cma.j.cn112148-20250126-00069","DOIUrl":"10.3760/cma.j.cn112148-20250126-00069","url":null,"abstract":"<p><p><b>Objective:</b> To preliminarily assess the biocompatibility and durability of the TruDelta<sup>TM</sup> transcatheter mitral valve replacement (TMVR) system. <b>Method:</b> Six adult sheep were divided into 3 groups based on the duration of follow-up: 30 days (<i>n</i>=1), 90 days (<i>n</i>=3) and 180 days (<i>n</i>=2). The TruDelta<sup>TM</sup> TMVR system was implanted through a transapical approach under transesophageal echocardiographic guidance. The operability of the TMVR system was evaluated using an instrument performance evaluation scale (consisting of 39 items), with scores ranging from 1 (worst) to 10 (best) assigned by the operator. Echocardiography was conducted preoperatively, immediately after surgery, and at 30, 90, and 180 days post-implantation. At the last follow-up time point, the intervention mitral valve membrane and major organs were dissected for observation. The artificial valves were taken for hematoxylin eosin (HE) staining and observed under a scanning electron microscope. <b>Result:</b> All six procedures were successfully completed using 29S size TruDelta<sup>TM</sup> TMVR device. At the final follow-up, echocardiogram demonstrated good valve function without obvious paravalvular leakage, with a transvalvular gradient of (7.8±3.2) mmHg (1 mmHg=0.133 kPa) and a mitral valve orifice area of (1.8±0.2) cm<sup>2</sup>. Autopsy findings revealed no structural valve failure and almost complete endothelialization (>75%) with 90 to 180 days. This study was a prospective cohort study led by Fuwai Hospital, Chinese Academy of Medical Sciences, with participation from eight regional tertiary hospitals as sub-centers, and enrolled 18 701 patients with confirmed coronary heart disease between January 2015 and May 2019. Among them, 8 429 patients with ACS were included in this study. The estimated cumulative LDL-C exposure was calculated by multiplying LDL-C by age. Participants were then divided into four groups based on quartiles. Baseline data and coronary angiography data were collected, and participants were followed for 2 years. The primary endpoint was MACCE, which was composed of all-cause death, cardiac death, myocardial infarction, revascularization, and stroke. Spearman correlation analysis was used to estimate the correlation between cumulative LDL-C exposure and the severity of coronary artery disease. The differences in MACCE among the four groups were compared, and multivariate Cox regression was used to divide the estimated cumulative exposure LDL-C into two groups, three groups, and four groups to analyze its relationship with MACCE. <b>Results:</b> The 8 429 ACS patients included in the study had an age of (60.9±11.4) years, with 1 951(23.1%) females. Spearman correlation analysis revealed that estimated cumulative LDL-C exposure was positively associated with the preoperative SYNTAX score, three-vessel lesions disease, left main disease, and the number of target lesions (correlation coefficients <i>r</i>=0.14, 0.10, 0.04 and 0.03, respectively, with all <i>P</i><0.05). The 2-year follow-up results indicated that the incidence rates of MACCE, all-cause death, cardiac death, myocardial infarction, and stroke in ACS patients grouped by different levels of estimated cumulative LDL-C exposure were statistically significant (all <i>P</i><0.05). The results of the Cox multivariate regression analysis showed that when the estimated cumulative LDL-C exposure was treated as a continuous variable and analyzed in two, three, and four groups, with the lowest group as the reference, the risk of MACCE occurrence in the high-value group increased by 21% (95%<i>CI</i> 1.08-1.37, <i>P</i>=0.002), 24% (95%<i>CI</i> 1.07-1.43, <i>P</i>=0.004), and 21% (95%<i>CI</i> 1.02-1.43, <i>P</i>=0.025) respectively. <b>Conclusions:</b> A positive correlation was found between estimated cumulative LDL-C exposure and severity of coronary artery disease. The median follow-up duration was 3.9 years. The 367 non-culprit vessels of the 233 patients were divided into the VOCO group (33 vessels, 9.0%) and the non-VOCO group (334 vessels, 91.0%). Parameters pertaining to the hemodynamic pattern of non-culprit vessel stenosis between the two groups were compared. Receiver operating characteristic (ROC) curves were used to assess the correlation between hemodynamic pattern and VOCO, and Cox multivariate regression and logistic multivariate regression analyses were applied to identify independent predictors of VOCO. <b>Results:</b> The 233 enrolled patients were aged (62.5±12.9) years, with 193 males (82.8%). In the VOCO group, the maximum quantitative flow ratio (QFR) decreased within 20 mm of the QFR-assessed segment, the difference in QFR across the entire vessel, the length of functionally significant vessel, and the maximum gradient of QFR decrease (dQFR/dsmax) were significantly greater than those in the non-VOCO group. ROC curve analysis showed that the optimal threshold for predicting VOCO using dQFR/dsmax was 0.009 6 (area under the curve: 0.691, 95%<i>CI</i>: 0.606-0.775, <i>P</i><0.001). Multivariable Cox regression analysis revealed that dQFR/dsmax was an independent predictor of VOCO (<i>HR</i>=1.199, 95%<i>CI</i>: 1.070-1.343, <i>P</i>=0.002). When anatomical and functional stenosis severities were included in the model, a high pullback pressure gradient (PPG) index (<i>HR</i>=1.572, 95%<i>CI:</i> 1.052-2.351, <i>P</i>=0.027) emerged as an independent predictor of VOCO.
