{"title":"[Association between pericoronary fat attenuation index and high-risk plaques in patients with FFR-negative acute coronary syndrome].","authors":"J Y Li, J Y Luo, Z M Xu, Y Xue, Y Li, Y L Han","doi":"10.3760/cma.j.cn112148-20251201-00841","DOIUrl":"10.3760/cma.j.cn112148-20251201-00841","url":null,"abstract":"<p><p><b>Objective:</b> To investigate the relationship between fat attenuation index (FAI) measured by coronary computed tomography angiography (CCTA) and high-risk plaques in patients with acute coronary syndrome (ACS) and negative fractional flow reserve (FFR). <b>Methods:</b> This single-center cross-sectional study enrolled a total of 342 consecutive patients with a confirmed diagnosis of ACS, who underwent coronary angiography and had negative FFR results (>0.80) at the Department of Cardiology, General Hospital of Northern Theater Command, from January 2016 to December 2022. All patients underwent CCTA within 1 month before the onset of ACS. According to the CCTA findings of the target vessels, patients were divided into a high-risk plaque group (<i>n</i>=165) and a non-high-risk plaque group (<i>n</i>=177). Semi-automated software was used to evaluate the FAI centered on the culprit lesion. The FAI values were compared between the two groups. A multivariate logistic regression model was applied to explore the association between high-risk plaques and FAI. The diagnostic performance of the model was assessed using receiver operating characteristic curve analysis. <b>Results:</b> The age of the patients was (64 (53, 78)) years, and there were 178 (52.0%) males. The FAI value was significantly higher in the high-risk plaque group than in the non-high-risk plaque group ((-79.79±9.10) HU vs. (-84.21±10.14) HU, <i>P</i>=0.001). Multivariate logistic regression analysis revealed that FAI was independently associated with high-risk plaques (<i>OR</i>=1.056, 95%<i>CI</i> 1.022-1.092, <i>P</i>=0.001). {"title":"[Analysis of lipid-lowering therapy in Chinese patients at high and very high risk of atherosclerotic cardiovascular disease].","authors":"L Zhang, Z Y Zhou, S D Zhou, S Y Zhan, Y Zhang, J P Li","doi":"10.3760/cma.j.cn112148-20251116-00805","DOIUrl":"10.3760/cma.j.cn112148-20251116-00805","url":null,"abstract":"<p><p><b>Objective:</b> To analyze the utilization, intensity, and associated factors of lipid-lowering therapy in Chinese patients at high or very high risk of atherosclerotic cardiovascular disease (ASCVD). <b>Methods:</b> This nationwide hospital-based cross-sectional study was led by Peking University First Hospital. Patients at high or very high risk of atherosclerotic cardiovascular disease (ASCVD) were recruited from 1 785 medical institutions across China between September 2022 and November 2024. Clinical data, including age, gender, and lipid profiles, were collected. We described the utilization status of lipid-lowering therapy and identified factors associated with the intensity of lipid-lowering therapy using multivariate logistic regression analysis. <b>Results:</b> A total of 189 674 ASCVD patients were included in the final analysis (153 066 high-risk (80.7%); 36 608 very high-risk (19.3%)). Low density lipoprotein cholesterol attainment rates were low in both high-risk (25 542/153 066, 16.7%) and very high-risk (6 585/36 608, 18.0%) groups. Non-treatment rates were 36.9% (56 436/153 066) in high-risk and 14.4% (5 280/36 608) in very high-risk patients. Treatment in both groups was predominantly moderate-intensity statin monotherapy, while high-intensity treatment rates were low (5.0% high-risk (7 685/153 066); 17.0% very high-risk (6 240/36 608)). Multivariate analysis showed that age >75 (high-risk patients: <i>OR</i>=0.55, 95%<i>CI</i>: 0.49-0.61; very high-risk patients: <i>OR</i>=0.51, 95%<i>CI</i>: 0.46-0.57) and rural residence (high-risk patients: <i>OR</i>=0.69, 95%<i>CI</i>: 0.66-0.73; very high-risk patients: <i>OR</i>=0.64, 95%<i>CI</i>: 0.60-0.68) were significantly associated with lower treatment intensity. Women in the very high-risk group had a lower propensity to receive any lipid-lowering treatment (<i>OR</i>=0.62, 95%<i>CI</i>: 0.57-0.67) and high-intensity therapy (<i>OR</i>=0.90, 95%<i>CI</i>: 0.84-0.97).
