There is some information about the role of defects of local immune protection in the formation of systemic immune reactions. A big problem is created by cases of the development of a systemic inflammatory response syndrome, which is often associated with a deficiency of immune responses in the area of distress. \u0000Objective: To determine the ratio of the state of local and immune reactions in Sami women. \u0000Methods: 49 practically healthy Sami women aged 21 to 44 years living in Lovozero village of Murmansk region were examined. The comparison group consisted of 50 practically healthy women of the Arkhangelsk region of the same age range. The hemogram, phagocytic activity of peripheral venous blood neutrophils, the content of lymphocyte phenotypes (CD3+, CD4+, CD8+, CD10+, CD95+) by indirect immunoperoxidase reaction and flow cytometry, the concentration of IgM, IgG, IgA by enzyme immunoassay, circulating immune complexes by precipitation were studied. The composition and quantity of microflora, cytogram, phagocytic activity of neutrophils, sorption capacity of epithelial cells, the content of sIgA and CEC IgG were studied in the separated mucous membranes of the pharynx, intestines and urine. The results are presented as the arithmetic mean and the error of the mean (Mm). For comparison between groups, an independent sample t-test or a nonparametric Mann-Whitney U-test was used, depending on the distribution. \u0000Results: It was found that the immune status of Sami women is characterized by the usual activation of cell-mediated cytotoxicity mechanisms in the northern territories, a deficiency of phagocyte activity and the accumulation of CEC, but these features are more pronounced in them. A decrease in the frequency of neutropenia registration does not compensate for the deficiency of phagocytic protection, but increases the secretory activity of neutrophils. The secretory activity of leukocytes is associated with their higher migration capacity into the mucous membranes, which does not compensate for the low activity of local phagocytes, the sorption capacity of epithelial cells and sIgA. A feature of the Sami immune reactivity is higher concentrations of IgA and IgM against a background of low IgG content. \u0000Conclusion: The migration activity of leukocytes is initiated by preventive reactions in places of distress and begins with reactions of hemodynamic changes. Methods. The study involved students of Khanty-Mansi Autonomous Okrug-Yugra with a daily study load, 100 girls and 53 boys, and employees of the \"Ambulance\" station in Khanty-Mansiysk, working in shifts, 31 women and 26 men The photoperiodic chronotype stability was studied using the computer program Photoperiodic chronotype stability calculator (registration no. 2019661664) in the form of a mobile application for Android Photoperiodic stability index. Results. The photoperiodic stability of the chronotype in women is: with regular daily employment - 3.5 (1.50-6.00) arb. units, with night shift work - 1.0 (0.00-4.00) arb. units; for men: with regular daily employment - 3.0 (1.00-4.00) arb. units, with shift night work - 1.5 (0.00-3.00) arb. units \u0000Volunteers with a regulated daytime job gravitated toward choosing to shift activities to the afternoon and refusing morning activity. In the group with shift work, the proportion of people oriented toward the morning type of work capacity was increased. In the group with shift and night work, there were no representatives of a definitely morning chronotype among women, and definitely an evening chronotype among men. In the summer season, male and female respondents, regardless of the production regime, showed an increase in the signs of the morning chronotype. \u0000Conclusion. The production and intersexual features of the chronotypological properties of the inhabitants of the North have been established. With the superposition of extreme climatic and geographical factors and social and production conditions, a general pattern of increased rigidity of the chronotype is revealed. {"title":"Subjective well-being, health and health related quality of life of Nenets adolescents living in the Yamalo-Nenets Autonomous Okrug","authors":"M. Dyakovich","doi":"10.17816/humeco107427","DOIUrl":"https://doi.org/10.17816/humeco107427","url":null,"abstract":"Introduction: In connection with the further development of the Arctic, the issues of subjective well-being, health-related quality of life and self-assessment of the health of indigenous youth of the circumpolar territories of our country are relevant. \u0000Aim: to assess the subjective well-being, health-related quality of life and the risks of health disorders among adolescents - representatives of the indigenous population of the Yamal. \u0000Methods: In 51 adolescents Nenets aged 1417 years old, studying in the regional center of the Yamal district, methods were used to measure subjective social well-being (SWB) by R. Cummins (2005), health-related quality of life according to PedsQL 4.0 Generic Core Self-report, 2010 (HRQoL), risks major general pathological syndromes (MGPS) were used. \u0000Results: There were no statistically significant ethnic and gender differences in the SWB components among Nenets and Russian adolescents. HRQoL indicators of both sexes did not have significant differences depending on ethnicity. According to results of self-assessment, Nenets adolescents (unlike Russians) are dominated by individuals with a minimal risk of health disorders. There are gender differences in the values of the risks of arterial hypertension, functional disorders of the liver and borderline mental disorders among the Nenets, which are higher in girls than in boys. More than a third of positive-minded Nenets teenagers have a high risk of health problems. {"title":"THE RESEARCH LANDSCAPE OF HEALTH EFFECTIVENESS RESEARCH IN RUSSIA: AN ANALYTICAL REVIEW","authors":"K. Shelygin, Lada Ivanona Lozkina","doi":"10.17816/humeco109249","DOIUrl":"https://doi.org/10.17816/humeco109249","url":null,"abstract":"Significance \u0000The relevance of the study lies in the systematic analysis of scientific and methodological approaches to the study and evaluation of health care as a multidimensional and multi-level system. \u0000Aim: to systematise the results of original research aimed at examining the effectiveness of health care. \u0000Methods \u0000A retrospective categorical and substantive analysis of original studies belonging to the genre \"scientific article\" and devoted to the effectiveness (quality) of the health care system in Russia, published in domestic scientific periodicals during the period from 2011 to 2021, was carried out. The categorical analysis included the following parameters: \"healthcare system\", \"efficiency\", \"methods\". Based on the inclusion and exclusion criteria, 48 sources were selected. All original studies were divided into two groups according to the author's specialty: medical and non-medical. The content analysis allowed revealing and parameterization of scientific-methodological tendencies of researches of public health effectiveness by statistical-structural-dynamic characteristics of researches and specialities of the author (medical and non-medical specialities). The main methods used in the original research were analysed. \u0000Statistical significance of differences was revealed by Fisher's exact test at significance level p0.05. \u0000Results \u0000A categorical analysis for the period 2011-2021 revealed 144 publications. The usual selection process excluded 17 publications. For further analysis 48 scientific articles were used. The structure of scientific publications was dominated by the results of original research - 64%. No systematic reviews and meta-analyses were identified. The increase in the dynamics of publication activity on the stated topic was due to original research. Most of the trials were conducted at the meso-level - 70.8%. There are virtually no works devoted to the results of original research conducted at the countrywide level. \u0000A set of sociological, medical-statistical and econometric methods is used as the main research tool for studying efficiency. Sociological methods and methods of analysis of official medical statistics prevail in \"pure\" form. The methods of demography and mathematical modelling were used least of all. No quantitative differences were found in the authors' specialisation. Differences are observed in the authors' gravitation towards the study of different types of efficiency depending on the researchers' specialty, as well as the frequency of studies of economic and social efficiency in their study at the meso- and macro-level, and the use of methodological apparatus in the study of these types of efficiency. \u0000Conclusions \u0000The core of the publication activity is formed by the results of original research obtained at the meso-level; research conducted at the macro-level is practically absent. Systematic reviews and meta-analyses are completely absent. Data of the death registration records from the registry offices of three districts of the Koryak autonomous okrug (3720 records) and of the Kirovsk city in Murmansk oblast (2394 records) for the period 1968-91, have been processed. The average annual mortality rates per 100 thousand of the studied population aged 20-59 years were calculated based on the All-Union Population Census of 1979. The obtained indicators were standardized according to the European standard. \u0000Results. The calculation of generalized regional mortality indicators, adopted in official \u0000statistics, does not give a real understanding of the mortality rates of the indigenous population, primarily the ISNPN, and its main causes. The maximum contribution to the ISNPN mortality rate is determined by a high level of their alcoholization, both as a result of direct toxic effects, and indirectly, being the cause of suicides, accidents. Obtaining actual indicators of the ISNPN mortality and morbidity is very difficult. The purpose of the study is to identify the dynamics of the emotional component of the subjective well-being of the personality of students at various time stages of the COVID-19 pandemic. Methods. The study involved students of psychological and pedagogical areas of Tyumen State University, Kurgan State University, Ural Federal University. B.N. Yeltsin, a total of 911 people. The research method is the scale of subjective well-being by G. Perruet-Badu, adapted by M.V. Sokolova. Measurements were taken in April 2020, May 2021, October 2021, January 2022. Results. In April 2020, at the beginning of the pandemic, the lowest indicators of the emotional component of the subjective well-being of the individual were observed among university students. A year later, by May 2021, there was an increase in subjective well-being, which, however, did not reach a statically significant level. The trend of improvement in subjective well-being continued six months later (October 2021), reaching a statistically significant value compared to the period when the pandemic began. The fourth measurement (January 2022) of subjective well-being revealed its stabilization, which indicates the adaptation of students to the conditions of the pandemic. Conclusions. The emotional component of the subjective well-being of the personality of students is subject to dynamics during the COVID-19 pandemic, which is fixed at a statistically significant level. During the two years of the pandemic, university students have adapted to the stressful situation of the pandemic, which is reflected in the improvement and stabilization of the emotional component of subjective well-being. There is a formal contradiction in the Republic of Mari El: favourable ecological situation in localities of the Republic versus the above-limit morbidity rate caused by the negative impact of habitat factors. It highlights the relevance of conducting a specifying quantitative examination with the use of modern econometric approaches. \u0000Materials and methods. The panel data on morbidity and environmental pollution in 15 municipal entities for 2009-2019 were used, as well as cross-cultural data of the fluctuating asymmetry for 2019. The analysis was based on the concept of cointegration and modeling with a mechanism of balance adjustment. \u0000Results. It was established that over the long term the increasing pollution level of drinking water from the distribution network and the atmosphere influences the increase in figures of congenital abnormalities (malformation), deformations and chromosome breakages in children aged 0 to14 years. It was found that the difference between the whole population being ill with malignant tumors and the adult population aged 18 and older in different municipal entities is statistically associated with the change in environmental quality (indicators of the fluctuating asymmetry). \u0000Conclusion. Actions should be taken on providing the population of the municipal entities with pathogen-free drinking water as well as onreduction of pollutant emissions. Being ill with malignant tumors is not related to internal contamination. Suturina","doi":"10.17816/humeco106718","DOIUrl":"https://doi.org/10.17816/humeco106718","url":null,"abstract":"Abstract: The objective of the study was to estimate the prevalence and clinical manifestation of decreased ovarian reserve in women of reproductive age from Eastern Siberia \u0000Methods and Results: During a cross-sectional study, performed in 2017-2019 in Irkutsk Region and Buryat Republic (Russia), we invited 1119 women aged 18 to 40 yrs, who underwent an annual preventive medical examination at their place of work. Of 1119 women, 907 women aged 31,78 5,13 years were included in the study. We used clinical, instrumental, and laboratory research methods and statistical analysis. Of the 907 women included in the study, 117 participants 12.9% had a follicle number per ovary of less than 5 and were diagnosed as patients with decreased ovarian reserve (DOR). According to the laboratory study, 56 out of 117 (47.8%) AMH was below 1.2 ng/ml. The average value of which was 0.53 0.43 ng/ml. In women with reduced AMH levels, a shortening of the menstrual cycle and high parity were observed. Based on a review of studies published in PubMed, Scopus Cochrane, and Google Scholar, the article outlines the putative pathophysiological mechanisms of particulate matter exposure to the cardiovascular system, primarily represented by oxidative stress, inflammation, endothelial dysfunction, lipid metabolism disorders, autonomic dysfunction, and impaired hemostatic system. Attention is drawn to the exposure of particulate matter to the cardiovascular system, and is presented as a short-term and long-term effect. The consequences of short-term exposure are the development of myocardial infarction, an increase in the frequency of hospitalization for CHF, atrial fibrillation, an increase in mortality from stroke and the risk of sudden death, the development of pulmonary embolism. Arterial hypertension, aggravation of coronary heart disease, chronic heart failure, development of atherosclerosis and dyslipidemia, and increased risk of mortality from coronary heart disease can be considered the consequences of long-term exposure to particulate matter.
