Colonic diverticular disease is defined as clinically manifest or symptomatic diverticulosis, either by inflammation, diverticular bleeding or segmental colitis. It is a frequent cause of hospitalization in industrialized countries and also makes a major contribution to health care costs. Due to the spread of the Western-style diet, low in fibre and high in processed foods, the prevalence of diverticulosis is now increasing globally. Obesity is a significant risk factor contributing to the increased prevalence of both diverticular disease and diverticulitis and its complications, particularly in the younger population, previously considered to be at much lower risk than the geriatric population. Diverticulitis occurs when one or more diverticula, together with adjacent colonic tissue, undergo an inflammatory process. Approximately 15% of patients who suffer an episode of acute diverticulitis will experience complications, the most common of which is peridiverticular abscess, which can be complicated by peritonitis. Less common complications are colonic lumen stenosis and fistulae. The fungal infections are one of the most aggressive types of existing infections, their opportunistic character enabling them to cause invasive infections, ultimately leading to a higher morbidity and a higher rate of mortality. The present study focuses on the presence of Candida spp. in 19 out of a total of 70 patients admitted to the Critical Care Burn Unit in the Clinical Emergency Hospital Bucharest, between 01.01.2019-31.12.2019. No other fungal species, besides Candida spp., were identified in this patient lot. The aim of this study was to analyze the risk factors and the dynamics of the biological parameters of the patients presenting Candida spp. infections, in order to determine how these contribute to the prognostic and final outcome of these patients. {"title":"The Impact of Vertebroplasty on Sagittal Parameters in Traumatic Thoracic Vertebral Fractures","authors":"Kemal Paksoy","doi":"10.31689/rmm.2023.30.1.57","DOIUrl":"https://doi.org/10.31689/rmm.2023.30.1.57","url":null,"abstract":"Objective: It is well-established that vertebroplasty has effects on sagittal parameters due to various pathologies of the spine. Our objective in the study was to reveal the impact of vertebroplasty that was applied to compression fractures due to trauma on sagittal parameters. Material and Method: Twenty patients were included in the study. Scoliosis radiographs were shot preoperatively and at first month during the postoperative period. Spinopelvic parameters of each patient including pelvic incidence (PI), sacral slope (SS), pelvic tilt (PT), lumbar lordosis (LL), thoracic kyphosis (TK), thoracolumbar kyphosis (TLK), cervical lordosis (CL), and sagittal vertical axis (SVA) were assessed. Visual Analogue Scale (VAS) and Oswestry Disability Index (ODI) assessments of the patients were performed as well. Results: Spinopelvic parameter values of the patients were assessed statistically through the analysis of the Paired sample T test before surgery and in the first month during the postoperative period. It was determined that the changes in the PT, SS and LL values of the patients between preoperative and first month postoperative were at the level of p 0.659, p=0.716, p=0.012; and the changes were not significant. It was determined that the changes in the TL, TK, CL, SVA, VAS and ODI values of the patients between preoperative and first month postoperative were at the level of p <0.001and the changes were significant. Conclusion: Vertebroplasty is an efficient technique for relieving pain in thoracic fractures that have a score of 4 and over, based on the thoracolumbar injury classification score and that are type A, based on the AO Spine thoracolumbar injury classification score. This condition results from a deficiency of the porphobilinogen deaminase enzyme, which plays a role in heme production, a crucial component of haemoglobin in the bloodstream. This deficiency leads to the accumulation of substances called porphyrins in the body, which can trigger the appearance of severe and potentially life-threatening symptoms. She was a known case of Bipolar Affective Disorder Type 1 with psychosis on treatment with quetiapine, duloxetine, escitalopram, and etizolam for the last one and a half years and had no history of adverse effects with those drugs. Recently Lamotrigine was added to her regimen at a dose of 50 mg/day. She developed Stevens-Johnson syndrome (SJ syndrome) within two weeks of adding lamotrigine which progressed to Toxic Epidermal Necrolysis (TEN), SCORTEN scores 3, in the next 3-4 days. The sequence of events, reports of laboratory investigations, and management of TEN have been elaborated in this case report. Adverse reactions to biological therapy can limit their widespread use, so early detection and treatment can adjust attempts to stop these molecules. TNF Alpha blockers may cause the following skin reactions in alpha patients: injection site reactions, infections, immune-mediated reactions (psoriasis, psoriasis, drug-induced lupus, vasculitis, hidradenitis, alopecia), allergic or neoplastic reactions. We present the case of a patient with RA who developed skin lesions during biological therapy and was diagnosed with drug induced lupus based on clinical elements, associated autoimmunity, and dermatological evaluation. The skin lesions were attributed to the interaction of three medications (biosimilar Etanercept, Leflunomide, and Isoniazid), all of which have been implicated in causing these side effects. The solutions that saved the patient were temporarily discontinuing the immunosuppressive medication and replacing it with a local corticoid, followed by the continuation of Etanercept in associated with Methotrexate, and the patient was able to continue the biological medication and obtain a favorable response to the treatment. A regimen including triple association (platinum-taxanes-fluorouracil) is generally considered superior in efficacy, but may be associated with severe toxicity. In the case of recurrence, the options are limited and the prognosis is generally unfavorable. Chemotherapy alone or in combination with an anti-EGFR monoclonal antibody (Cetuximab), immunotherapy or re-irradiation for selected cases are feasible options in loco-regional or metastatic repalapse. We present a case of nasopharyngeal cancer (NPC), with negative prognostic and predictive factors multimodally treated with an intensive chemotherapy regimen associating Cetuximab with a median survival higher than the median value reported in most studies. Replacing 5-Floururacil with Capecitabine and Cisplatin with Carboplatin may be an option to increase treatment tolerance and should be evaluated in randomized trials. The use of induction chemotherapy as a “new standard” before radio-chemotherapy for cases with negative prognostic factors should also be the subject of future studies. In case of replaced left hepatic artery, its ligation can lead to hepatic injury or ischemia, while preserving it can cause difficulties during lymphadenectomy. In literature there is no consensus regarding preserving replaced left hepatic artery during gastrectomy for cancer. A recent study, analysing adverse effects of ligating an aberrant left hepatic artery, shows in pacients with over 5 times elevated transaminase levels, increase in hospital length and postoperative complications. On the other hand, there are studies that consider ligation of aberrant left hepatic artery safe, the only inconvenient being postoperative transient elevation of transminase levels, when ligated artery diameter is over 1.5 mm. Matherial and methoods: We report the case of a 65 years old male, known with myocardial infarction, admitted for epigastric pain, nausea, vomiting, dysphagia for solids and important weight loss. Upper gastrointesinal endoscopy with biopsy and computed tomography showed eso-gastric tumoral mass, signet ring cell carcinoma, no metastases. Intraopertive, we found replaced left heaptic artery arising from left gastric artery, close to the celiac trunk, its diameter being approximately 1 cm. Total radical D2 gastrectomy with mechanical eso-jejunal Roux-en-Y anastomosis was performed. Postoperative evolution was favourable surgically, but the patient had SarsCov2 infection during hospitalisation The final pathology report showed 18 lymph nodes examined, 5 being with adenocarcinoma metastases. Conclusions: Preserving replaced left hepatic artery during gastectomy for cancer is preferable, lyphadecnectomy not being affected. The assumption that sαKl could inhibit the insulin-like growth factor 1 (IGF-1) signaling pathway has increased the interest of its research in acromegaly - a pathology characterized by an excess of growth hormone (GH) and IGF-1. This review aimed to identify the role of sαKl in acromegaly. Most studies have identified higher concentrations of sαKl in acromegaly versus non-GH pituitary adenomas (PAs) or healthy subjects. Also, the sαKl level was higher in active acromegaly versus controlled or cured disease. Moreover, sαKl positively correlated with GH, IGF-1, and GH-secreting PA size. The increased level of sαKl seems to be due to the autonomous secretion of GH, considering their drastic decrease after transsphenoidal surgery. The data regarding the relationship between sαKl, age, and gender in acromegaly are inconsistent. The latest research noted a correlation between sαKl and various parameters for monitoring the quality of life in acromegaly. In conclusion, sαKl could reflect the disease activity. Smoking and smokeless tobacco both are increasing worldwide. The common oral precancerous lesions are oral leukoplakia, erythroplakia, nicotina palati and oral sub mucous fibrosis (OSMF). Screening is best tool to find out such lesions. The current study is conducted to know the prevalence of tobacco consumption and also to examine oral cavity of tobacco consumer to detect precancerous lesions among the staff. Method: The present study was cross sectional study carried out tertiary care hospital staff. The study was conducted over a period of 6 month from May 2017 to October 2017. Ethical clearance for study was obtained from Institutional Ethical Committee. The staff members of class 3 and class 4 of hospital were taken as a study participants involving both of sex and who provided written informed consent. Sample of 120 staff members were enrolled for the study from provided sampling frame with the use of random sampling method. Study participants with previous history of hospital admission for any systemic illness, immunodeficiency or immunosuppressive patients, and participants who already diagnosed to have malignant cancer lesions in any part of body were excluded. Result: Of 120 study participants, 40% were belongs to 31 – 40 years of age group and majority were male (80%) and belonged to Hindu (90.83%) religion. The prevalence of tobacco consumption in any of form is 85%; of them 43.33% used smokeless tobacco and 10.83% used to with smoking and 30.83% had both forms of tobacco. Oral Sub Mucosal Fibrosis (OSMF) was the most common (70.58%) precancerous lesion found in the present study, followed by leukoplakia (16.67%). Conclusion: Current study revealed the high prevalence. There is urgent need to plan de-addiction at various level.
