It necessitates a delicate balance between airway and phonation. In this article, we discuss our experience with coblation in posterior cordotomy to treat patients with bilateral abductor paralysis (Coblation assisted Kashima Surgery). Patients and Methods: A prospective study was done among 27 patients with bilateral vocal fold paralysis who underwent coblation assisted Kashima surgery at our tertiary care hospital. The handicap index and dyspnoeic scores were assessed before and after surgery. All patients were tracheostomised and after successful Kashima surgery, the tracheostoma was closed. Patients were followed up for a period of 3 months for worsening of voice/ dyspnoea/ aspiration. Results: Of the 27 patients, 19 were males and 8 were females, most common aetiology of the vocal cord palsy was subsequent to thyroid surgery (Total thyroidectomy). The study showed that there was a significant mean difference between pre and post-tests among patients with dyspnoea index with P-value < 0.05 i.e., 0.02 . It also showed that there was significant mean difference between pre and post-tests among patients with VHI Index with P-value < 0.05 i.e., 0.04 . Conclusion: Kashima operation gives good results with respect to airway improvement. In the last two decades, there has been an increase in adolescents and adults and in Muslim-majority countries. Objective: The purpose of this study was to describe patients with a pin needle as tracheobronchial FB who went to otorhinolaryngology's Emergency Unit at Dr. Soetomo General Hospital Surabaya. Patients and Methods: Descriptive observations using retrospective data. The data taken from medical records in the Broncho-Esophagology (BE) division of Otorhinolaryngology ER unit at Dr. Soetomo General Hospital, Surabaya. It was taken during the period 2016-2018. The patients were divided into two groups by age, namely group A (≥10 year old) and group B (≤10 year old). Results: Among 40 tracheobronchial foreign bodies, there were 24 patients (60%) with pin needle aspirations; consisting of five male (20.83%) and 19 female (79.17%). The youngest in group A was 2.5 year old (average 5.38 ± 3.25 years, median 4.5 years) and the oldest in group B was nineteen year old (average 14.1 ± 2, 40 years, median fourteen years). All pin needle FB appear on the thorax X-ray (100%). Conclusion: There was an increase in the incidence of pin needle aspiration over the last three decades. Most of the patients were teenage girls. Thorax X-ray provided a good diagnostic imaging. It is known that temporary or permanent threshold shifts in hearing exist after loud noise exposure. The vestibular system can also be activated and therefore disturbed the equilibrium mechanism. Consequently, this study aimed to assess the effect of noise on the vestibular system in patients with chronic noise exposure using a video head impulse test (vHIT). Patients and Methods: The present study included 34 subjects. They divided into two groups: the study group consisted of 17 patients with chronic noise exposure and the control group included healthy subjects with normal peripheral hearing sensitivity. Basic audiological evaluation and vHIT were conducted on all subjects in the study. Results: There was a statistically significant difference in vHIT lateral canal gain between the study and control groups. A statistically significant negative correlation between duration of noise exposure and lateral canal gain was also present. In addition, there was a negative correlation between lateral canal gain and pure tone thresholds at 2,3,4,6&8KHz frequencies. {"title":"Endoscopic versus Microscopic Myringoplasty in Anterior Central Tympanic Membrane Perforations","authors":"H. Hussein, A. Patients and Methods: A prospective comparative study was conducted on fifty patients with anterior TM perforation who were randomly and equally distributed among two groups for EM and MM. Both groups were compared regarding the operative details and postoperative outcomes. Results: The graft success rates after 6 months for EM and MM were 88% and 72% respectively ( p=0.157 ). The operative duration was significantly shorter in the EM group ( p<0.001 ). Intra-operative blood loss was significantly less in EM ( p<0.001 ). ABG was significantly improved in both groups ( P<0.001 ) with no significant difference between the two groups ( P=0.081 ). Post-operative pain was significantly less in EM ( p<0.001 ). There was no significant difference between the two groups regarding the complications ( P=0.049 ) with no complications with EM. In both groups, there was no difference between patients with anterior canal wall protrusion (ACWP) and patients without ACWP regarding operative duration ( p=0.123 and 0.372 respectively). There was a significant relationship between ACWP and the graft taking rate after 6 months in both groups ( p=0.015 and p<0.001 respectively). We present a case of an extranasopharyngeal angiofibroma that has occurred in the maxillary sinus, as well as review published literature on similar cases. A 14-year-old male presents with right-sided facial swelling, facial numbness and epistaxis secondary to a vascular lesion occupying the right nasal cavity. Biopsies revealed an angiofibroma. Imaging showed erosion of the anterior maxillary wall, thinning of the anterior wall of the pterygopalatine fossa whilst sparing of the nasopharynx, pterygoid plates and vidian canals. The tumour was excised uneventfully via a right hemi-facial degloving surgery. Maxillary ENAs presents most often with facial swelling, followed by nasal obstruction and epistaxis. The anterior and medial maxillary sinus walls were more commonly affected. In our case, the posterior and inferior walls were unaffected. These tumours were treated primarily by surgery, and generally will require open surgical approaches. The aim of this study is to evaluate the efficacy of combined laser posterior cordotomy and endoscopic suture latero-fixation on respiration and phonation. Patients and Methods: This study was a randomized controlled trial, conducted on forty patients, divided into 2 groups: Group (A) included 20 patients underwent combined laser posterior cordotomy and endoscopic suture latero-fixation, while group (B) involved 20 patients subjected to laser posterior cordotomy without latero-fixation for managing BVFP. Results: Group A showed a statistically significant shorter time interval of decannulation from tracheostomy tube after the procedure. ( P=0.032 ). No significant difference between both techniques regarding adequacy of respiratory chink, decannulation rate and voice handicap index scores. Pyogenic granuloma is a misnomer since neither it is due to bacterial infection nor does it produce any pus. Also, histopathologically there is no granuloma formation. In general, pyogenic granuloma is treated by surgical excision. Here, we describe a case of pyogenic granuloma of the tongue, include a related literature regarding possible causes, sites, differential diagnosis, biopsy findings and its treatment. Case presentation: A 10-year-old child developed swelling at the lateral side of the tongue one month after dental procedure. By examination, it was soft with a smooth surface. The mass was excised using coblation system without recurrence till six months. Conclusions: Dental procedure may induce trauma to the tongue and act as an initiating factor for formation of pyogenic granuloma. Histopathological examination is a corner stone for the definitive diagnosis. Background: Allergic nasal polyps are benign and are characterized by mucosal inflammation and expansion into the lumen of the nasal cavity. The treatment of these lesions can include steroids, saline nasal spray, mucolytics, topical/ systemic decongestants, topical anticholinergics, anti-leukotrienes or receptor blockers, and antihistamines, but steroids are the most effective drugs for medical polypectomy. Patients and Methods: 96 patients with persistent bilateral nasal obstruction were included in our study during period from January 2017 to December 2018. 42 patients used oral steroid as group B and 54 patients used oral macrolides as group A. Patients were scheduled for follow-up visit (after three months) with respect to computed tomographic scan of paranasal sinuses scoring and endoscopic nasal scoring. The severity of polyposis was significantly lower in the macrolides group vs. the steroid group based on both radiological and endoscopic scoring. Non-significant difference detected in the patient satisfaction between both groups. Objective: To evaluate the effects of sildenafil citrate on the guinea pigs cochlear structure and function using Otoacoustic Emissions (OAEs) and light microscopic H & E cochlear sections. Patients and Methods: This experimental study was conducted on 45 healthy adult male pigmented guinea pigs in Otolaryngology-Head and neck Surgery and Histology Departments in Suez Canal University, Ismailia-Egypt. Animals were randomly divided into three groups: Group 1 (n=15): control group. Group 2 (n=15): received the therapeutic dose of sildenafil citrate. Group 3 (n=15): received double the therapeutic dose of sildenafil citrate. Results: DPOAEs recorded from the three groups showed that the lowest amplitude was at 1006 kHz (10.4dB) and the highest at 6060 & 8084 kHz (43.9 & 42.1dB) respectively without statistically significant intergroup difference. In the H & E cochlear sections, there was disorganization and vacuolation of almost all cells of organ of Corti in group 2 and 3 compared to group 1. Lightly stained cytoplasm, karryorrhectic nuclei and pyknotic ones were also seen. These changes were more marked in group 3 which showed also some cellular loss in some sections. ABSTRACT Background: Several studies reported voice changes in patients with thyroid disorders. Objective: The objective of this study was to examine the voice characteristics of a group of patients with hyperthyroidism. Patients and Methods: Twenty-one patients with clinical hyperthyroidism and Twenty-one healthy volunteers in the age range 24-55 years were subjected to voice assessment using both subjective (auditory perceptual assessment) and objective tools (acoustic and aerodynamic analyses). Results: Despite the absence of perceptual voice changes in our patients with hyperthyroidism, subtle changes in the acoustic and aerodynamic parameters could be detected with a trend towards laryngeal dysfunction. Maximum phonation time and Harmonic/Noise ratio were significantly lower, and shimmer was significantly higher in hyperthyroid patients when compared to the reference group. Pitch and jitter were also higher in hyperthyroid patients, but the difference was not statistically significant.
