Церковский","doi":"10.24931/2413-9432-2018-7-4-4-10","DOIUrl":"https://doi.org/10.24931/2413-9432-2018-7-4-4-10","url":null,"abstract":"The aim of study is to evaluate the tolerability and effectiveness of photodynamic therapy as an organ‑preserving treatment in patients with vulvar leukoplakia. 50 patients with a verifed diagnosis of «vulvar leukoplakia» were included in the study. The age varied from 27 to 74 years. The method of treatment assumed the use of the photosensitizer рhotolon (RUE «Belmedpreparaty», Belarus) administered intravenously in doses of 1.8–2.5 mg/kg. Photoirradiation of pathological foci was carried out 2.5–3 hours after intravenous injection of photolon® using a semiconductor laser «UPL PDT» (LEMT, Belarus, λ=661 nm) at exposure doses from 30 to 100 J/cm2 with a power density of 100–170 mW/cm2. The treatment was performed under medical anesthesia. The results of treatment were evaluated using clinical data. Adverse reactions and complications after the introduction of the photosensitizer and photoirradiation have not been observed. Complete clinical regression of the treated pathological foci was noted in 100% of cases with a follow‑up observation 1 month after the treatment. At follow‑up after 3 months, local recurrences of the disease were detected in 4 cases, which were successfully treated with repeated photo‑dynamic therapy sessions. The percentage of complete regressions was 92%, partial – 8%. With the increase in the incidence of AK, mainly due to the popularization of recreation in countries with increased insolation, there is an increasing interest in developing new methods of diagnostics and treatment and improving the existing ones. Studies that are aimed at determining the final efficacy of PDT, taking into account the resulting adverse reactions and long-term cosmetic results, are becoming increasingly popular. The nature of the light needed to excite a photosensitizer (PS) opens up new possibilities in the field of experimental studies that are aimed at reducing adverse reactions with similar efficacy of the applied therapy. Its efficacy can be compared to pharmacological therapy, while in some cases its advantages may even lead to a more stable hypotensive effect. SLT may be used as a primary treatment for primary open-angle glaucoma patients and patients with ocular hypertension, while also considered safe and effective in cases when a repeat procedure is required. SLT may potentially decrease the demand for antiglaucoma drugs, improve patient’s response to treatment, make the treatment more comfortable and overall increase the patient’s quality of life. New modifications of standard laser treatment procedures have been emerging lately. This article summarizes scientific data on the efficacy and safety of the new generation of laser trabeculoplasty. Kaprin","doi":"10.24931/2413-9432-2018-7-3-21-28","DOIUrl":"https://doi.org/10.24931/2413-9432-2018-7-3-21-28","url":null,"abstract":"This work presents the results of performing intraoperative photodynamic therapy (IOPDT) on 22 patients with recurrent pelvic tumors(cervical cancer – in 18 patients, cancer of the corpus uteri – in 3 patients, cancer of the anal canal – in 1 patient). Prior to the PDT procedure, the patients were injected with photolon photosensitizer (PS) at a dose of 1.0–1.1 mg/kg. After the injection of PS, local fluorescence spectroscopy of tumor lesions was performed to determine the accumulation of drug in various areas of tumors and healthy tissue. Intraoperative laser irradiation was carried out 3–5 hours after the photolon injection with light at 662 nm wavelength using \"Latus-2\" laser device with a power density of 140 mW/cm2and the density of light energy of 40–60 J/cm2, the number of irradiation fields was 3–5 depending on the anatomical features.The follow-up period after surgical treatment combined with PDT was from 6 to 24 months. Analyzing the immediate results of the treatment, there were no undesirable events or increase in the number of postoperative complications compared to patients treated without IOPDT. Their ability to induce the synthesis of photoactive protoporphyrin IX in the healthy tissues of the rabbit bladder when instilling the drug solutions at various concentrations has been estimated. It was shown that 5-ALA HE results in the induction and accumulation of PPIX in the rabbit bladder epithelium at much lower concentrations than 5-ALA. Shirokih","doi":"10.24931/2413-9432-2018-7-3-29-35","DOIUrl":"https://doi.org/10.24931/2413-9432-2018-7-3-29-35","url":null,"abstract":"Currently, reconstructive-plastic surgery is becoming the standard for surgical and combined treatment of breast cancer (BC) patients.With a one-stage reconstruction of the breast, the use of biological implants that can replace muscle autografts and, consequently, reduce trauma, blood loss and operation time, and also to avoid defects in the donor zones is becoming topical. From 2014 to 2017, 151 reconstructive operations on 121 patients with BC (average age 41.5 years) using synthetic and biological materials were carried out in P.Herzen Moscow Oncology Research Institute. 0 stage of BC TisN0M0 was diagnosed in 11 (9.1%) patients, I stage – in 52 (42.9%), IIA – in 41 (33.9%), IIB – in 9 (7.4%), IIIA – in 4 (3.3%), IIIB – in 2 (1.7%), IIIC – in 2 (1.7%). To strengthen the lower slope, a biological implant – Permacol acellular dermal matrix, was used in 34 cases. Cosmetic result was rated as excellent in 20 (58.9%) cases, good in 11 (32.3%), unsatisfactory in 3 (8,8%). Biological materials are an important addition to various options for breast reconstruction. In many cases, the consequences of this disease are fatal. Therefore, a simple, fast, accurate and affordable diagnostic system for the early detection of this disease is necessary for the timely administration of antimalarial drugs. The malarial parasite, during its intra-erythrocyte development, processes a significant amount of hemoglobin, which in this case turns into a hem form called hemozoin. Hemozoin and hemoglobin have different molar extinction coefficients at certain optical wavelengths, hence, light absorption and an optoacoustic signal (OAS) from the infected cell will be different from that of a healthy cell. The paper describes the developed theoretical model intended for studying the influence of intra-erythrocyte malarial parasite development on optoacoustic signals. The OAS were calculated based on the models of healthy and infected blood modeled on the basis of a 3D model. The simulated OAS were analyzed in the temporal and frequency domains to obtain signs of infection at various stages. The calculated OAS spectra have different amplitude levels, which indicates that the optoacoustic method can be useful for differentiating various intraerythrocyte stages of the malarial parasite. DBPBM in different modifications of the method and the mechanical deepithelization were performed in experiment on 19 human cadaver eyes. We use system Ophtho-Burr for DBPBM that includes a tip and ophthalmic burrs with diameter 1mm, 2.5 mm, 5 mm, varying the speed of rotation of burr. For 3 eyes — polishing with the application of burr with a diameter of 5 mm, the rotation frequency of 4000 rpm using 2 passes over the surface of the cornea. For 3 eyes — burr with a diameter of 2.5 mm, rotation frequency of 4000 rpm in 2 perpendicular passes. For 3 eyes — diameter burr 1 mm at speed of 8000 rpm in 2 perpendicular passes. For 3 eyes — diameter burr 1 mm at speed 4000 rpm with 4 passes along the surface of the cornea. For 4 eyes — diameter was 1 mm at speed of 4000 rpm in 2 perpendicular passes. For 3 the eyes was conducted mechanical deepithelization. Subsequent histopathological investigation of the cornea was performed by method of semi-thin section, with polychrome staining.Results. The obtained results of histological studies of DBPBM in different versions on cadaver cornea is allowed to choose the optimal variant of surgical intervention for use in clinical conditions. It consisted in a selection of burr with a diameter of 1 mm, the rotation frequency of the 4000 rpm and 2 perpendicular passes with a moderate compression of the cornea. The abovementioned technique of intervention allows removing completely the corneal epithelium with a basal membrane and save uniform in thickness, fully deepitelizationed Bowman’s membrane.Conclusion.Careful and accurate removal of dysplastic corneal epithelium in cases of the recurrent corneal erosion syndrome using proposed method in clinical conditions should contribute to the creation optimal environments for adequate reepithelization with steady epithelial-stromal adhesion. For routine clinical use of hyperthermia, it is necessary to control the uniformity and localization of heat within the tumor. Local heating can be achieved by using special thermal agents, such as nanoparticles doped with rare-earth ions. Measurement of the temperature of the thermal agents will allow timely regulation of the applied laser radiation excitation power and optimization of the hyperthermia process.The paper presents the results of a study on the non-invasive determination of the YPO4 nanoparticles doped with Nd3+ temperature with sensitivity of 0.2% °С-1 in 30-60°С temperature range. The temperature of the nanoparticles was calculated from the Nd3+ luminescence spectra in the 800-1000 nm range under excitation into 4F5/2 energy state by 805 nm laser. A calibration procedure for recalculating the ratio of the luminescence intensities from the Stark sublevels of the 4F3/2 Nd3+ state into the values of the real NP temperature in accordance with the Boltzmann distribution is given. An algorithm for calculating luminescence intensities for individual Stark components is proposed. After calculating the intensities corresponding to each individual Stark component, all the intensities related to the transition from the upper and lower Stark sublevels of the 4F3/2 state are summed, and then their ratio is calculated. The resulting ratio is normalized to the value of the ratio at room temperature and, in accordance with the calibration dependence, is recalculated into the NP heating temperature. The set of multilayered phantoms representing a border between normal brain-tissue and a tumor with photosensitizer \u0000and gold nanoparticles (spheres or stars) in different concentrations were made to investigate the macroscopic parameters observed during the \u0000neurosurgical biopsy sample collection by means of the optical probe. These investigantions will allow to define the border of a brain tumor and \u0000control the type of tissue being collected by a stereotactic cannula during surgery. The impact of gold nanospheres and gold nanostars added \u0000in different concentrations to the bottom-layer of phantom was analyzed by fluorescence quenching or enhancement due to energy transfer \u0000between nanoparticles and fluorescent molecules.
