These cysts are usually asymptomatic, and are detected by routine radiographs.\u0000 \u0000 \u0000 \u0000 This case report presents the management of a large nasopalatine duct cyst in an 11-year-old pediatric patient. The patient presented with symptoms such severe mobility in central incisors and swelling in the anterior part of the palate. Diagnostic imaging confirmed the presence of a large cyst in the nasopalatine region. Surgical intervention was performed to excise the cyst, and the central incisors were splinted with a rigid splint for 4 weeks.\u0000 \u0000 \u0000 \u0000 The case of a large nasopalatine duct cyst in a child patient poses several important clinical considerations. Nasopalatine duct cysts are relatively rare, especially in pediatric patients, making this case particularly noteworthy. The presentation of a large cyst in a child raises questions about the etiology, diagnosis, and treatment options for such cases.\u0000 \u0000 \u0000 \u0000 This case report highlights the importance of considering nasopalatine duct cysts as a differential diagnosis in pediatric patients presenting with maxillary swelling and associated symptoms. Early recognition and appropriate management are essential to prevent potential complications and ensure optimal outcomes for the patient. After a challenging diagnostic workup, she was diagnosed by tissue biopsy to have primary central nervous system angiitis.\u0000 \u0000 \u0000 \u0000 Although primary central nervous system angiitis has been reported increasingly recently, its pathogenesis is still unknown and its diagnosis is still very challenging. Gastroparesis, a condition characterised by delayed stomach emptying, presents debilitating symptoms and is notably linked to neuropathic disorders, including diabetes mellitus. Despite an apparent higher prevalence in LMICs compared to the global average, epidemiological data remains scarce. The paper highlights pioneering research in Pakistan, Brazil, and China, showcasing a transformative shift toward comprehensive studies that delve into nuanced aspects of gastroparesis epidemiology, gender-specific patterns, and innovative therapeutic approaches such as acupuncture. However, challenges impeding clinical management and research in LMICs are identified, encompassing the absence of extensive epidemiological studies, lack of treatment guidelines, and the scarcity of specialised training programs for healthcare professionals. These challenges are intertwined with broader issues such as limited healthcare infrastructure, resource disparities, and healthcare workforce shortages. The paper proposes a multifaceted approach for addressing these challenges, involving international collaboration, capacity building, and the integration of gastroparesis management into primary healthcare services. It occurs frequently throughout the operating period and affects surgical patient outcomes differently in terms of morbidity and mortality. Because of coagulopathy, metabolic acidosis, multiple organ failure, hemodynamic instability, and infections, a core temperature below 34 °C is strongly associated with mortality.\u0000 \u0000 \u0000 \u0000 This study aimed to assess the magnitude and associated factors of intra-operative hypothermia in pediatric patients undergoing elective surgery at the Ayder Comprehensive Specialized Hospital.\u0000 \u0000 \u0000 \u0000 A prospective observational cross-sectional study was conducted on 399 pediatric patients undergoing elective surgery at Ayder Comprehensive Specialized Hospital in Tigray, Ethiopia, from May 1, 2023, to July 30, 2023. Participants in the study were selected by a systematic random sampling technique. The data collection procedure was chart review and intra-operative temperature measurement, and the collected data were analyzed by SPSS version 23. The independent variables were analyzed using binary and multi-logistic regression. The odds ratio, 95% confidence interval, and p-value of <0.05 were considered statistical significance.\u0000 \u0000 \u0000 \u0000 The magnitude of intra-operative hypothermia was 52.9%. Neonate and infant [AOR: 6, 95% CI: 3.7, 9.8], (AOR=4.5, 95% CI: 2.9, 7) respectively, volume of fluid administered greater than half liter [AOR: 4.37, (95% CI, 3, 6.4)], patients who underwent surgery during the morning [AOR: 5.3, (95% CI: 3.8, 7.4)], and duration of surgery and anesthesia greater than 120 minutes [AOR: 2.7, (95% CI, 1.8, 4)] and (AOR=3.4, 95% CI, 2.4, 4.9], respectively, were factors significantly associated with intra-operative hypothermia.\u0000 \u0000 \u0000 \u0000 This study revealed a high magnitude of intraoperative hypothermia among pediatric patients. Being neonates and infants, having a cold volume of IV fluid administered greater than half a liter, entering surgery during the morning, the duration of surgery, and the anesthesia time were significantly associated with intra-operative hypothermia. Patients in the experimental group were implemented with enteral nutrition continuity management program, and their eating behavior was adjusted three days before the end of tube feeding. Food intake was intermittent at 2 hours, 3 hours and 4 hours on the first day, the second day and the third day of intervention, respectively, and all patients stopped eating at night. Abdominal distension assessment, appetite assessment, application of gastric motility drugs and patient satisfaction were compared between the two groups after tube feeding.\u0000 \u0000 \u0000 \u0000 Three days after the end of tube feeding, abdominal distention assessment, bowel sound auscultation and appetite assessment were statistically different (P<0.05) between the two groups. There were differences in the first day (15 vs 6, P<0.05), the second day(9 vs 3, P<0.05) and the cumulative number(17 vs 7, P<0.05) of gastrointestinal drugs, but no differences in the third day (2 vs 1, P>0.05). Graft-versus-host disease (GVHD) occurs in 30 to 70% of cases and often diminishes the quality of life of transplant patients. This study aimed to determine the prevalence of vaginal complications of GVHD following hematopoietic stem cell transplantation.\u0000 \u0000 \u0000 \u0000 This study employed an analytical cross-sectional design. All patients referred to Shariati Hospital in Tehran between 2019 and 2020 who underwent hematopoietic stem cell transplantation were considered for inclusion in this study if they met the inclusion criteria. Inclusion criteria encompassed non- not sexually active women aged 18 to 70 who received stem cell transplantation more than 100 days prior. Exclusion criteria comprised patients who experienced GVHD during the first 100 days post-transplantation. Additionally, individuals over 75 and patients with metastatic cancer were excluded.\u0000 \u0000 \u0000 \u0000 A total of 55 patients were recruited, with ages averaging 40±9.9 years for recipients and 38.5±12.8 years for donors. Notably, 63.3% and 58.2% of patients exhibited oral and ocular symptoms, respectively. Regarding genital involvement, 49.1% experienced vaginal symptoms, while 25.5% had vulvar involvement. Among the 27 patients with vaginal involvement, 2 (7.4%) were categorized as mild, 17 (63%) as moderate, and 8 (29.6%) as severe. Univariate analysis identified reduced vaginal discharge (OR=6.56), vaginal tightness (OR=6.23), pelvic pain (OR=5.50), and vaginal involvement (OR=3.81) as significant predictors of other organ symptoms. Moreover, vaginal involvement (OR=3.68) emerged as the sole significant predictor of the co-occurrence of oral, ocular, and other organ symptoms. However, their efficacy and safety remain controversial.\u0000 \u0000 \u0000 \u0000 RCTs published before 15th January, 2023 were included by a systematic literature search, which compared the clinical outcomes between HIO and TIO. The primary outcome was the estimated blood loss (EBL). Three independent authors screened and extracted the data and resolved disagreements by consensus. The ROB2.0 tool was used for evaluating the risk of bias.\u0000 \u0000 \u0000 \u0000 A total of 1026 patients (511 TIO and 515 HIO) from 9 studies were analyzed in the meta-analyses. The EBL between TIO and HIO group was similar, while HIO was associated with lower proportion of patients required transfusion (P=0.002), less units of blood transferred (P<0.001) and lower overall complication rate (P=0.008). There were no significant differences between TIO and HIO in mortality (P=0.37), length of stay (P=0.97), bile leak rate (P=0.58), liver failure rate (P=0.96), reoperation rate (P=0.48), postoperative hemorrhage rate (P=0.93) and incidence of postoperative ascites (P=0.96). The operative time of HIO was usually no more than 15 minutes longer than that of TIO (P<0.001).\u0000 \u0000 \u0000 \u0000 Comparing with the TIO, HIO increased the operative time and failed to further reduce the EBL in patients with liver surgery. This report presents a case of bilateral Vestibular Schwannoma with a cooccurring Meningioma in in a Nepali child and provides an overview of the literature on this condition.\u0000 \u0000 \u0000 \u0000 A 15-year-old male presented with bilateral sensorineural hearing loss, seizures, and neurological deficits and was ultimately diagnosed with concomitant bilateral acoustic neuroma and meningioma. The patient underwent radiosurgery for bilateral vestibular schwannoma and non-operative management of the meningioma. Long-term follow-up revealed symptomatic improvement, emphasizing the importance of a multidisciplinary approach in managing such complex cases. The management of these tumors requires tailored treatment strategies guided by tumor characteristics and associated risks.\u0000 \u0000 \u0000 \u0000 Meningioma and vestibular schwannoma are common tumors of the CNS. Their coexistence is possible in Neurofibromatosis type 2 (NF2) but is exceedingly rare in pediatric age group. The tumors, often coexisting, pose diagnostic challenges. Diagnosis relies on clinical and genetic features, with multidisciplinary management involving various specialists. Treatment aims to preserve function and quality of life, utilizing approaches such as bevacizumab and surgical intervention. The role of radiation therapy remains uncertain. Genetic testing and regular monitoring are vital for early detection and intervention.\u0000 \u0000 \u0000 \u0000 The co-occurrence of acoustic neuromas and meningiomas is poorly understood, with limited reported cases and unclear pathophysiological mechanisms. Effective pharmacological treatments for NASH and liver fibrosis are limited.\u0000 \u0000 \u0000 \u0000 This study systematically reviews and meta-analyzes the safety and efficacy of resmetirom, a selective thyroid hormone receptor-β agonist, in NASH and liver fibrosis treatment. By analyzing data from clinical trials, we aim to offer evidence-based recommendations for resmetirom’s use in managing these conditions and identify avenues for future research.\u0000 \u0000 \u0000 \u0000 Electronic databases (PubMed, Scopus, Science Direct, Google Scholar, ClinicalTrials.gov, and Cochrane CENTRAL) were systematically searched, supplemented by manual screening of relevant sources. Only English-language randomized controlled trials were included. Data extraction, risk of bias assessment, pooled analyses, and meta-regression were performed.\u0000 \u0000 \u0000 \u0000 Three randomized controlled trials involving 2231 participants were analyzed. Resmetirom demonstrated significant reductions in hepatic fat fraction (SMD -4.61, 95% CI -6.77 to -2.44, P < 0.0001), NASH resolution without worsening fibrosis (RR 2.51, 95% CI 1.74 to 3.64, P = 0.00001), and liver fibrosis improvement (RR 2.31, 95% CI 1.20 to 4.44, P = 0.01). Secondary outcomes showed significant improvements in lipid profiles, liver enzymes, and NASH biomarkers with resmetirom treatment. Meta-regression revealed associations between covariates and primary outcomes.\u0000 \u0000 \u0000 \u0000 Resmetirom exhibits promising efficacy in reducing hepatic fat, improving NASH resolution, and ameliorating liver fibrosis with a favorable safety profile.
