Over\u0000the last decade, pharmacological research has significantly enhanced our understanding of many elements determining medicine prescription. The objective of this study is to carefully assess the clinical\u0000pharmacokinetic studies involved female subjects in India.\u0000\u0000\u0000\u0000We conducted a systematic review to assess clinical trial records' pharmacokinetic data in\u0000the female population of all age groups. The keywords 'pharmacokinetics’ or 'pharmacokinetic parameters’ or ‘bioavailability’ was used in the clinicaltrials.gov website and clinical trial registry of India\u0000[CTRI] website to find eligible clinical trials or records carried out between January 1, 2000 and June\u000030, 2022. Interventional studies, pharmacokinetics, pharmacokinetic parameters, bioavailability, female participants, India were all included in an advanced search filter on the two websites.\u0000\u0000\u0000\u0000In a total of 714 records, 17 (2.4%) records of interventional clinical trials with pharmacokinetic data involving the female population were assessed. All 17 trials were conducted in 15 research\u0000settings, with 27.3% of studies conducted in Maharashtra, followed by Karnataka (12.7%) and New\u0000Delhi (9.1%). A total of 30 pharmacokinetic parameters were studied in trials with peak concentration\u0000(C max) and area under the curve (AUC) of plasma or tissue concentration, accounting for 23.3% of\u0000the trials. The majority of drug trials focused on treating neoplasms accounted for 14 trials (76.4%).\u0000\u0000\u0000\u0000This review provides an overview of the current status and gaps in pharmacokinetic research in female populations. Periostin is a matricellular protein secreted by fibroblasts associated with cancer and is also secreted in chronic inflammatory conditions such as CP.\u0000\u0000\u0000\u0000The aim of this study is to compare the GCF periostin level among CP, EC, and healthy people and the evaluation of the plausible role of periostin in this association.\u0000\u0000\u0000\u0000This case-control study was conducted on 80 participants including 20 people with both EC and CP (EC-CP), 20 only CP cases (H-CP), 20 only EC cases (EC-H), and 20 healthy people (H-H). Then, the gingival crevicular fluid (GCF) periostin levels for all the participants were measured through enzyme-linked immunosorbent assay (ELISA). Finally, the data were analyzed using Stata software version 11.\u0000\u0000\u0000\u0000This matched case-control study was conducted on 80 participants (40 cases and 40 controls). The participants were divided into four groups : 1) 20 patients with EC and severe CP (EC-CP), 2) 20 patients with EC and without CP (EC-H), 3) 20 patients without EC and with severe CP (H-CP), and 4) 20 healthy participants (H-H). Then, the GCF periostin levels for all the participants were measured through enzyme-linked immunosorbent assay (ELISA). Finally, the data were analyzed using Stata 11.\u0000\u0000\u0000\u0000The mean value of periostin levels was significantly different between groups (p<0.001) and the lowest and highest mean was observed in EC-CP and H-H groups, respectively. The mean of GCF periostin before treatment in the EC-CP group was 2.14 which was lower than that after treatment; however, this difference was not significant (P=0.086). Also, the mean of GCF periostin in the H-CP group was 3.96 before non-surgical treatment and after treatment it significantly raised to 6.79 (P=0.049).\u0000\u0000\u0000\u0000Periostin is a potential biomarker for CP diagnosis in EC patients and also can be used to prevent CP. Recurrent vulvovaginal candidiasis interferes with emotional and sexual relationships.\u0000\u0000\u0000\u0000This study aimed to compare the effect of vaginal Salvia officinalis, clotrimazole, and their combination on the prevention of recurrent vulvovaginal candidiasis.\u0000\u0000\u0000\u0000This randomized triple-blinded trial was conducted on 111 women aged 18-49 years with vulvovaginal candidiasis. Participants were randomly assigned into one of the three groups of vaginal tablets of Salvia officinalis-400mg and placebo of clotrimazole-100mg, clotrimazole-100mg and placebo of Salvia Officinalis-400mg, or Salvia officinalis and clotrimazole that were applied every night for 7 consecutive days. A recurrence rate of vulvovaginal candidiasis was evaluated by WET test and vaginitis symptoms examination after 12 weeks. In the case of positive evaluation, the culture of vaginal discharge was performed.\u0000\u0000\u0000\u0000Totally, 29 patients (80.6%) in group clotrimazole, 33 patients (94.3%) in group Salvia Officinalis and 35 patients (97.2%) in group Salvia officinalis-clotrimazole were treated (P=0.43). Out of cured patients, 4 (13.8%) patients in the group clotrimazole, 1 patient (3%) in group Salvia Officinalis, and no case in group Salvia officinalis-clotrimazole experienced the recurrence of infection. There was a significant difference among the 3 groups in terms of recurrence (P=0.023). Data were collected from October 2019 to March 2020 using KoBo collect toolbox. Chi-square and Fisher exact were used to compare baseline differences between the intervention and control group, a generalized estimating equation was used to evaluate the impact of health education intervention.\u0000\u0000\u0000\u0000There was no significant difference in nutritional status (body mass index for age percentile and mid-upper arm circumference) between the intervention and control group at baseline. IR is found in roughly 80% of obese women with PCOS and 30–40% of lean women. IR is an impairment of insulin to mediate metabolism in skeletal muscle, adipocytes and liver. Following weight gain, the severity of IR in women with PCOS increases. IR affects many PCOS women who are not overweight, and minor weight loss improves clinical results in those with near-normal body weight. The effect of exercise on IS is optimized through glucose transport and metabolism. Exercise is the first-line treatment for PCOS. Moderate intensity (60–70% VO2max; comparable to 75–80% HRmax) and high-intensity aerobic exercise (heart rate >80%) improves IR in women with PCOS. These exercises are performed for at least 30 minutes per session and 3–5 days per week. A single bout of exercise (2–72 hours postexercise) can improve IS immediately. Exercise improves IS and glycemic management as well as helps in lowering the abdominal fat. Heterotopic pregnancy carries high morbidity and mortality. Therefore, it needs high index of suspicion to diagnose and manage accordingly.\u0000\u0000\u0000\u0000Our case reports a 39-year-old woman who presented with heterotopic triplet pregnancy at 5 weeks and 5 days of gestation. It resulted in a successful delivery of intrauterine monozygotic twins at 37 weeks. This pregnancy is an outcome of ICSI treatment due to low ovarian reserve. Only two embryos were obtained through the protocol and double frozen embryo transfer was done afterwards. Tubal pregnancy was successfully managed expectantly.\u0000\u0000\u0000\u0000Our case highlights the importance of early surveillance and close monitoring of patients undergoing multiple embryo transfers. With declining maternal mortality, physicians need to focus on decreasing maternal morbidities, including postpartum mental health disorders.\u0000\u0000\u0000\u0000To evaluate the incidence of PPD and the influence of demographic and pregnancy related factors on PPD, and to identify major symptom subsets manifested postnatally.\u0000\u0000\u0000\u0000This cross-sectional study, conducted on 435 women between 2 weeks to 6 months postpartum, in a tertiary care centre in South India, was administered with questionnaires including Edinburgh Postnatal Depression Scale (EPDS) and patient demographics and pregnancy related factors. Chi squared test was used to evaluate the association between EPDS scores and demographic and pregnancy variables. Subset analysis was carried out to identify the predominant symptoms in the patients.\u0000\u0000\u0000\u0000Completed questionnaires were obtained from 427 postpartum women, among which 26.2% (n=112) had EPDS score >12. Among demographic variables, EPDS scores showed a significant association with relationship problems (p=.000) only, while among pregnancy factors, preterm birth (p=.019), neonatal intensive care admission (p=.005), pregnancy complications (p=.009), and history of depression (p=.043) were significantly associated with EPDS scores. Subset analysis revealed a high mean EPDS score for anxiety (1.91), followed by depressive (1.38) and anhedonia (0.91) symptoms.\u0000\u0000\u0000\u0000Several clinical factors and adverse events during pregnancy, including socio-demographic factors and pregnancy-related complications itself, can set the stage for PPD and other mental health conditions. {"title":"Impact of Vitamin D supplementation on menstrual cycle irregularities among women with polycystic ovary syndrome who were treated with metformin: A systematic review with meta-analysis","authors":"E. Alenzi","doi":"10.2174/1573404819666221103142456","DOIUrl":"https://doi.org/10.2174/1573404819666221103142456","url":null,"abstract":"\u0000\u0000There is inconsistency in the evidence about the effect of adding vitamin D supplements to metformin therapy among women with polycystic ovary syndrome (PCOS).\u0000\u0000\u0000\u0000In this systematic review, we described the differences in menstrual cycle irregularities between women who received vitamin D supplementation and those who didn’t receive vitamin D among women diagnosed with PCOS and metformin.\u0000\u0000\u0000\u0000We reviewed previous literature to identify all relevant studies. The included studies' quality and risk of bias were assessed using the Consolidated Standards of Reporting Trials (CONSORT) and the Reporting of Observational Studies in Epidemiology (STROBE) instrument. Cochrane's Q-test and I2 statistic were used to assess the heterogeneity across included studies. The fixed-effect model was used to calculate the total effect size.\u0000\u0000\u0000\u0000Six studies were included, involving 633 women diagnosed with PCOS. Supplementation of vitamin D with metformin could significantly impact the improvement of menstrual cycles compared to receiving metformin alone (OR = 2.11, 95% CI, 1.40- 3.19). There was no evidence of heterogeneity across included studies. The appropriate solution to reduce the FOC has not yet been identified.\u0000\u0000\u0000\u0000The present study aimed to evaluate the effect of haptonomy intervention on FOC and maternal-fetal attachment (MFA) among primigravid women.\u0000\u0000\u0000\u0000This quasi-experimental study was conducted on 68 primigravid women with a gestational age of 24-28 weeks who referred to health centers in Tabriz, Iran with a FOC score above 65. Participants were randomly assigned into the intervention and control groups. The intervention group (n=34) received haptonomy in 5 sessions, once a week for 45-60 minutes, and control group received an information pamphlet about pain management options in labor and fetal development during pregnancy. The data were collected using Wijma Delivery Expectancy/Experience Questionnaire-version A, Cranley’s MFA Scale, and Revised Pregnancy Related Anxiety Questionnaire, before the intervention and at gestational age of 35-37 weeks. The data were analyzed using Chi-square, independent t-test, and ANCOVA.\u0000\u0000\u0000\u0000After intervention and controlling the effect of baseline score, the mean score of FOC in the intervention group was significantly lower than that in the control group [Adjusted mean difference (AMD): -23.67, 95% CI: - 28.12 to -19.23, P =0.001]. Further, the mean score of MFA indicated a significant difference between the two groups [AMD: 10.12, 95% CI: 6.44 to 13.8, p <0.001].\u0000\u0000\u0000\u0000The results of the present study revealed the effectiveness of the haptonomy intervention in reducing FOC and increasing MFA. Despite the recent progress in the treatment of advanced OC, oncologists in many parts of the world still face several difficulties and constraints in their daily management of their patients. In addition to the shortage of resources and expertise, non-adherence to the up to date international guidelines has been identified as a major obstacle towards the improvement of treatment outcomes in the majority of developing countries, including the Middle East and North African (MENA) countries. A multidisciplinary gynecological oncology panel from the MENA region met virtually in December 2020 to discuss the challenges and hurdles faced by the community oncologists in the management of advanced OC.
