The vasculogenic form of erectile dysfunction (ED) is the most widespread of all forms of ED. As a rule, young men in the initial stages of the disorder are not ready to accept phalloprosthetics, making it a relevant concern to search for alternative ways of their sexual rehabilitation, including X-ray endovascular surgery. No less relevant is further improvement of the quality assessment methods for erectile dysfunction both for diagnostic purposes and for determining of efficacy of the treatment provided.Aim. To assess the quality of erectile function by monitoring nocturnal penile tumescence (NPT) in patients after X-ray endovascular treatment of vasculogenic ED.Materials and methods. Over 560 patients with suspected vasculogenic erectile disorder have been examined since 2012, with X-ray endovascular treatment provided to 121 patients with confirmed veno-occlusive ED. To assess the quality of erectile function in patients, we monitored NPT using an Androscan MIT registrar from January 2022 to September 2023, in each case one month before and after the surgical treatment of vasculogenic ED. Included in the study were 40 patients who underwent surgery with us, aged 25 to 48 years old (34.3 ± 7.4 years on average) and having vasculogenic ED, including those with veno-occlusive erectile disorders (n = 33), arterial insufficiency of the cavernous bodies (n = 5), hemodynamic and clinical signs of arteriovenous ED in the subcompensation stage (n = 2). Data were compared on changes in the penile diameter and duration of rigidity during NPT, reflecting the main hemodynamic indicators of the penile blood flow.Results. In the compensated stage of ED, the increase in the penile diameter during NPT was 20–30 %, with duration of tumescence being 43.2 ± 20 min; in the subcompensated stage, 10–30 % and 23 ± 21 min respectively. One month after treatment, these indicators were 45 ± 15 % and 76.3 ± 21 min respectively. After surgery, all patients demonstrated a recovery of the erectile function, and the linear and angle-independent blood flow indicators tended to normalize. The data obtained from the monitoring of NPT helped identify a correlation between the linear blood flow indicators, their derivatives, as well as the time and character of penile rigidity during NPT, making it possible to differentiate the types of vasculogenic ED and the criteria for its clinical compensation.Conclusion. At present, the instrumental diagnostics of ED is limited to the Doppler sonography of the penile vessels, which does not consider the multifactorial nature of the causes of erectile disorders in men of various ages. Registration of NPT using a standalone device allowing to reconduct the examination and to record the results graphically as erectograms can be used for minimally invasive objectification to confirm the recovery of the erectile function in the event of compensated and subcompensated stages of vasculogenic ED. Acephalic spermatozoa syndrome is characterized by high proportion of headless (acephalic) spermatozoa in the ejaculate. Sperm morphological changes in this syndrome were characterized, however, the etiology and pathogenesis of this syndrome have not been under evaluated. In recent years, with the progress in sequencing technology and other high-performance methods of genome, proteome and other omics technologies, it has become possible to identify many genetic causes of disorders of spermatogenesis and male infertility, as well as a better understanding of their mechanisms. In accordance with the Russian Society of Urology guidelines, gonadotropins, selective estrogen receptor modulators and antioxidants can be used in the treatment of male infertility. According to the European Association of Urologists guidelines and American Urological Association/ American Society for Reproductive Medicine guidelines, this list may also include gonadotropin-releasing hormone and aromatase inhibitors (AI).Research objective: to review up-to-date literature data on effect of IA on male fertility and clarify the place of AI in the treatment of male infertility.The data obtained indicate that AI can increase the concentration, mobility and percentage of normal forms of spermatozoa, reduce sperm DNA fragmentation and normalize protamination processes. These effects were more pronounced in patients with overweight and obesity, as well as in patients with testosterone-to-estradiol ratios of <10. Given the important role of estrogens in spermatogenesis, when using AI, it is advisable to control the hormonal status and prevent estradiol from falling below the reference range, taking into account its multiple effects. However, a lack of quality data causes inability to accurately predict the effect of AI treatment on pregnancy rate and childbirth. The main method of correction of arteriovenous malformation is superselective embolization of vessels of the malformation zone. This clinical example presents a rare case of surgery in which embolization was unacceptable due to technical difficulties associated with the complexity of vascular angioarchitectonics, with the impossibility of superselective entry into varix through the arterial channel. To develop and implement an algorithm for early diagnosis of a localized form of prostate cancer based on the determination of the specificity of the prostate-specific antigen (PSA) marker in different age groups.Materials and methods. In the framework of a screening program for early detection of diseases of the urogenital system developed at the Stavropol Regional Clinical Specialized Uroandrological Center, middle-aged and elderly men were examined using the following characteristics: IPSS (International Prostate Symptom Score), serum PSA level, digital rectal exam, transrectal prostate ultrasound, urodynamic tests, as well as transrectal ultrasound-guided prostate biopsy in cases of elevated total PSA. The data (for 2010–2021) on the frequency of detection of localized prostate cancer depending on patient age (40–59 years, 60–69 years, ³70 years) and total PSA level were analyzed.Results. With an increase in total blood PSA >4 ng/ml, there is a clear trend towards a decrease in the detection of benign prostatic hyperplasia and an increase in the detection of adenocarcinoma, which is confirmed by the data of histological examinations of biopsy specimens. In 81 % of cases, transrectal multifocal biopsy of the prostate gland (adenocarcinoma and prostatic intraepithelial neoplasia was detected) under transrectal ultrasound control is justified, including when PSA values are in the “gray zone”, which shows a rather high value of this method.Conclusion. Total serum PSA level is a readily available indicator for differential diagnosis of prostate diseases including detection of localized prostate cancer in out-patient conditions. The obtained data points at high specificity of this marker in men over 40 years old. Measurement of serum PSA level at the age of 40–49 years is necessary for early detection of oncological pathology. To provide comparative analysis of laparoscopic retropubic simple prostatectomy combined with provisional clamping of internal iliac arteries, uretherocystoanastomosis (LRP + CIIA + UCA) and transurethral resection of prostate (TURP) for surgical treatment benign prostatic hyperplasia (BPH).Materials and methods. 103 patients of age ³40 years old with refracted symptoms of urinary tract compression caused by BPH. The period of observation: from 2019 until the May 20, 2022; 48 and 51 patients were primary observed before and 6 months after LRP + CIIA + UCA and TURP. Criteria of inclusion: prostatic volume 60–80 cm3.Pre-operative treatment included such parameters as prostate-specific antigen level. Volume of prostate, volume of residual urine, urodynamic parameters, IPSS (International Prostate Symptom Score) + quality of life (QoL) scale result. Postoperative observation after 6 months included uroflowmetry, residual urine volume, evaluation of lower urinary tract symptoms and their influence on IPSS and QoL scale result. Efficacy of treatment of BPH is a summary of all earlier described parameters.Results. Symptomatic regression in patients who underwent LRP + CIIA + UCA and TURP was found among 15 (93.75 %) and 13 (81.25 %) patients relatively; complications distributed throughout 1, 2 and 3 classes by Clavien-Dindo criteria: after TURP – among 71.4, 22.9 and 5.7 % patients, after LPA + CIIA + UCA – among 35.4, 24.2 and 11.3 %, relatively. After 6 months maximal urine flow improved up to 22.7 ± 2.2 ml/seс, 16.8 ± 1.5 ml/seс, relatively.Conclusion. LRP + CIIA + UCA is effective for urodynamic and life quality improvement like TURP. Despite the characteristic clinical signs, sometimes there are difficulties in diagnosis, which leads to delayed treatment and irreversible damage to the testicle.The article describes a clinical case of testicular torsion in a young patient with errors in diagnosis, which led to the removal of the testicle. As a result of a diagnostic error – an incorrect collected anamnesis and not timely execution of ultrasound dopplerography, conservative treatment for epididymoorchitis was carried out. The given example illustrates the importance of a thorough study of the anamnesis of the disease – the onset and nature of pain, nausea, as well as local symptoms that are not characteristic of other acute testicular diseases. In addition, if a testicular torsion is suspected, it is necessary to perform urgent, sometimes repeated ultrasound dopplerography of the scrotum organs, which is of irreplaceable value for solving the issue of emergency surgical treatment. This study aimed to describe a distinct subpopulation of azoospermic patients with isolated elevation of follicle-stimulating hormone (iFSH) and poor outcomes of microdissection testicular sperm extraction (microTESE).Materials and methods. A retrospective analysis of microTESE outcomes was conducted among 565 patients with non-obstructive azoospermia (NOA). Testicular pathology was assessed by the dominant histological pattern and Bergmann–Kliesch score (BKS). Descriptive statistics were presented for the iFSH subgroup. Inhibin B levels, the sperm retrieval rate (SRR), and BKS were compared in iFSH patients and other NOA patients.Results. The overall SRR was 33.3 % per microTESE attempt. The median BKS was 0.6 (interquartile range 0–2). Of all NOA patients, 132 had iFSH, and microTESE was successful only in 11 of those cases, with an SRR of 8.3 %, while the total SRR in other NOA patients was 38.1 % (p <0.001). iFSH had a sensitivity of 32.1 % (95 % confidence interval 27.4–36.8) and specificity of 94.1 % (95 % confidence interval 90.8–97.5) as a predictor of negative microTESE outcomes.Conclusion.
