Testicular neoplasms (TN) are one of the most common oncological diseases among young men. Currently the sexual function of such patients remains understudied. The aim of the study was to assess the state of sexual function in men with testicular neoplasms before treatment. Material and methods. The cross-sectional continuous single-center study included 35 men with TN. The average age of patients was 34.2–8.2 years (95% CI: 31.4-37.1). The comparison group included 28 healthy volunteers, average age 30.5–4.9 years (95% CI: 28.6-32.4). Some social characteristics of the patient (marriage and children), the state of sexual function (using the IIEF-15 and ICF questionnaires), the level of anxiety and depression (according to the HADS scale), the levels of total and free testosterone, globulin-binding sex hormone (SHBG), follicle-stimulating hormone (FSH), luteinizing hormone (LH) and prolactin were evaluated. Results. Patients in the main group showed a decrease in erection in 10 (28.6%), satisfaction with sexual intercourse in 12 (34.3%), impaired orgasmic function in 2 (5.7%), decreased libido and overall satisfaction in 9 (25.7%). Nonparametric analysis revealed a statistically significant decrease in libido (p < 0.05), satisfaction with sexual intercourse (p < 0.05) and overall satisfaction (p = 0.007) in patients of the main group compared with the control group. Patients with testicular neoplasms had disorders of the mental component – in 12 (34.3%), erection – in 20 (57.1%), neurohumoral – in 25 (71.4%), ejaculatory – in 10 (28.6%) patients. Subclinical depression was observed in 13 (37.1%) and clinically pronounced depression in 11 (31.4%) patients of the main group. Anxiety and depression were statistically significantly more common in the main group (p = 0.0013). The level of anxiety and depression was more pronounced in unmarried (p = 0.013) children-free patients (p = 0.025). Low levels of total testosterone were detected in 13 (39.4%), free testosterone in 12 (38.7%), LH in 11 (33.3%), FSH in 6 (18.7%) patients from the main group. Existing methods for treating erectile dysfunction (ED) do not always provide the necessary therapeutic effect and/or may not be recommended for certain patients. Aim. Evaluate the effectiveness of correcting erectile dysfunction using platelet-rich plasma (PRP) and extracorporeal shock wave therapy (ESWT). Material and methods. 100 men were randomly assigned to 3 groups. Group 1 (n=20) received ESWT treatment on the penis twice a week for 6 weeks. Group 2 (n=40) and Group 3 (n=40) had two visits per week for 6 weeks, involving both ESWT and PRP injections into the penis. In Group 2, PRP activation was performed using ESWT, while in Group 3, it was done using a 10% solution of CaCl2. Patients were evaluated at 0 and 60 days of the study, assessing IIEF-5, SEP, EHS, GAQ, blood testosterone levels, and penile Doppler ultrasound with PGE1. Results. In group 1 IIEF-5 improved from 14.5 (10.5-17) to 19.5 (15.5-21) (СЂ<0.05). SEP changed from 2 (1-2) to 3 (2-4) (СЂ<0.05). EHS improved from 1.5 (1-2) to 3 (2.5-3) (СЂ<0.05). Baseline PSV was 16.3 cm/s (12.2-22.7), at 60 days post ESWT was 24 cm/s (19.4-26.8) (СЂ<0.05) and RI changed from 0.7 (0.7-0.9) to 0.9 (0.8-1) according D-PDU (СЂ<0.05). 14 patients (70 %) noted positive dynamics by GAQ at the last exam. In group 2 IIEF-5 was 13 (11-15) at 0 days, 18 (16-20) at 60 day (СЂ<0.05). SEP improved from 2 (1.5-2) to 3 (3-4) (СЂ<0.05). EHS changed from 2 (1-2) to 3 (2-3) (СЂ<0.05). D-PDU results demonstrated increase median PSV 15.6 cm/s (12.1-22.8) to 27 cm/s (20.6-33.5) (СЂ<0.05) and median RI from 0.8 (0.7-1) to 1 (0.8-1) (СЂ=0.02). 34 men declared positive effects according to GAQ (85 %) In group 3 IIEF-5 results improved from 13 (9-15) to 18.5 (15-20.5) (СЂ<0.05). SEP improved from 2 (1-2) to 3 (3-4) (СЂ<0.05). EHS changed from 1 (1-2) to 3 (3-3) (СЂ<0.05). PSV increased from 17 cm/s (10.3-25) to 27.8 cm/s (20-36.6) (СЂ<0.05). RI improved from 0.8 (0.7-0.9) to 0.9 (0.8-1) (СЂ=0.005). 33 patients respond to an improvement of erectile functions (82.5 %) by GAQ. Conclusion. All treatment methods were well-tolerated by all patients. The study results indicate a positive trend in improving erectile function and increasing total testosterone levels in the blood. When comparing groups, combination therapy significantly improves erectile function according to SEP, EHS, and penile Doppler ultrasound assessments. To determine the role of taxonomic bacterial diversity of the urethral and testicular microbiota in the development of male infertility using high-throughput next-generation sequencing (NGS).Material and methods. In the period between 2018 and 2022 we examined 53 patients. Three study groups were formed: Infertile patients with nonobstructive azoospermia and concomitant varicocele (n = 13) – group 1; Infertile patients with obstructive azoospermia (n = 29) – group 2; Fertile patients with confirmed paternity and testicular pathology requiring histological verification to exclude testicular oncopathology (n = 11) – group 3. Each patient underwent TESE with testicular both and urethral tissue sampling in order to compare the bacterial landscape and control the purity of the method. The obtained material was sequenced using a high-throughput method (NGS).Results.According to our findings, the testicular tissue of obstructive azoospermia patients had significantly depleted bacterial diversity in comparison to nonobstructive azoospermia and concomitant varicocele ones. Also, the fertile group turned out to be the most diverse in its taxonomic community. These results may suggest the bacterial microbiome’s influence on men’s reproductive health.Discussion.Our urogenital bacterial diversity analysis showed that human testicular tissue is not a microbiologically sterile environment and also presented new data associated with testicular tissue and its possible relations with male infertility.Conclusion. Purpose of the work: to conduct a comparative analysis of the microbiota of prostate secretions in patients with CP/CPPS IIIa and IIIb using mass spectrometry of microbial markers (MSMM). Material and methods. A prospective comparative study was conducted from 2018 to 2023 at the SM-Clinic medical center. 205 patients with prostatitis-like symptoms and 70 males who applied for a screening examination with no complaints at the age of 25 to 50 years were selected. The subjects were divided into three groups: group 1 – patients with CP/CPPS category IIIa, group 2 ( n = 85) patients with CP/CPPS category IIIb with clinical manifestations of chronic prostatitis and group 3 ( n = 70) – without clinical manifestations prostatitis/control group. A study of the microbiota of prostate secretions was carried out using gas chromatograph mass spectrometer \"Maestro\" (LLC \"Interlab\", Russia). Results. The results of statistical analysis showed that the level of cocci and bacilli, anaerobes, actinobacteria, fungi and yeasts, as well as total viruses in patients with CP/CPPS IIIa, compared with patients with CP/CPPS IIIb and the control group, was significantly higher ( p = 0.0020 ; p < 0.0001; p < 0.0001; p = 0.0254; p = 0.0254). It was revealed that in patients with CP/CPPS IIIa, compared with patients with CP/CPPS IIIb and healthy people, there was a significant increase in the total load of cocci and bacilli, anaerobes, actinobacteria, fungi and yeasts, as well as total viruses . When analyzing the relationship between the absolute number of microorganisms, it was revealed that an increase in the level of many microorganisms is associated with an increase in the probability of detecting a leukocyte level ≥10. An analysis of the relationship between the occurrence of individual microorganisms and leukocyte values in prostate secretions ≥10 showed that an increase in the indicators of many microorganisms, compared with the control group, increased the chances of having a leukocyte level ≥10 several times. Conclusion. The negative consequences of the pandemic caused by the COVID-19 virus are being actively studied nowadays. It is known that the new coronavirus infection aggressively affects not only the organs of the respiratory system, but also the male reproductive system through binding to the ACE-2 receptor on the surface of cells of reproductive organs. The purpose of the study: to identify the impact of the virus on erectile and reproductive functions in young men of childbearing age 20–45 years who have undergone a new coronavirus infection. Material and methods. The study included 75 men of reproductive age 20–45 years who had undergone COVID-19. During the period of COVID-19 disease and 12 weeks after recovery, the study participants completed a questionnaire to assess erectile function \"International Index of Erectile Function\" (ICEF-5). The men underwent a blood test for hormones: luteinizing hormone (LH), follicle-stimulating hormone (FSH), the ratio of FSH to LH (FSH/LH), testosterone total, prolactin, thyroid-stimulating hormone (TSH), T4 free, prostate specific antigen total (PSA total). The analysis of the ejaculate of 29 study participants was also carried out during the illness and 12 weeks after the recovery from COVID-19. Results. When processing the ICEF-5 questionnaire data, erectile dysfunction was detected in 81% of cases, p < 0,01 during the illness, after coronavirus infection, sexual disorders persisted in 44% of cases, p < 0,01. A study of the hormonal status in 80% of men during the COVID-19 disease revealed a significant decrease in the level of total testosterone, p < 0,01, which remained reduced in 64% of men 12 weeks after recovery, p < 0,01. According to the results of 29 spermograms, 42% of the study participants during the COVID-19 disease had oligospermia, p < 0,01, in 44% of cases oligozoospermia was noted, p<0,01. Clinically, varicocele is found in 15% of the male population, with 35% of men with primary infertility and 70-81% of men with secondary infertility. Uncontrolled, excessive amounts of reactive oxy-gen species (ROS) can cause single- and/or double-strand breaks in sperm DNA, commonly referred to as sperm DNA fragmentation. The purpose of the study was to determine the effect of surgical treatment of varicocele on the level of DNA fragmentation and to develop prognostic criteria for restor-ing the fertility of patients. Materials and methods. The study is based on a comparative analysis of the results of examination and treatment of 93 patients without concomi-tant pathology, with pathospermia, increased levels of DNA fragmentation and varicocele, who received surgical treatment. All patients were followed up for 3-9 months after surgery. The control group did not receive postop-erative therapy - 34 people, the main group (59 patients) received drug re-habilitation - a complex of spermatoprotectors and antioxidants - for 3 months after surgical treatment. Results of the study and their discussion. In patients who received surgical treatment, there was a normalization of the level of DNA fragmenta-tion after 3 months in the postoperative period, on average the positive dy-namics was 21% (from an average fragmentation level of 22.82% to 15.84%, p <0.05), the prognosis for pregnancy in the couple became favorable. An improvement in DNA fragmentation indicators was observed in parallel with an improvement in spermogram indicators in the postoperative period. Progression of left testicular hypotrophy in the postoperative period was not detected. No relapses or other complications were noted. Conclusions. According to the data of electron microscopy ofspermatozoa,two siblings with total asthenotheratozoospermia have a total absence of external and internal dynein axoneme handlesin the flagella ofspermatozoa, indicating the presence of PCD. Both patients had a history of repeated failures ofIVF/ICSI programs. The patients did not have situs viscerum inversus and signs of damage to the respiratory system. The othertwo brothers were healthy and fertile, and both have two children. Non-severe asthenozoospermia was revealed in one of fertile brothers(who isin a consanguineous marriage), and situs viscerum inversus was mentioned in hisson. Whole-exome sequencing revealed a pathogenic variant ofthe c.461A>C in exon 4 ofthe CCDC103 gene in a homo/hemizygous state in both siblings with PCD, in their fertile brother – in a heterozygousstate. Assessment of ejaculate parameters allows to assume possible causes of male infertility and helps to determine further treatment. According to modern literature there is a slight but constant deterioration of ejaculate parameters with time. Purpose of the study: to evaluate semen analysis parameters in patients examined in one medical center. Materials and Methods: retrospective evaluation of semen analyses performed in the same laboratory from January 1, 2017 to October 31, 2023 was performed with a total of 47902 patients included. The ejaculate volume, concentration and number of spermatozoa, the proportion of spermatozoa of different categories of motility, sand the proportion of morphologically normal spermatozoa were evaluated. Statistical processing of data was performed using MS Office and SPSS v26 software packages. Results: the mean semen volume from 2017 to 2023 was 3.5 В± 1.5 mL, sperm concentration was 51.9 В± 39.3 million/mL, progressively motile spermatozoa was 48.2 В± 16.5%, number of progressively motile spermatozoa in the sample was 93.7 В± 94.3 million, motile spermatozoa 55.6 В± 17.4%, total motile spermatozoa was 107.5 В± 105.6 million, mean number of morphologically normal forms was 2.8 В± 1.4% in 2017, 2.2 В± 0.9% in 2023, and 3.5 В± 1.5% during the observation period. Mean pH of ejaculate was 7.6 В± 0.3 with noted tendency to increase (7.38 В± 0.33 in 2017, 7.6 В± 0.38 in 2023). The proportion of samples with azoospermia increased from 2.8% in 2017 to 4.5% in 2023. Mean number of patients with cryptozoospermia was 1.5%.
