Chakravorty","doi":"10.38192/14.2.12","DOIUrl":"https://doi.org/10.38192/14.2.12","url":null,"abstract":"Health services across the world are on the one hand the most regulated of all professions, the workforce planning is the most advanced and the restrictions on licence to practice most stringent, especially across borders- yet the workforce is dependent on the ability of large scale movement of professionals across nation-states. The UK NHS has over 41% of its doctors and a quarter of all nurses who were born / trained overseas. This is not much different to many other advanced health systems. \u0000It is also recognised that patient outcomes, recruitment and retention of healthcare professionals who are in great demand depends on autonomy, job satisfaction and a sense of value, equality and a just environment. \u0000A fundamental tenet of equality, celebrating diversity and inclusion is how organisation or society deals with the immigrant professionals or ‘foreigners’. The ability to correctly pronounce ‘foreign’ names is one of the fundamental and most obvious demonstration of respect one accords to a fellow human. \u0000There is a long standing practice of transfiguring foreign sounding names to anglicised or westernized versions for ease of use. The westernization of foreign names is predominantly imposed by the dominant segments of populations on those considered inferior. There is clear evidence of social inequalities linked to one’s race, ethnicity, immigration and gender. Transfiguring one’s name is a clear example of such discrimination, microaggression and a form of bias. \u0000It is time that we understood the impact of such microaggressions of colleagues and our patients. It should be vital for all professionals to take the time and attention needed to get the pronunciation right and in the process understand the cultural diversity, identity and demonstrate due respect and equality. It is also imperative on those of us with foreign sounding names that we help colleagues navigate the rich heritage of our culture and identity and do so with humility. Despite, increase in psychological distress, migrant HCWs don’t use the existing resources to support their needs. \u0000  \u0000This paper summarises discussions from the workshop focusing on reasons/barriers in uptake of mental health and well-being resources in the UK, the scope of peer support initiatives and its boundaries in supporting colleagues in the UK and India. Zamvar","doi":"10.38192/14.2.9","DOIUrl":"https://doi.org/10.38192/14.2.9","url":null,"abstract":"In June 2021, an Employment Tribunal accepted the claim from the complainant that the UK regulator, the General Medical Council may have discriminated against a doctor based on his race, ethnicity or religion in their processes and outcomes. [1] This is perhaps the first time that such a claim of discrimination based on race, ethnicity or religion has been passed in the UK, hence this is hailed as a ‘landmark judgement’. It is not the first time that doctors have claimed that GMC and its processes demonstrate institutional racism. [2] The Kline report described the existence of ‘In groups and out groups’ determined by qualifications (including by country and within the UK by medical school) and ethnicity (including within BME populations). They found that members of ingroups can receive favourable treatment and those in out-groups are at risk of bias and stereotyping.[3] Amitabha Banerjee, wrote in the BMJ, that overt and covert racism within the medical profession is rarely talked about but existed then, and still, unfortunately, exists now, albeit to a lesser extent, to this day.[4] The case of Hadiza Bawa-Garba, a Paediatric trainee who was struck off the register by the GMC on negligence and then subsequently restored demonstrated discrimination in the processes, as the duty White consultant did not face any disciplinary process. Complaints are more likely to be against BAME doctors, and when they proceed to the regulator or the law courts, they are more likely to lead to more serious punitive measures and guilty verdicts. [5–7] \u0000The debate on whether GMC’s processes were unfair, discriminatory and racist started in 1996.[8] However, even in 2003, the GMC council has debated the issues of institutional racism and expressed its commitment to fairness [9] but little progress has been made even 2 decades later. In its submission to William’s review in 2018, BAPIO had recommended that the review must advise the GMC and the NHS to acknowledge the existence and impact of racial discrimination and make concerted efforts to improve this image nationally and abroad. [10] About the differential outcomes for doctors from minority ethnic backgrounds in examinations, assessments, and recruitment, the GMC is committed to ensuring that medical education and training pathways are valid, fair and justified. [11] \u0000  \u0000The world including the NHS is changing. The Care Quality Commission (CQC) has issued an open statement, together with the NHS Race and Health Observatory, Nursing and Midwifery Council (NMC) and the GMC, calling on healthcare leaders to ensure that health and care staff across the country are protected from racism or any other form of discrimination. In this paper, we examine the attainment gap, causes for these disparities and some recommendations to reduce the gap. \u0000Our review shows that there is significant DA between groups of doctors on the basis of gender, ethnicity, race and country of primary medical qualification. The likely causes are bias, lack of opportunity, poor supervision, mentorship, sponsorship, dichotomous treatment of doctors based on training or non-training status and cultural exclusion. Data is not monitored or reported and there is little organisational accountability. Solutions are likely to include transparent data on recruitment as well as progression for benchmarking, training support for all doctors, initiatives which are sensitive to gender, parental responsibility, cultural heritage, language and robust supervision including mentorship and sponsorship. \u0000This scoping review forms part of the Alliance for Equality in Healthcare Professions project on Differential Attainment chaired by the British Association of Physicians of Indian Origin (BAPIO) and will be integrated into the Bridging the Gap project undertaken by BAPIO Institute for Health Research (BIHR). Bamrah","doi":"10.38192/14.1.6","DOIUrl":"https://doi.org/10.38192/14.1.6","url":null,"abstract":"The British Association of Physicians of Indian Origin (BAPIO) undertook a humanitarian project to come to the aid of several migrant doctors who were in the UK to take the Professional & Linguistic Assessment Board’s (PLAB) part 2 clinical examination, and had become stranded due to the lockdown during the first surge of the COVID-19 pandemic. The BAPIO PLAB stranded doctors’ project started as a serendipitous exercise in the third week of March 2020 with first a handful of migrant doctors, until it reached a peak of 267 doctors from 19 countries, and involved collaboration with multiple voluntary organisations, stakeholders, regulatory agencies and governments. \u0000There was no denying the complexity and intricacy of the demands on the stranded doctors, but it was even more pleasing to witness how the project team were more than ready to meet the challenges. Under the umbrella of BAPIO, the project team doctors, who previously had barely known each other, took on all the challenges - teaching, pastoral support, career advice, writing curriculum vitae, finding food, accommodation and funds for those in need; organising professional support, links with the General Medical Council, the High Commission of India and the U.K. Home Office. The project was concluded on 19 September, with all the doctors either returning home or making a decision to work in the NHS when conditions allowed them to. \u0000It is not possible to know how many passed their PLAB part 2 exams, but we estimated that over 50% did. The weekly virtual meetings, the camaraderie, the scale of the project, and most importantly bringing it to a closure without any major crisis, was only possible through sheer determination, understanding the needs, professionalism, leadership and excellent communication. Healthcare workers from the frontline of defence, yet, the pandemic affects not only healthcare workers but also their families. There is growing awareness of the benefits of COVID-19 support groups, for professionals and for the wider community. Migrant professionals make up a significant proportion of the human resources in delivering health care globally, as in the UK. Therefore any such initiative, should be culturally tailored to address their needs. \u0000Aim: To outline the development of a community support initiative, in the form of a non-profit organisation, for migrant minority Filipino healthcare professionals and their communities. \u0000Method: A mixed-method study with retrospective data collection.  A dedicated helpline was set up during the pandemic. A Plan-Do-Study-Act (PDSA) method was adopted. The cohort was divided into two groups, health care professionals and non-healthcare group, with interventions delivered by four divisions, which were health care, non-healthcare, information governance and public relations - social media divisions. Interactive focus group and webinars were facilitated and data collected via Emotions Behind the Mask questionnaire. \u0000Results: PDSA outcomes, the dedicated helpline responded to 40 phone calls in 6 weeks, calls were linked to basic needs of daily living or work-related issues. In 5 months, the organisations collaborated with 25 local, national, and international organisations delivering community and professional support. The online platform audience reached an audience of 27,795. \u0000Conclusion: The combined professional healthcare and community support initiative demonstrated an impact on healthcare staff with possible benefits in workforce retention.
