Wenhong","doi":"10.3760/CMA.J.ISSN.0254-5098.2019.10.014","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.0254-5098.2019.10.014","url":null,"abstract":"Objective \u0000To standardize the operation and data processing in radiological health institutions in China, in order to promote the assessment of γ-spectrometry, improve the measurement and analysis capabilities of each laboratory, and accordingly improve accuracy of environment sample measuring. \u0000 \u0000 \u0000Methods \u0000By collecting the values of 232Th, 40K and 137Cs contents in soil samples from 2015 to 2017, the assessment results of γ-spectrometry laboratores were summarized and analyzed. \u0000 \u0000 \u0000Results \u0000The technical institutions from 25 provinces were involved in the assessment until 2017, including disease control and prevention centers, entry-exit inspection and quarantine agencies, research institutes, universities and companies. The number of participants was increased from 40 to 48, and the qualified rate exceeded 90%, and the excellent rate was no less than 11% each year. The percentage of relative deviation less than 10% between the measuring result of 232Th, 40K and 137Cs contents and the reference value was larger than 80% in 2017. The accuracy became better. However, the number of institutions with U and Z score less than 1 was increased from 11 to 25, suggesting that the precision of measuring result has also been greatly improved. \u0000 \u0000 \u0000Conclusions \u0000Radiological health institutions in China have good measurement and analysis capabilities with regard to γ-spectrometry, which can meet the needs of environmental sample measurement and in-food radionuclide risk monitoring in an emergency of nuclear and radiation. The distribution of using CT scans was analysed according to the age, gender, anatomical regions and the duplicated cases of the patient, then estimated the paediatric CT scan rates of four hospitals in Shanghai. \u0000 \u0000 \u0000Results \u0000Retrospective review of RIS showed 249 350 CT examinations from 175 672 young patients were performed between 2011 and 2014 in the surveyed hospitals. More males (62%) were scanned than females (38%). The majority of children receiving the examinations were 1 to 5 years old, which amounted to 40.3% in all. Cardiac CT accounted for 14.5% among all children of ≤1 year old CTs and accounted for 63.0% among all cardiac CT scans. There were 33 634 (19.1%) individuals who had received 2 or more CT scans during the four years. The system consists mainly of information filling, data review, retrieval query, graphic report and data export. \u0000 \u0000 \u0000Results \u0000Since 2016, a network submission method was used for the first time in the national food radioactive material risk monitoring. The system improved the timeliness and validity of data reporting and provided a more effective means than before for the normative management and scientific application of data. \u0000 \u0000 \u0000Conclusions \u0000The national food radionuclides monitoring network was successfully constructed and in use. In the past 4 years, the system has been running normally and 5 business databases have been established. More than 8 million data of monitoring and survey have been collected. The national radiological health information platform has been established, covering 5 information systems including individual dose, occupational radiological diseases and occupational health, medical radiation protection, food radionuclide monitoring and occupational radiological disease reporting. Up to 2018, more than 8 million pieces of monitoring data have been collected, and more than 1 400 users have been authorized. In this paper, the current situation of radiological health informatization construction in the past 10 years in China was analyzed. Data were acquired by Philips 128iCT, and CT dose index (CTDIvol) and dose-length product (DLP) were recorded and compared among three groups. Methods In accordance with the relevant regulations, standards and literatures available in China, the system was designed and implemented to survey the requirements of diagnosis institutions of occupational radiation sickness and administrative departments. Results The system was initially established with expectant functions. The system functional modules consisted of case reporting, case information enquiry, statistical analysis and system management. The diagnosis institutions of occupational radiation sickness had used the system to report the cases diagnosed in 2017. Conclusions The system achieved the standardized case reporting for the institutions of occupational radiation sickness, and provided scientific data for the health surveillance of radiation workers and decisions of governments. Animal tissue was used to simulate subcutaneous adipose tissue of patients, and the HU and RSP of the animal tissue were measured. The effect of subcutaneous adipose tissue on conversion between HU and RSP were analyzed by piecewise fitting. \u0000 \u0000 \u0000Results \u0000The precision of conversion curve was improved significantly with the measured HU and RSP of adipose tissue in the fitting. All patients received radical radiotherapy with or without concurrent chemotherapy. Short-term and long-term efficacy and related prognostic factors were observed. Kaplan-Meier method was used for survival analysis, Log-rank test was used for univariate analysis, and Cox proportional hazards regression model was used for multivariate analysis. \u0000 \u0000 \u0000Results \u0000The 2-year overall survival (OS) and disease free survival (DFS) were 83.4% and 72.5%, respectively. During the follow-up periods, 46 patients (21.8%) died, including two from non-tumor-related diseases, and one from second primary colon cancer. Totally 57 patients (27%) had recurrence and metastasis, including 16 (28.1%) with local recurrence, 27 (47.4%) with distant metastasis, and 14 with local recurrence and distant metastasis(24.6%). Univariate analysis showed that 2-year OS and DFS were significantly correlated with pathological type, pre-treatment squamous cell carcinoma antigen (SCC) value and FIGO stage (OS: χ2=7.123, 6.014, 8.398, P<0.05; DFS: χ2=11.832, 8.003, 7.731, P<0.05). In addition to the above factors, 2-year DFS was also associated with pelvic lymph node metastasis (χ2=9.286, P<0.05). Multivariate analysis showed that pathological type, pre-treatment SCC value and FIGO stage were independent prognostic factors of OS(HR=2.963, 2.473, 2.574, P<0.05).
