All India Institute of Medical Sciences Raipur started the dedicated COVID service facility, and the workforce and logistics were shifted to the new dedicated service. This has impacted the inpatient services provided, including psychiatry services. Hence, we conducted a retrospective record review of inpatient admission during COVID-19, which was compared with the previous year’s admission rates. It is characterized by complex and vivid visual release hallucinations with insight into the unreality of visual experiences, which cannot be explained by any other psychiatric or neurodegenerative disorder. Most often these hallucinations are visual only without hallucinations in other sensory modalities. Here, we report an atypical case of CBS in an 80-year-old male who presented with complex visual scenic, auditory, and olfactory hallucinations after visual loss secondary to glaucoma with no insight and no cognitive decline. Evidence suggests that there are millions of people living with dementia and this number is ever increasing as the younger age mortality is on the decline. The various causes of dementia can be diagnosed by a thorough medical history, a general and systemic examination, laboratory investigations, and neurological imaging. Although cognitive impairment is required and even sufficient for the diagnosis of dementia, the disease may also be accompanied by neuropsychiatric symptoms known as behavioral and psychological symptoms of dementia (BPSD), which are widespread and have a significant impact on the disease’s prognosis and treatment. In this case report, we will be discussing about a novel approach undertaken when managing the BPSD symptoms of two patients older than 90 years of age. The patients were admitted to the inpatient department of a tertiary care hospital. This comorbidity has potentially serious consequences including suicide attempts, hospitalization, and rapid cycling. Endoxifen could play a role in the management of BI based on the inhibition of protein kinase C, which also has a role in substance abuse. We present two cases of patients with bipolar mania and comorbid substance use disorder, who were effectively and safely treated with adjunct Endoxifen. The addition of Endoxifen addition reduced manic symptoms as well as substance use. Furthermore, the inclusion of Endoxifen permitted the use of lower doses of other drugs in one patient and reduced the pill burden in the other patient. However, the efficacy of repetitiver TMS in acute phase mania is unclear. The aim of this study is to assess the efficacy of theta burst stimulation (TBS) as an add-on therapy in the treatment of acute phase mania stimulation both right and left dorsolateral prefrontal cortex (DLPFC) in a randomized and sham-controlled design.\u0000\u0000\u0000\u0000Forty right-handed patients between 18 and 59 years were randomly allocated to active and sham groups. Then, intermittent TBS (right DLPFC) and cTBS (left DLPFC) sessions (half an hour apart) were delivered in two sessions per day for 5 days in a week. Youngs Mania Rating Scale (YMRS) and Clinical global impression-Bipolar Disorder (CGI-BP) were assessed at baseline and at 7th and 21st days.\u0000\u0000\u0000\u0000Our study fails to show a significant effect on of active stimulation in comparison to sham over psychopathology YMRS (F = 0.300; P ≤ 0.612), CGIBP-S (F = 0.432; P = 0.562), CGIBP-P (F = 0.202; P = 0.701), and CGIBP-W (F = 200; P = 0.705) in intention to treat protocol across 21 days. Minimal side effects were reported and none of the patients discontinued TBS citing side effects.\u0000\u0000\u0000\u0000We conclude that it is safe and well tolerated yet has inconclusive short-term therapeutic benefits.
