[Functional and aesthetic significance of the uvula in cleft palate repair]. [悬雍垂在腭裂修复中的功能和美学意义]。

The morphological investigation of the resected hemi- uvula was done. Palatoplasty was performed in all cases.</p><p><strong>Results: </strong>According to morphological results, most of the resected hemi-uvula consisted of vascularized fibrous tissue, covered with epithelium. In three groups of patients (with unilateral, bilateral and isolated cleft palate), the duration of the surgery and intraoperative blood loss did not exceed similar values for conventional methods. The volume of infusion therapy revealed a deficit of fluid intake of no more than 30%, which indicates early restoration of swallowing function.</p><p><strong>Conclusion: </strong>The technique of preserving one of the «hemi-uvulas» lead to excellent aesthetic results and increasing functionality. Resection of one of the «hemi-uvulas» is safe and physiological.</p>","PeriodicalId":35887,"journal":{"name":"Stomatologiya","volume":"102 6. (Experimental and clinical study)].","authors":"S V Tekucheva, F F Losev, A G Nadtochiy, S N Ermoljev, A A Fokina","doi":"10.17116/stomat202310206244","DOIUrl":"https://doi.org/10.17116/stomat202310206244","url":null,"abstract":"<p><strong>Purpose: </strong>To describe the ultrasound anatomy of the masticatory muscles based on a comparison of the results of macroscopic and ultrasound studies of the masticatory muscles in vitro.</p><p><strong>Material and methods: </strong>In the experimental part, a macropreparation of the masticatory muscle was studied; in a clinical example, an ultrasound of the masticatory muscles was performed on a patient with normal occlusion using the author's methods for analyzing echograms.</p><p><strong>Results: </strong>The ultrasound anatomy of the masticatory muscles is described based on a comparison of data from the study of a macroscopic specimen and ultrasound images of the masticatory muscle of an experimental animal. Using a clinical example of a patient with normal occlusion, the results demonstrate the usage of the authors' developments in describing ultrasound images of the masticatory muscles.</p><p><strong>Conclusion: </strong>The study resulted in an algorithm for describing ultrasound images of masticatory muscles, including methods for qualitative and quantitative assessment of ultrasound images using proprietary developments involving elements of artificial intelligence technologies.</p>","PeriodicalId":35887,"journal":{"name":"Stomatologiya","volume":"102 6. {"title":"[Dental gel Cholisal at the stage of conservative treatment of inflammatory periodontal diseases].","authors":"I M Rabinovich, N B Petrukhina, E V Shikh, V M Polyakov, I V Sandler","doi":"10.17116/stomat202310205134","DOIUrl":"10.17116/stomat202310205134","url":null,"abstract":"<p><strong>Purpose of the study: </strong>To study the effectiveness of using the drug Cholisal as part of the conservative treatment of chronic periodontitis.</p><p><strong>Material and methods: </strong>We selected 100 patients aged 35 to 65 years of both sexes with a diagnosis of moderate chronic periodontitis in the acute phase with a periodontal pocket depth of 3.5-5 mm. Depending on the tactics of conservative treatment of CGP, patients were divided into two groups of 50 people. In the main group, in addition to standard treatment, the dental gel Cholisal was used, and in the control group, therapy was standard. 10 days after professional hygiene, patients in both groups were examined and underwent an index assessment of the condition of periodontal tissues and adherence to treatment.</p><p><strong>Results: </strong>In patients of the control group, 10 days from the start of treatment, the depth of periodontal pockets in the control group decreased slightly from 4.7±0.28 mm to 4.2±0.21 mm (<i>p</i>=0.074), and the Green-Vermillion hygiene index decreased by 25.3±1.79% (<i>p</i>=0.041), Silnesse-Loe plaque index by 59.1±2.16% (<i>p</i><0.001), PMA index by 51.5±1.92% (<i>p</i><0.001) and Muhlemann-Cowell bleeding index by 42.2±1.75% (<i>p</i><0.001). In the main group, the effectiveness of treatment of chronic periodontitis with conservative therapy using Cholisal was higher. According to the results of a literature search in PubMed and RINC, 30 articles were found and selected, including 288 flaps from the iliac region. The length of the vascular pedicle, the diameter of the vessels, the complications, the prevalence of the flap in the reconstruction of the head and neck, the thickness and the size of the flap were evaluated. Both of these flaps have the same nutrition, but different levels of dissection. In comparison with the groin flap, the SCIP flap has a longer vascular pedicle, which can be enlarged due to new techniques, which also allows it to be used as an ultra-thin flap, and it reduces the need for secondary procedures for defatting and reduces the risks of the complications in the donor area. The thickness of the SCIP flap is less than the groin one, which, according to the author, increases the aesthetic level of operations in the facial area in the case of the texture and color of the flap. Vascular anatomy of the SCIP flap is variable, therefore, the use of preoperative planning is necessary. Despite this, complications in the donor or recipient area during reconstruction with a SCIP flap are minimal. It has been proved that the SCIP flap is versatile and safe in all aspects of reconstruction. {"title":"[Microanatomy features in third molars roots adjacent to mandibular canal].","authors":"A V Kuzin, A P Vedyaeva, V V Sogachyova, A N Gurin, G V Remizov","doi":"10.17116/stomat202310206116","DOIUrl":"10.17116/stomat202310206116","url":null,"abstract":"<p><strong>The purpose: </strong>Of the study is to reduce the risk of postoperative neuropathy of the inferior alveolar nerve by improving diagnostic methods, assessment of individual topographic and anatomical features and extraction technique of impacted teeth adjacent to the mandibular canal.</p><p><strong>Materials and methods: </strong>According to the CBCT examination, orthopantomography and macroscopic examination of removed third molars roots (<i>n</i>=140) the relative position of the mandibular canal and the roots of the third molars were studied.</p><p><strong>Results: </strong>Three variants of close mandibular canal and third molars position have been identified. In the lateral and apical nerve position, the root surface depressions were detected. With inter-radicular position fit, the mandibular canal and the nerve bundle form a «bed» in between impacted tooth roots.</p><p><strong>Conclusion: </strong>The injury of neurovascular bundle prognosis during extraction with an interadicular mandibular position depends on roots anatomy and their convergence degree. The advantages and disadvantages of such fastening systems are described. The analysis and systematization of available data on medical adhesive compositions that can be used in maxillofacial prosthetics to hold facial epitheses has been carried out. Much attention is paid to the chemical composition of pressure-sensitive adhesives, water-based and silicone-based. Examples of such compositions are given. The physicochemical properties of medical adhesive systems that cause adhesive-cohesive interaction are described. Patients and dentists are at high risk of cross-infection due to the spread of aerosols in the form of droplets suspended in the air. The ultrasonic scaler is the largest source of aerosols and spatter in the dental office. Aerosols remain in the air for a long time even after the completion of a dental procedure and have a potential risk of inhalation.</p><p><strong>Purpose of the study: </strong>To evaluate the spatial distribution of aerosols and splashes during a dental appointment during ultrasonic dental treatment.</p><p><strong>Material and methods: </strong>The study was conducted on a mannequin equipped with phantom jaws, where dental plaque removal was simulated using an ultrasonic scaler filled with fluorescein. The amount of contamination was measured using a transparent grid with a square marking of 1 cm<sup>2</sup>. The grid was placed on top of a disk of filter paper and the area of contamination was measured by counting the number of soiled squares. If a square had at least 1 yellow area, then it was considered polluted.</p><p><strong>Results: </strong>Aerosols and splashes were distributed up to 60 cm from the head restraint. Aerosols can spread over more than 1 m.</p><p><strong>Conclusions: </strong>The study showed the spread of aerosols and splashes during dental appointments. The authors analyzed the established knowledge about clinical and morphological changes at various stages of eruption of the lower third molars, studied new data describing the pathogenetic, histological and biochemical features of their development, packaging, movement to the occlusal plane.
