Group 1 consisted of 5 patients with the diagnosis of «Upper micrognathia», group 2 included 5 patients with «Upper micrognathia after a series of surgeries for unilateral cleft lip and palate» and group 3 comprised 5 patients with diagnosis of «Upper micrognathia after a series of surgeries for bilateral cleft lip and palate». Movement of the maxillary fragments was measured 3 and 6 months after activation using cone beam computed tomography and intraoral scanner data. The data was analyzed using specialized software Amira 5.4.5 to create a 3D-model of the upper jaw. This allowed for precise measurement and analysis of the movement of the maxilla over time.</p><p><strong>Results: </strong>Despite the initial goal of surgically assisted maxillary expansion being to correct transverse deficits it has been found that spatial changes occur in all planes and axes. Patients with upper micrognathia resulting from bilateral cleft lip and palate are more likely to experience recurrence of displacement of the fragments of the upper jaw following expansion, compared to patients with a unilateral cleft during the postoperative period. The results of two-factor analysis of variance confirmed a statistically significant dependence of the average values of O and C content in enamel over the centuries (<i>p</i><0.0001), the average values of P, Zn, O and Mg in dentin (<i>p</i><0.0001), the average values of O and C content in cement (<i>p</i><0.0001). After 3 months, restoration of the function of the damaged nerve is observed in 84%. Permanent paresthesia lasting a year or more was detected in 4 patients (1.4%).</p><p><strong>Conclusion: </strong>The main causes of nerve trauma during surgery to remove an impacted tooth were identified. {"title":"[Influence of the crystal index on the formation of tooth enamel resistance].","authors":"I L Gorbunova, D V Penkov","doi":"10.17116/stomat202510403111","DOIUrl":"10.17116/stomat202510403111","url":null,"abstract":"&lt;p&gt;&lt;strong&gt;Objective: &lt;/strong&gt;The purpose is to substantiate the prospects of calculating the crystallinity index of tooth enamel as a criterion of individual potential risk of dental caries.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Material and methods: &lt;/strong&gt;120 enamel samples of intact premolars of the upper jaws were examined, removed according to orthodontic indications in persons aged 18-35 years living in Omsk, previously genotyped by polymorphisms G/G, G/A and A/A of the kallikrein-4 gene at mutation points G2664153A and G2142A. The following parameters of the oral fluid of the examined were analyzed: pH, Ca&lt;sup&gt;2+&lt;/sup&gt; using ion-selective electrodes on the pH meter OP2/208/01 and the potentiometer EV-74, the product of the solubility of hydroxyapatite (mineralizing potential) and the degree of oversaturation with hydroxyapatite was calculated, the protein concentration in the oral fluid was determined by the method using a biuretic reaction. The crystallographic features of hydroxyapatite of tooth enamel were studied using transmission electron microscopy methods using the JEM-2100 «JEOL» apparatus (accelerating voltage 200 kV, crystal lattice resolution 0.14 nm) and infrared spectroscopy using the Nicolet- 5700 «Thermo Electron Corp.» apparatus (the number of spectra accumulation is 30cm-1, resolution is 4cm-1). Based on the data obtained, the crystallinity index of tooth enamel is calculated. Protein determination in tooth enamel was carried out according to the Barnstein method.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Differences in the values of the crystallinity index of intact tooth enamel of individuals were revealed, which is due to a certain degree of ordering of the structure and relative size of crystallites in the studied samples of tooth enamel. With the crystallinity index of enamel samples equal to 3.54, the relative size of the crystallites and the degree of ordering of the crystal lattice of hydroxyapatite are higher than enamel samples with a more defective crystallite structure, the crystallinity index of which is 3.27. With a crystallinity index equal to 3.27, there is a predominance of pathological homozygote A/A of the kallikrein-4 gene in both studied mutation points, which is accompanied by a significant increase in the mass fraction of enamel protein, as well as a number of indicators of the supraventricular oral fluid involved in mineralization processes: phosphorus content, active concentration of calcium, protein, the product of hydroxyapatite solubility and the degree of oversaturation hydroxyapatite.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Conclusion: &lt;/strong&gt;The crystallinity index of tooth enamel, being an integral indicator of the geometric structure of apatites, can serve as an indirect criterion for determining the resistance of tooth enamel. {"title":"[Targeted bone regeneration in the treatment of patients with chronic generalized periodontitis].","authors":"M B Khaykin, A L Savelyev, I M Bayrikov","doi":"10.17116/stomat202510404133","DOIUrl":"10.17116/stomat202510404133","url":null,"abstract":"<p><strong>Objective: </strong>To determine the volume of augmentation necessary for adequate replacement of the alveolar bone defect in chronic generalized periodontitis, considering the dynamics of changes in the physical properties of its components in the postoperative period.</p><p><strong>Material and methods: </strong>The study analyzed the results of surgical treatment of 197 patients with chronic generalized periodontitis of moderate severity. In the main group (<i>n</i>=97), treatment was performed using the author's method for determining the volume of an osteogenic graft; In the control group (<i>n</i>=100), the required volume of augmentation was determined by previously known methods. To repair the defects of the alveolar bone in the patients of the main group, we used our proposed multicomponent osteogenic graft with the calculation of the volume of fractions of granular material according to the coefficient of natural compaction and the coefficient of saturation with liquid to prevent uncontrolled changes in the shape and volume of the graft in the recipient's bed in the postoperative period. {"title":"[The significance of using the method of shared decision-making in preparing patients for orthognathic surgery].","authors":"A T Galazov, A Yu Drobyshev, N A Redko, T M Dibirov, V M Mikhailyukov, A A Miterev","doi":"10.17116/stomat202510404126","DOIUrl":"10.17116/stomat202510404126","url":null,"abstract":"<p><strong>Objective: </strong>Evaluation of the effectiveness and impact of the shared decision-making method on patient satisfaction with the quality of preoperative preparation for combined (orthodontic and surgical) treatment.</p><p><strong>Material and methods: </strong>The study included 97 patients with skeletal forms of distal and mesial occlusion, who were prepared for combined (orthodontic and surgical) treatment using the shared decision-making method. The effectiveness of the shared decision-making technique used in preparing patients for combined treatment was assessed by means of a questionnaire developed.</p><p><strong>Results: </strong>On the 2nd day after surgery, a patient survey revealed 25 different complaints related to the clinical manifestations of the effects of orthognathic surgery and rehabilitation features. The most frequently reported complaints were difficulty breathing (60.8%), swelling in the face (38.1%), difficulty eating (37.1%), bimaxillary splinting (21.7%), weakness/drowsiness (20.6%), numbness (19.6%), facial pain (15.5%). {"title":"[Radiological assessment of long-term outcomes of restorative treatment in patients with shortened dental arches].","authors":"F F Losev, A G Nadtochii, T V Braylovskaya, M S Kotik","doi":"10.17116/stomat202410302150","DOIUrl":"https://doi.org/10.17116/stomat202410302150","url":null,"abstract":"<p><strong>Background: </strong>Restoring the integrity of the dentition with orthopedic structures should be carried out strictly according to indications, taking into account the condition of the supporting teeth and/or teeth limiting the defect.</p><p><strong>Objective: </strong>To conduct a retrospective analysis of the results of orthopedic treatment of patients with shortened dentition according to clinical and radiological methods.</p><p><strong>Materials and methods: </strong>The material for the study was data from cone beam computed tomography (CBCT) of 126 patients with shortened dentition (most patients with the absence of a second molar, as well as the absence of the first and second molars), who sought a consultation about missing teeth in the orthopedic and surgical department of the Federal State Budgetary Institution National Medical Research Center TsNIISiCHLKh Ministry of Health of Russia.</p><p><strong>Results: </strong>At the stages of treatment for patients in the study group, insufficient attention is paid to the restoration of terminal defects of the dentition in the upper and lower jaws, especially the restoration of second molars. This may be due to improper planning of dental treatment and insufficient motivation of the patient to carry out comprehensive dental rehabilitation. According to CT studies, the number of complications of orthopedic treatment in the area of supporting teeth and/or teeth limiting the included defect or terminal defect of the dentition in the masticatory region increases depending on the period of use of the orthopedic structure.</p><p><strong>Conclusions: </strong>The use of cantilever structures leads to functional overload of the supporting teeth.
