Most screening facilities are located in the tertiary hospitals, which are mostly not readily accessible to patients because of fear and bureaucracy of accessing services at these tertiary hospitals. In Nigeria, cervical cancer is the second most common female malignancy after breast cancer and national guidelines on effective screening is not readily available. At the general outpatients′ Clinic, University College Hospital, Ibadan, screening is not well-established and patients are referred to the gynecology clinic for papanicolaou (Pap) smear, which result in poor compliance as a result of the protocol involved. Aim: To asess the prevalence and risk factors for abnormal cervical cancer screening amongst patients of the Primary Care Clinic at UCH, Ibadan, towards justifying expansion of cervical cancer screening service points in tertiary health institutions in Nigeria. Subjects and Methods: A cross-sectional study using the self-administered structured questionnaire on questions relating to the socio-demographic characteristics and lifestyle behavior including sexual history, after which Pap smear and visual inspection of the cervix with acetic acid were performed. Two hundred and eighty consented women aged 20-65 years were interviewed by one of the researchers between June and August 2012 using interviewer-administered structured questionnaires. The questionnaire sought information on their sociodemographic characteristics, and lifestyle behavior including sexual history, after which Pap smear and visual inspection of the cervix with acetic acid were done, respectively, for all the respondents at no cost. The results were analyzed using Statistical Package for Social Sciences (SPSS) version 11. Cross-tabulation was used to explore statistical associations between categorical variables. Variables were summarized using frequency distribution, means, proportions, and findings were presented in tables, histograms and pie charts. Chi-square statistic was used to assess the association between categorical variables. The level of significance was set at P < 0.05. Result: The mean age of the women was 42.5 (11.1) years, and their modal age was 30-39 years. The majority, 92.2% (258/280) of the women were Yoruba. Two hundred and twenty-eight (81.4%) of the women were married with majority 149 (65.4%) in the monogamous union. Two hundred and thirteen (76.1%) of the women had abnormal cervical cytology. One hundred and sixty-six (77.9%) of the women with abnormal cytology had inflammatory smears, 33 (15.5%) and 6 (2.8%) had mild and moderate dysplasia, respectively, while the remaining 8 (3.8%) had atypical cells. Aim: The study was aimed at determining the rate of all gynecological procedures performed; the common gynecological procedures and their indications, and the cadre of surgeons that performed different gynecological procedures within the period under review. Materials and Methods: A 1-year retrospective chart analysis of all gynecological procedures performed at Aminu Kano Teaching Hospital (AKTH) between 01/10/2012 and 30/09/2013 was conducted. Patients with complete relevant information in the registers were included in the audit and those with incomplete data were excluded. Data was further cleaned and analyzed using Microsoft Excel for Mac 2011, for frequencies and percentages. Results were presented by simple statistical tables. Results: A total of 6,604 patients were gynecological attendees, out of which 646 patients had gynecological procedures performed, giving an institutional gynecological procedure rate of 9.8% (646/6,604), within the period under review. Emergency gynecological procedures accounted for 5.9% (38/646), while the elective cases accounted 4.0% (26/646). Majority of the gynecological procedures was among the 20-40 years age group. About 20 different types of gynecological procedures were done and manual vacuum aspiration (MVA), for incomplete miscarriage accounted for 58.8% (380/646), while excision of transverse vaginal septum was the least at 0.3% (2/646) of all the gynecological procedures. Interns, registrars, senior registrars, and consultants were involved in performing the different gynecological procedures. Between 69 and 100% (446/646-646/646) of some of the major gynecological procedures were carried out by consultants, 14-27% (90/646-174/646) by senior residents (SRs), while the junior residents and interns performed only MVAs among the gynecological procedures at 53.1% (343/646) and 7.3% (47/646), respectively. Conclusion: The common gynecological procedure performed in AKTH is MVA by all cadres of surgeons, and consultants performed the highest number of all the gynecological procedures except MVA. A more regular audit of services rendered by the department is advocated. Aim : The objective of this study was to determine the pattern and relative frequencies of gynecological cancers as seen at the Federal Teaching Hospital, Abakaliki, Nigeria. Materials and Methods : A 2-year retrospective study of female genital tract malignancies was conducted at the Federal Teaching Hospital, Abakaliki. The case notes of patients admitted for female genital tract malignancy between January 1, 2012 and December 31, 2013 were retrieved from the ward admissions and discharge books and the operating theater record books. The data were analyzed using Statistical Package for Social Science (SPSS), IBM SPSS statistics Version 20, IBM incorporation and licensors 1989,2011 New York USA and the results expressed in descriptive statistics by simple percentages. Result: A total of 1,178 women were seen during the study period, while 99 were found to have gynecological malignancy. The proportion of gynecological malignancies was 8.4%. Majority of the patients were in the 5 th and 6 th decades of life, most (60.6%) had cervical cancer, followed by ovarian cancer (19.2%), endometrial cancer (10.1%) vulva cancer (7.1%), and the least was choriocarcinoma (3.0%). Tumors of fallopian tube and vagina were not seen during the study period. Majority of the patients presented late and the common presentation was vaginal bleeding. Conclusion : Despite the preventable nature of cancer of cervix, it remained the most common female genital tract malignancy in Abakaliki, southeast Nigeria. Aim: The aim was to investigate whether the presence of maternal polymorphism Glu298Asp in exon seven of the eNOS gene increase the risk of RSA in Egyptian women. Subjects and Methods: Hundreds women were randomly selected as the case group. They had at least three RSA before 20 th weeks of gestation, same partner and at least one live birth and compared with 100 women, same age range, with no history of abortions or complicated pregnancy as control group. All were investigated for the polymorphism using the polymerase chain reaction-restriction fragment length polymorphisms method. Data were expressed descriptively as percentages for qualitative values and mean ± standard deviation for quantitative parametric data and comparison of qualitative data was done. Results: Frequency of GG genotype 50/100% in cases and 67/100% in control group. Heterozygous GT frequency was 46/100% in cases and 26/100% in control (P ≤ 0.01, odds ratio [OR] =2.37, and 95% confidence interval [CI] =1.30-4.34). Homozygous TT was 4/100% in the cases and at 7/100% in control (P = 0.68, OR = 0.77, and 95% CI = 0.21-2.76). GG genotype versus GT and GG (P = 0.01, OR = 2.03, and 95% CI = 1.15-3.60). Cases with TT genotype were more susceptible to abortion at an older age with a mean of 29 (4.76) (P = 0.02). {"title":"Male infertility in Nigeria: A neglected reproductive health issue requiring attention","authors":"P. Male factor infertility is responsible for about 40-50% of all infertility cases. Despite its high prevalence in Nigeria, not much effort has been made at tackling the problem. The impact of male factor infertility is likely to increase if adequate measures are not taken. This paper reviews the main factors that are responsible for the problem and also highlights the need to focus on prevention and management; how those affected could be assisted by government, agencies and the private sector. Internet search of studies on male infertility was done, and those relevant for this study were reviewed. The major causes of infertility in Nigeria are sexually transmitted infections and hormonal abnormalities. Effort should be made in arriving at a proper diagnosis, and adequate treatment given where causes are treatable. Otherwise the patients should be adequately counseled. In irreversible cases, assisted reproductive techniques may be suggested. This procedure as at now is beyond the reach of the average Nigerian citizen. Aim: To study the correlation of body fat and blood lipids with autonomic nervous system activity in PM Indian women. Subjects and Methods: A total of 60 PM sedentary women without any gross systemic disease were selected as a control group and 23 PMW, who were regularly doing morning walk for 1-year was selected as a study group. Body mass index (BMI), waist/hip (W/H) ratio were recorded; lipid profile analyzed, and conventional autonomic function tests were carried. Results were analyzed using SPSS version 16. Results: There was a significant difference in BMI, W/H ratio, lipid profile, autonomic function tests between the two groups. Aging and menopausal duration (MD) were positively correlated with pulse rate, systolic blood pressure, total cholesterol (TC), low-density lipoprotein (LDL) cholesterol, orthostatic tolerance test (OTT), and negatively correlated with Valsalva ratio (VR), deep breath test (DBT), 30:15 relative risk (RR) intervals ratio, isometric hand-grip test (HGT), cold pressor test (CPT). BMI and W/H ratio were positively correlated with TC, triglyceride, LDL cholesterol, OTT, and negatively correlated with isometric HGT, CPT. TC and LDL were negatively correlated with VR, deep breath difference test, 30:15 RR interval ratios, isometric HGT, CPT, and positively correlated with OTT. Multiple regression analysis revealed LDL cholesterol as the only significant independent explanatory variable of DBT. LDL cholesterol and age were the significant independent explanatory variables of 30:15 RR interval ratios, CPT. TC, LDL cholesterol, and age were the significant independent explanatory variables of isometric HGT. Conclusions: Aging, MD, BMI, and W/H ratio all contribute significantly to worsening of autonomic functions and lipid profile. Pathological examination showed dilated fallopian tube containing yellow material. Microscopic examination showed Aspergillous filaments surrounded by dense infiltrate of neutrophils and lymphocytes. Even in a single class of tumor, there exists inherent heterogeneity with biological behavior ranging from benign to the highly aggressive malignant tumor. The management of the patient also depends on the histological type of the tumor. These facts fascinated and prompted us to undertake the present study. Aim: To analyze the modes of presentation and various histopathological patterns of ovarian tumor. Materials and Methods: It was a retrospective observational study. The study was conducted in Department of Pathology, B. J. Medical College Pune, India from July 2006 to June 2011. All the histopathology slides of ovarian tumors during the study period were retrieved and reviewed along with the patient′s demographics, clinical features, and gross findings. Data thus collected were analyzed. Results: A total of 226 cases of ovarian tumors out of 1098 cases of female genital cancers were studied. Age ranged from 12 to 80 years. The surface epithelial tumors were the most common ovarian tumor constituting 163 cases (72.1%), followed by germ cell tumors 45 cases (19.9%). The most common complaint in the present study was pain in the abdomen (115 cases, 50.9%) irrespective of the nature of the ovarian tumor. Bilaterality was common in malignant tumors (66.7%, 16/24). Right and left side was almost equally affected among unilateral tumors. The size of the tumor varied from 3 to 32 cm. Aim: The aim was to determine efficacy of AmniSure rapid immunoassay placental alpha-microglobulin-1 test for accurate diagnosis of true ROM in women with watery discharge after 28 weeks gestation, compare with conventional methods. Subjects and Methods: Study was carried out in women presenting with ROM to labor the room of referral rural medical institute in Central India after ethical approval. Prospective study was performed in women presenting with symptoms/signs of ROM after 28 weeks of gestation. Sterile speculum examination was performed to observe pooling of liquor. Nitrazine, ferning tests were done to diagnose ROM. Vaginal examination was performed to determine cervical dilatation, effacement, station of presenting part in term cases. If all or 2 of 3 tests (pooling, ferning, and nitrazine) were positive, provisional diagnosis of ROM was made. Confirmation of ROM was done at birth. However, if 2 of 3 tests were negative, sterile speculum examination was repeated after 30 min of the first test. Test performance was calculated by comparing AmniSure results against final diagnosis at birth. Of 200 patients between 28 and 42 weeks gestation recruited for study, 31.5% (63/200) were preterm, 68.5% (137/200) term. Statistical analysis of data collected in the electronic database using SPSS version (Amnisure International LLC, 30 JFK Street, 4th Floor, Cambridge, MA 02138, USA). Results: AmniSure rapid immunoassay, rapid method for diagnosis of ROM, has 100% specificity, 99.44% sensitivity (one false negative due to meconium and immediate cesarean section). There is variable admixture of both malignant epithelial and stromal component seen in this tumor. We report a case of a primary carcinosarcoma of ovary in a 72-year-old post-menopausal female presenting with the complaint of abdominal distension. Staging laparotomy was done for this patient, and final histopathology was reported as the carcinosarcoma of ovary.
