{"title":"Educational technology for patient participation in the safe administration of antineoplastic chemotherapy: methodological study.","authors":"Elaine Cristina Salzedas Muniz, Isabel Cristina Kowal Olm Cunha, Elena Bohomol","doi":"10.1590/1983-1447.2025.20240316.en","DOIUrl":"10.1590/1983-1447.2025.20240316.en","url":null,"abstract":"<p><strong>Objective: </strong>to develop an educational technology to promote patient participation in the safe administration of antineoplastic chemotherapy drugs.</p><p><strong>Method: </strong>applied methodological study conducted between 2021 and 2023, in three hospitals in a city in the interior of São Paulo, developed in five stages: integrative literature review; approach to the reality of the healthcare production environment; development of educational technology; evidence of face and content validity by judges; analysis of semantic agreement of the educational technology.</p><p><strong>Results: </strong>an infographic and nine verification cards were developed. In the validation of 15 judges, the overall Content Validity Index for the infographic and verification cards was obtained at 82% and 84%, respectively, and the overall Content Validity Ratio was 0.63 and 0.75. In the last stage. The search was carried out in February 2024, in the sources: Virtual Health Library, Scientific Electronic Library Online, Web of Science, Scopus, MEDLINE, CAPES Periodical Portal and Brazilian Digital Library of Theses and Dissertations. Cross-sectional studies conducted in the hospital environment, published between 2004 and 2024, were included. After selecting the recruited material (n=1,297), the sample (n=29) was analyzed using descriptive statistics.</p><p><strong>Results: </strong>Of the selected studies, 86.2% indicated superiority in the percentage of absence coverage recommended by the entity. Absenteeism rates ranged from 0% to 44.7% among nurses and from 0.47% to 46% among technicians/assistants. Seventy-three fourth-year students were included, with five student losses during the study. The initial sample consisted of 68 students, randomized into a control group (n=33) and an experimental group (n=35). For the experimental group, the evaluator could not be seen during their access to the scenario; for the control group, the presence of the evaluator was visible. To compare the groups, the chi-square test was used for categorical variables, ANOVA for intragroup differences, and Student's t for independent samples for intergroup analysis.</p><p><strong>Results: </strong>there was an increase in the means of the theoretical test (<0.001) between the pre-test and post-test, in both groups. In the evaluation of the students' skills in basic life support, there was no significant difference between the groups (p=0.455).</p><p><strong>Conclusion: </strong>there was retention of theoretical knowledge and skills in both groups, regardless of the presence of the evaluator. The planning, educational intervention and pre- and post-intervention audits were conducted according to JBI recommendations. Data were collected using a questionnaire, and the intervention consisted of dialogue, videos and an educational game. The selection strategy was conducted by educational clusters, schools were selected by probabilistic sampling and participants by convenience. The data were submitted to inductive thematic analysis.</p><p><strong>Results: </strong>teachers reported a feeling of frustration and overload due to the lack of continued training to assist in the process of including children with special healthcare needs in the school environment. Strategies included playful activities and planning in partnership with special educators. They cited the nursing professional as a facilitator for the inclusion process.</p><p><strong>Conclusion: </strong>in the teachers' perception, there are difficulties related to the child, the training process, institutional aspects and infrastructure to enable inclusion. This study was based in southern Brazil, involving professionals from various health areas from different regions of the country. Data collection took place between 2023 and 2024, through in-person meetings and online questionnaires made available via Google Forms®. To assess reliability, Cronbach's alpha coefficient was calculated.</p><p><strong>Results: </strong>The translated and adapted version of the Creighton Interprofessional Collaborative Evaluation was modified according to the experts' suggestions. Validity and reliability evidence were measured using the Content Validity Index and Cronbach's Alpha, respectively. Data collection was performed using a specific instrument developed by the researchers, with two assessments at different times: at risk classification in emergency services, and as the patient was transferred to another service for the last time. Predictive performance was assessed using the area under the curve (AUC) of the ROC, sensitivity, and specificity, comparing the scores across the evaluated outcomes and time points.</p><p><strong>Results: </strong>For clinical deterioration, the National Early Warning Score 2 - Brazilian Version showed an AUC of 0.703, 95% CI (0.63-0.77), p<0.001, and an AUC of 0.871, 95% CI (0.82-0.91), p<0.001 in the first and second assessments, respectively. For admission to the Intensive Care Unit, all three scores remained accurate only in the second assessment, with the National Early Warning Score 2 - Brazilian Version yielding the best results, AUC: 0.830, 95% CI (0.76-0.89), p<0.001 compared to the others. Data was collected from August to October 2023 using non-participant observation. The study included 25 licensed practical nurses working in the unit in question. Data analysis was based on reflective thematic analysis.</p><p><strong>Results: </strong>play was primarily used to facilitate the execution of procedures. It was evident that participants did not link and integrate this resource into their care practices. Furthermore, the toy library was not understood as a space associated with licensed practical nurses. The themes \"Play in care practices\" and \"Interactions with the hospital toy library\" detail the professional practices observed.</p><p><strong>Conclusion: </strong>the use of play in licensed practical nurses' care practices was reinforced by the symbolism that play is not feasible during pediatric hospitalization and that using the toy library is not part of its scope, which determined the limited incorporation of this resource. To develop the game, two workshops were initially held with 19 children and adolescents aged 10 to 14, participants in a social project within a community in a medium-sized city in the Midwest region of São Paulo state, Brazil. The workshops began with a screening of the short film LOU (Disney Pixar®), followed by expressive activities such as drawing and writing, as well as group discussions on the topic addressed. The game was developed by a research team comprising undergraduate and post-graduation students, based on data collected during the workshops with the target audience and a review of the relevant literature. This process highlighted different types of violence, reflections, and experiences shared by the participants, which directly contributed to defining the rules, characters, narrative, dynamics, and content of the game. Experimental, quasi-experimental and observational studies were included. Methodological quality was assessed using the Joanna Briggs Institute tool. Narrative synthesis was used to present the results.</p><p><strong>Results: </strong>The review encompassed 21 articles. The most investigated aspect was sealing (71.4%), followed by structural integrity (33.3%), filtration efficiency (19.0%), and, finally, tensile testing of the fastening straps (4.8%). Damage such as dirt accumulation, broken nose clips, and strap breakage, as well as degradation of elastic resistance, were observed, leading to inadequate fit and reduced filtration capacity. Four studies were classified as high quality, while 17 were of moderate quality.</p><p><strong>Conclusion: </strong>Extended use and reuse of N95 respirators are not recommended, as they increase occupational risk. Although they maintain adequate filtration efficiency in different contexts, this is not sufficient to ensure user protection.
