{"title":"Profile of the attention provided to suicide attempts by adolescents in emergency rooms and associated factors.","authors":"Danton Matheus de Souza, Carlos Alberto Dos Santos Treichel, Lucca Garcia Moreira Ribeiro, Lisabelle Mariano Rossato","doi":"10.1590/1983-1447.2024.20240049.en","DOIUrl":"https://doi.org/10.1590/1983-1447.2024.20240049.en","url":null,"abstract":"<p><strong>Objective: </strong>to identify the profile of emergency department visits for suicide attempts by adolescents and their associated factors.</p><p><strong>Method: </strong>a cross-sectional, documentary, and retrospective study was conducted with 140 medical records of adolescents treated for suicide attempts in an emergency department linked to a teaching hospital in the municipality of São Paulo, Brazil, between January 2015 and May 2023. Data was collected between August and November 2023. Descriptive and inferential analysis (Pearson's chi-square test) was performed, considering a p-value≤0.05 as statistically significant. National ethical guidelines were respected.</p><p><strong>Results: </strong>drug intoxications were the most common, associated with females (p<0.001) and continuous use of psychotropic medications (p=0.01). Poisonings were associated with the absence of continuous psychotropic medication use (p=0.02) and treatment in the emergency room (p=0.01). Males were associated with intoxications from alcohol and drugs (p<0.001), wounds from sharp objects (p<0.05), and precipitation (p=0.03).</p><p><strong>Conclusion: </strong>this study found a prevalence of emergency department visits for suicide attempts through intoxications associated with sex, medication use, and treatment in the emergency room. The keywords \"health literacy,\" \"adolescent,\" and \"social determinants of health,\" including their orthographic derivatives, were combined in a search strategy used in 12 databases. The identified articles were independently screened, selected, and extracted by two researchers. Data was analyzed and discussed based on Dahlgren and Whitehead's model of social determinants of health.</p><p><strong>Results: </strong>Twenty-one studies found one or more social determinants related to health literacy, namely: sex, cognitive characteristics, eating habits, family structure and models, school characteristics, school environment, and parents' conditions. The analysis was performed using the Chi-square or Fischer's exact tests, with the outcome being the specific admission nursing consultation for SARS-CoV-2.</p><p><strong>Results: </strong>Among the 1,272 individuals investigated, 21.3% (95% CI: 19.1-23.6) tested positive for antibodies against SARS-CoV-2. Of these, 10.3% (95% CI: 7.0-14.6) underwent a specific nursing consultation for SARS-CoV-2. The most prevalent signs and symptoms were loss of taste (33.2%) and smell (32.8%), body aches (28.8%), and fever (28.4%). The analysis indicated associations between contact with symptomatic COVID-19 case (p=0.039), contact in the yard with a prisoner with COVID-19 (p=0.012), and sore throat (p=0.010), with the outcome.</p><p><strong>Conclusions: </strong>The low prevalence of specific nursing consultations for SARS-CoV-2 and the high prevalence of COVID-19 reinforce the need for mandatory implementation of this strategy in groups living in closed environments. The data results from a bibliographic survey, the use of documentary sources and database consultations, with the material being treated using Thematic Content Analysis.</p><p><strong>Results: </strong>the following categories emerged: Professional trajectory of Nurse Izaura; Commuting between the United States and Italy; and the Nursing Air Evacuation Course.</p><p><strong>Conclusion: </strong>the study highlights the pioneering role of nurses in the Brazilian troops sent to Italy, the necessary preparation and completion of the course to join the air transport service for the wounded. The CFIR provides a classification of factors that affect the implementation of a technology and comprises five domains: characteristics of the intervention, external environment, internal environment, individuals, and process. The study setting included 13 Health Units in the southern region of the city of São Paulo-SP. 39 health professionals and 10 managers participated. Data was collected through semi-structured interviews. These were recorded and transcribed, and the text data was coded based on pre-established codes derived from the CFIR domains.</p><p><strong>Results: </strong>Four categories were found that impacted the implementation of the AA model in services: 1) Characteristics of the Advanced Access model that impacted implementation; 2) Organization of the health sector and the Advanced Access model; 3) Characteristics of the individuals involved in the innovation; and 4) The planning and leadership model for implementation.</p><p><strong>Final considerations: </strong>The social, structural, and professional conditions in the municipality of São Paulo made it difficult to implement the AA model. Pre-production included: script, storyboard, recruitment and validation by experts. During production, the video was created and validated by experts and the target audience. In post-production, the video was edited. The experts consisted of 20 health workers, professors and researchers (child health, immunization, health education and video production). The target audience consisted of 11 parents and guardians. Instruments were used for experts and the target audience and submitted to Content Validity Index analysis, with a minimum parameter of 0.80 for validity and evaluation.</p><p><strong>Results: </strong>The video, lasting 4 minutes and 25 seconds, was considered validated by experts and evaluated by parents and guardians, with an Overall Content Validity Index of 0.99 and 1.00, respectively. The Maternal Mortality Ratio values were subjected to Prais-Winsten linear regression analysis using Stata program, version 14.0. The evolution of the Maternal Mortality Ratio in the regions was presented on maps created using the TabWin software.</p><p><strong>Results: </strong>A total of 8,229 maternal deaths were recorded in the period. The year 2021 recorded the highest Maternal Mortality Ratio (113.1 maternal deaths per 100,000 live births). The Spearman or Kruskal Wallis statistical tests were used and the significance level was updated to 5% for the study's statistical analyses.</p><p><strong>Results: </strong>Structural empowerment was moderate among nurses and \"opportunity\" presented greater mean among the dimensions. Although the correlations were weak, support was the dimension that showed the greatest results with the other variables. It is more noticeable when: shorter training time; greater age and; longer time at the institution. A semi-structured questionnaire was applied for socio-occupational and health characterization, and the Stanford Presenteeism Scale. Prevalence ratios were used to evaluate associations and statistical significance using Pearson's X2; binary logistic regression to investigate the impact of socio-occupational variables on presenteeism and linear regression for the scale domains.</p><p><strong>Results: </strong>Female (65.8%), mean age 32±8.59 years, no children (68.4%), nursing staff (55.3%), no previous (74.4%) or acquired (87.5%) comorbidities. Of the total, 85 (55.93%) professionals had low presenteeism. Previous comorbidity χ2(1)=6.282;p=0.012, sick leave (χ2 (1)=7.787;p=0.005, R2 Negelkerke= 0.069) and medication use (χ2 (1)=8.565;p=0.003, R2 Negelkerke= 0.077) were predictors of low presenteeism.</p><p><strong>Conclusion: </strong>There was no significant association between sociodemographic variables and presenteeism. Data were collected through a clinical/sociodemographic form and a Quality of Life Instrument. For analysis, descriptive statistics were used, and Pearson's Chi-square and Fisher's Exact tests were applied to assess the association between quality of life and clinical/sociodemographic variables.</p><p><strong>Results: </strong>Out of the total participants, the ones who scored high quality of life were mostly, female (52.5%), in the age group 15 to 17 years old (56.3%), 51.3% self-identified as white, and 40.1% were attending high school. In the preoccupation domain, a significant association was found to the time since diagnosis (p=0.035), demonstrating that young adolescents with a longer time of diagnosis had a worse quality of life.</p><p><strong>Conclusion: </strong>The majority of adolescents in this study (51.3%) had been living with diabetes for 10 or more years.
