The central question is whether the public interest is a usable concept in a time of social and political change. A historical overview of the public interest is provided. Two researchers are highlighted: David John Farmer and O.C. McSwite. The paper concludes by proposing the refusal of subjective identification with the public interest.\u0000\u0000\u0000Design/methodology/approach\u0000Discourse theory and Lacanian psychoanalysis are discussed in this paper. The emphasis in both approaches is to examine ethical challenges in politics and administration through new epistemological lenses. A further use of these research strategies is to identify existing institutional practices and situate administrative decision-making within those practices.\u0000\u0000\u0000Findings\u0000The findings in this paper indicate that while institutional resistance is useful, it can also be co-opted or result in retribution. In both cases, power is asserted and maintained by those who hold institutional power. David John Farmer’s work on anti-administration and O.C. McSwite’s work on administrative refusal are effective strategies to address the abuse of institutional power.\u0000\u0000\u0000Originality/value\u0000This paper introduces the concept of subjective identification to the literature of public administration. Scholarship does not interpret public administration as neutral, even though, on moral–ethical grounds, it frequently advises neutrality for practitioners. Five main schools of thought are surveyed. Neutrality and alternative expressions of it, such as nonpartisanship, expertise, impartiality or facilitation, are role prescriptions for practicing public administrators, and are typically offered as appropriate comportments in interacting with citizens and groups. At the same time, public administration is undeniably a political institution having political purposes and constitutive impacts. Indeed, the very existence of the administrative state is politically contestable. The paper aims to discuss these issues.\u0000\u0000\u0000Design/methodology/approach\u0000Critical reflection, political philosophy, political theory.\u0000\u0000\u0000Findings\u0000Scholars across the various schools of thought in public administration do not presuppose the presence of a neutral public administrator. Descriptions of the nature of ideologies, myths and metaphors are given, along with some examples of those that are harmful – such as the ideologies of the American Business Model and neo-liberalism.\u0000\u0000\u0000Findings\u0000PA should seek to engage and oppose such ideologies, myths and metaphors that are problematic – both those constructed within and without of PA itself. It is recommended that, for such analytical purposes, PA should turn toward post-traditional PA, including the insights of post-modernism and epistemic pluralism.\u0000\u0000\u0000Originality/value\u0000The concept of sub-administration is of significant utility both for PA theorists and practitioners in strengthening public administrative performance. Governance myths – myths that we tell about the state and our relationship to it or about the structures and figures making up our government and our relationships to them – can serve as a valuable means of gaining insight into civil society and for illuminating the goals and values of good governance. Categorizing governance myths can aid in that process. The paper aims to discuss this issue.Design/methodology/approachThis paper develops a typology of governance myths, and then explores mythic types and their implications for governance.FindingsA typology of myths facilitates systematic examination of fundamental stories told to explain and illustrate governance. Secondary data set was utilized to test the hypotheses of this study. Home health care and hospice care services have the potential to avert hospital admissions in patients requiring palliative care, which significantly affects medicare spending. With the aging population, it has become evident that demand of palliative care will increase four-fold. It was determined that current spending on end-of-life care is radically emptying medicare funds and fiscally weakening numerous families who have patients under palliative care during life-threatening illnesses. The study found that a majority of people registering for palliative and hospice care settings are above the age group of 55 years old.\u0000\u0000\u0000Design/methodology/approach\u0000Different variables like length of stay, mode of payment and disease diagnosis were used to filter the available data set. Secondary data were utilized to test the hypothesis of this study. There are very few studies on hospice and palliative care services and no study focuses on the cost associated with this care. Since a very large number of the USA, population is turning 65 and over, it is very important to analyze the cost of care for palliative and hospice care. For the purpose of this analysis, data were utilized from the National Home and Hospice Care Survey (NHHCS), which has been conducted periodically by the Centers for Disease Control and Prevention’s National Center for Health Statistics. Descriptive statistics, χ2 tests and t-tests were used to test for statistical significance at the p<0.05 level.\u0000\u0000\u0000Findings\u0000The Statistical Package for Social Sciences (SPSS) was utilized for this result. H1 predicted that patients in the age group of 65 years and up have the highest utilization of home and hospice care. This study examined various demographic variables in hospice and home health care which may help to evaluate the cost of care and the modes of payments. This section of the result presents the descriptive analysis of dependent, independent and covariate variables that provide the overall national estimates on differences in use of home and hospice care in various age groups and sex.\u0000\u0000\u0000Research limitations/implications\u0000The data set used was from the 2007 NHHCS survey, no data have been collected thereafter, and therefore, gap in data analysis may give inaccurate findings. To compensate for this gap in the data set, recent studies were reviewed which analyzed cost in palliative care in the USA. There has been a lack of evidence to prove the cost savings and improved quality of life in palliative/hospice care. There is a need for new research on the various cost factors affecting palliative care services as well as considering the quality of life. HAs are a type of “special district” established in state law to allow for specialization of function, while evading statutes that can limit local government borrowing and multi-year contracts. The paper aims to discuss this issue. Design/methodology/approach This paper uses a case example to introduce transparency and accountability in one local Georgia hospital and expands to include a descriptive analysis of transparency measures in 29 rural Georgia HAs. Findings Findings indicate that, like many other special districts in Georgia and the USA, Georgia’s rural HAs often act more like private entities than the public organizations they are. The lack of transparency demonstrated in this sector limits access to public information and reduces opportunities for citizen engagement, a necessary component of representative institutions. Research limitations/implications This case study is limited to Georgia HAs; however, the data support the lack of accountability and transparency found in many special district governments. The study evaluated 785 hospitals which operate ACO in contrast to 1,446 hospitals without an ACO.\u0000\u0000\u0000Findings\u0000In total, 89 percent of hospitals using ACO’s are located in urban communities and 87 percent are not-for-profit. Hospitals with a higher case mix index are more likely to have an ACO.\u0000\u0000\u0000Practical implications\u0000ACOs allow healthcare organizations to expand their geographic markets, achieve greater efficiencies, and enhance the development of new clinical services. Young, Kristina Lu","doi":"10.1108/IJOTB-03-2018-0026","DOIUrl":"https://doi.org/10.1108/IJOTB-03-2018-0026","url":null,"abstract":"\u0000Purpose\u0000The purpose of this paper is to analyze the study results conducted at a four-year university in Hawaii investigating the impact of providing nursing students with an educational intervention session aimed at improving cultural competence.\u0000\u0000\u0000Design/methodology/approach\u0000A descriptive-correlational research method was used to examine the correlations between a control group and experimental group using pre-and post-tests. The t-test for equality of means and Levene’s test for equality of variances were conducted for statistical analysis on pre-and post-test scores. In addition, a power analysis was conducted due to the small sample size.\u0000\u0000\u0000Findings\u0000The control group receiving no intervention scored lower on the post-test in overall competency by five points, while the experimental group increased their post-score by five points after receiving the intervention; however, this increase did not change the overall cultural competence score. The results indicate that the educational intervention of a two-hour didactic, discussion and presentation did not provide as robust as what was needed to increase domain scores for the experimental group. Further, the domains of awareness, skill, knowledge, encounter and desire cannot be taught by instruction alone and should be reinforced over time.\u0000\u0000\u0000Research limitations/implications\u0000The study was a convenience sample and limited by the small sample size. The sample may not be representative of all senior nursing students. The study is limited to one school of nursing in Hawaii; the results may not be generalized to other populations.\u0000\u0000\u0000Practical implications\u0000This research provides a foundation for future curriculum development and the evaluation of nursing programs. For instance, incorporating a value-added instructional project on cultural competence into each nursing class would increase cultural competence awareness and knowledge.\u0000\u0000\u0000Social implications\u0000This study also emphasizes the necessity of education in cultural competence for all health professionals, which has implications for improving quality, patient satisfaction and increased health outcomes.\u0000\u0000\u0000Originality/value\u0000This research is unique to examining and applying an educational intervention on cultural competence for nursing students in Hawaii. This research sheds light on studying the importance of culture competence for nursing students and other health professionals.
