This study aimed to determine the status of glycemic control and associated factors among type 2 Diabetes Mellitus (DM) patients at primary health care of Cakung District, Kebon Jeruk District and Rawa Bunga Village, Jakarta. This study was conducted from July to August 2019, and adopted a cross-sectional design. The respondents' data included sex, age, occupation, education, long suffered with diabetes, diagnosed chronic disease, antidiabetic and other regular drugs were obtained through a questionnaire. Measurement of glycated haemoglobin A1c level was carried out in a standardized laboratory in Jakarta. A total of 126 respondents met the inclusions and exclusion criteria, of which 70.6% were female. The mean age of patients was 61.46±9.086 years (35–85 years). HbA1c level was measured, and the results showed that 45.2% of respondents had good glycemic control (<7% of HbA1c level), while 54.8% had poor control (≥7% of HbA1c level). On the bivariate analysis, the number of antidiabetics was significantly associated with glycemic control (p<0.05). The poor glycemic control was significantly higher in patients with polytherapy (72.6%) antidiabetic compared to single antidiabetic (37.5%) (p=0.01). These findings highlighted the need for proper management of patients with polytherapy, in order to prevent the complication of type 2 DM.Keywords: Glycemic control, Jakarta, primary health care, type 2 diabetes mellitus Kontrol Gikemik dan Faktor yang Berhubungan pada Pasien dengan Diabetes Melitus Tipe 2 di JakartaAbstrakKontrol glikemik yang buruk merupakan faktor risiko utama terjadinya komplikasi pada pasien diabetes melitus (DM). Penelitian ini bertujuan untuk mengetahui kontrol glikemik dan faktor yang berhubungan pada pasien DM tipe 2 di Puskesmas Kecamatan Cakung, Kecamatan Kebon Jeruk dan Kelurahan Rawa Bunga, Jakarta. Penelitian dilakukan pada bulan Juli–Agustus 2019. Desain penelitian yang digunakan adalah cross-sectional. Data responden antara lain jenis kelamin, umur, pekerjaan, pendidikan, lama menderita penyakit DM, penyakit kronis lain yang diderita, obat DM dan obat rutin lain yang digunakan didapatkan melalui instrumen kuisioner. Pengukuran kadar HbA1c dilakukan di laboratorium yang terstandarisasi di Jakarta. Sebanyak 126 responden memenuhi kriteria inklusi dan eksklusi terlibat dalam penelitian ini, dan sebanyak 70,6% adalah berjenis kelamin perempuan. Usia rata-rata pasien adalah 61,46±9,086 tahun (35–85 tahun). Hasil penelitian menunjukkan bahwa 45,2% responden memiliki kontrol glikemik yang baik (<7 level HbA1c), sedangkan 54,8% responden memiliki kontrol glikemik yang buruk (≥7 level HbA1c). {"title":"Quality of Antibiotic Prescribing for Respiratory Tract Disease in Primary Healthcare Centers in the District of Tegal, Central Java, Indonesia","authors":"F. Furthermore, inappropriate use has been implicated in numerous problems, including the onset of resistance. This study is, therefore, aimed to evaluate the antibiotic prescribing quality for respiratory tract disease diagnosed at the outpatients of primary healthcare centers in Tegal District, Central Java, Indonesia. In addition, cross-sectional evaluation was employed, using data obtained within the period of June–August, 2018 in six locations, and subsequently subjected to qualitative analysis. The prescribing quality was evaluated by comparing the drug selected, dosage, frequency, and duration of administration stated in the prescriptions, with the terms in the Clinical Practice Guidelines for Primary Healthcare Facilities, 2014. A total of 1453 samples were selected through purposive sampling, where 632 were diagnosed with respiratory tract diseases, with common cold as the most frequent diagnosis (87.2%). In addition, exactly 621 (98.3%) failed to fulfill the rational antibiotic prescribing criteria, evidenced by irrational drug selection (22.0%), dosage (9.5%), frequency (1.7%), and duration of administration (65.0%). The physicians tend to comply more with the national treatment guidelines (OR: 1.828, 95%CI: 0.486–6.874, p-value 0.365), and the less experience of prescribers (<12 years of service) was identified as a negative contributing factor (OR: 0.536, 95%CI: 0.143–2.016, p-value 0.349). Furthermore, irrational prescription was observed in a much larger proportion, influenced by the prescribers’ qualification and experience. This is particularly consumed frequently, and is composed of glycerol, and the fatty acid, specifically differentiated into unsaturated, saturated, trans, and cis forms. Furthermore, these constituents are known to play many roles in the body, including in the hematopoietic process. This involves oxidation and consequently stem cell differentiation into many blood cells in long-term, although the effect short-term is currently unknown. The study aimed, therefore, to investigate the effect of short-term intake of different fatty acid types on hematological profile in an animal model, conducted at the Animal Laboratory, Universitas Padjadjaran in October 2018. In addition, each group comprised 6 mice, orally administered distilled water as control (Group A), fish oil (Group B), virgin coconut oil (Group C), and used-cooking oil (Group D), at a dose of 5 μl/g body weight/day for 2 weeks. Subsequently, analysis was performed using blood measurement with hematology analyzer. The results showed lower white blood cell (WBC) count in Group B compared to D (p<0.05), alongside lymphocyte count (p<0.01). Moreover, the WBC in Group C was lower than D (p<0.01), also observed in lymphocyte count (p<0.001), % lymphocyte (p<0.01), while the % granulocyte count was higher than group D (p<0.01). Therefore, the highest total leukocyte and lymphocyte number among the other groups, as well as higher percentage of differential lymphocyte count was observed with mice provided with used-cooking oil compared to coconut oil, alongside a lower percentage of differential granulocyte count (p<0.05). However, fatty acid intake in group A, B, C, and D had no significant impact on RBC and platelet parameters. In conclusion, used-cooking oil induces a change in hematological profiles compared to fish oil and virgin coconut oil, featuring the increased total white blood cells and lymphocyte, as well as reduced % granulocyte. This phenomenon is an intermediary pathway in the pathogenesis and therapy of various disease types, and is currently a new treatment route in the medical world. Hence, it is necessary to deeply understand the regulations, to prepare a treatment strategy in the future. Conversely, there is a high utilization of drugs, in line with the numerous disease cases in the community. Some limited chemical forms have large but necessary side effects, including anticancer agents. The high incidence of resistance and polypharmacy has been implicated in adverse drug interactions, and exorbitant costs, thus the initiative to return back to natural means. Furthermore, herbal medicines have been used in treatments for thousands of years, prior to the discovery of modern medicine. The various types previously studied have been highly beneficial in various disease cases. This review on researches within the last 5 years identified some herbal research articles in the aspect of autophagy, which was then summarized in a table form. Meanwhile, autophagy has been used for cancer therapy, and many plants have the potential to be therapeutic for various diseases through this pathway. In addition, prevention and treatment efforts are currently being challenged by resistance to insecticides and anti-malaria drugs. This instigates the need for a more specific prevention method. This article, therefore, aims to determine the potential of anti-PfRH5 antibody vaccines, based on liposome nanoparticles, as the final preventive modality in Plasmodium falciparum malaria. The method used in this review involved searching study literature. The search resulted in 60 journals, and a total of 48 were obtained after screening according to the topic of discussion, and thus adopted as references to this work. Furthermore Plasmodium falciparum life cycle is characterized by an invasion phase of erythrocytes, where the Reticulocyte-binding Protein Homologue 5 (PfRH5) and basigin (BSG) serve as essential ligand-receptor pairs. The addition of liposome nanoparticles is vital to protect the anti-PfRH5 antibody vaccine component, and prevent easy degradation, thus improving the vaccine efficacy in the inhibiting the merozoites invasion process. This potential target as a malaria vaccine is very significant and specific, and is supported by the emergence of natural immune responses capable of deterring parasite growth. The immune response involves the action of specific IgG, estimated to provide a protective effect by inhibiting the occurrence of a PfRH5-BSG bond, therefore preventing the invasion process occurrence.
