Increased number of drugs prescribed can elevate the risk of drug interactions. The purpose of this study was to determine the description and potency of the incidence of drug interactions that occurred in outpatients with type 2 diabetes mellitus. This research was non-experimental that observed retrospectively with a crosssectional approach. Subjects were outpatient type 2 DM patients at Panembahan Senopati Hospital Bantul in 2015 who had fulfilled the inclusion criteria, with a total of 260 prescription sheets. Drug interaction events were identified based on the Drug Interaction Facts literature by Tatro and Stockley’s Drug Interaction. Hypothesis testing was carried out using the Chi-Square test and the calculation of risk of incidence of drug interactions was displayed by odds ratio. The results showed that of the 260 prescription sheets analyzed, there were 121 identified potential drug interactions with the largest percentage is the interaction between insulin and aspirin (14.29%). Hypothesis testing using Chi-Square test showed a significant relationship (p=0.000) between the amount of drug prescribing and the potential occurrence of drug interactions (p<0.05). Result of odds ratio showed that patients receiving drugs ≥5 had 3.657 times higher risk of potential drug interaction (95% CI 2.173–6.157).","PeriodicalId":351729,"journal":{"name":"Indonesian Journal of Clinical Pharmacy","volume":"27 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114783617","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Fibrinogen elevation is a known risk factor and a powerful predictor of ischemic cerebrovascular disease. The objective of this study was to conduct a systematic review and meta-analysis of the effects of lumbrokinase treatment on fibrinogen concentration and blood and plasma viscosities. The search strategy included articles indexed by PubMed and other resources (Google Search), using medical subject headings (MeSH) and text words. A total of 185 articles were identified; however, only two articles were included in the final analysis, including 727 patients. The results demonstrated a significant reduction in the fibrinogen concentration (–0.67, 95% confidence interval [95% CI] [–1.22, –0.12]; p=0.02) and a significant reduction in plasma viscosity (–0.77, 95% CI [–1.06, –0.48]; p=0.00001) for the lumbrokinase-treatment group compared with the control group. However, no significant difference in blood viscosity was observed between the two groups (–0.61, 95% CI [–1.33, 0.11]; p=0.10). Lumbrokinase may be effective as an adjuvant drug alongside the standard treatment for ischemic cerebrovascular disease. However, these results should be interpreted with caution due to high heterogeneity, and further randomized controlled trials are necessary.","PeriodicalId":351729,"journal":{"name":"Indonesian Journal of Clinical Pharmacy","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129664626","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Autophagy contributes to a variety of physiological processes, including cell differentiation and various functions in embryogenesis. Some studies reported that the expressions of autophagy-related (Atg) proteins are found in placentas. This review article was focusing on the autophagy process and some Atg proteins which are involved in human placentation, especially in preeclampsia cases, since it has been well known that abnormal placentation and placenta dysfunction has crucial role in its development. Preeclampsia cannot be related to a single cause and the underlying mechanism of it is still not clearly understood. Recent hypothesis regarding the cause of preeclampsia is more focused on the inadequate trophoblast invasion and placentation. Scientists also suggested that other mechanism might be associated with this condition in preeclampsia, which is autophagy. Autophagy is a mechanism that essential for cellular remodeling which occurs during the development of multicellular organisms in the special process, by expressing an “eat-me” signal and cleared by neighboring cells. In preeclampsia patients, autophagy has an important role in trophoblast function under physiologically low oxygen conditions. The activation of autophagy in preeclampsia is shown by the different level of abundance of key protein of the Atg pathway. Some Atg proteins known to be related to preeclampsia are Beclin-1, LC3, and p62.","PeriodicalId":351729,"journal":{"name":"Indonesian Journal of Clinical Pharmacy","volume":"46 5","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"120984602","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Studies reveal that medication error is still a significant issue in health service, and many efforts have been done to minimise this through trainings. Integrative pharmacotherapy is a module delivered for medical students in clinical clerkship, which is adopted and developed from the World Health Organization Guide to Good Prescribing. The aim of the study was to examine the effect of the module on pre-test and post-test scores which consists of 17 clinical cases. The study was done in the Faculty of Medicine, Universitas Kristen Indonesia, East Jakarta from July to November 2018. Eighty one subjects were randomly selected from the data of 100 students who were screened from 200 students based on the exclusion criteria. There was an increase of post-test score which was statistically significant compared to pre-test score, from 42.07±12.45 to 58.47±8.54 (p=0.000, CI -19.36;-13.45). There were 69 (>85%) students having grade D and E in pre-test, but the number decreased to 18 (22,2%) out of 81 subjects in post-test. Both grade C and B students increased from 13.5% to 70.4% and 1% to 7%, respectively. Most of the students were first entries into clinical clerkship, thus, their pre-test scores were very low since they had not had the clinical experience yet. However, at the end of the clerkship, there was a significant increase on their knowledge on pharmacotherapy and prescription for particular diseases. An outcome study to measure the retention and to examine their success on the final competency examination should be done.","PeriodicalId":351729,"journal":{"name":"Indonesian Journal of Clinical Pharmacy","volume":"63 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"122810706","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Tata laksana yang tepat dan cepat diharapkan dapat menyelamatkan nyawa pasien. Sampai tulisan ini dibuat, belum terdapat satu jenis obat yang secara resmi diizinkan penggunaannya untuk terapi COVID-19. Kajian literatur ini bertujuan untuk 1) memaparkan tata laksana pengobatan dan 2) mendaftar serta menjelaskan alternatif obat yang dapat digunakan untuk SARS-CoV-2. Proses penelusuran artikel dalam kajian pustaka ini dilakukan pada sebuah database , yakni PubMed dengan kombinasi kata kunci ((“corona virus”) OR (“covid-19”) OR (“SARS-CoV-2”)) AND ((“treatment”) OR (“therapy”)). Hasil kajian ini menunjukkan bahwa tata laksana pasien dengan COVID-19 dapat berbeda antar- setting dan negara dengan mempertimbangkan ketersediaan sumber daya, khususnya obat. Pedoman terapi WHO dan pedoman di Indonesia saat ini merekomendasikan supportive therapy untuk penanganan COVID-19, antara lain: terapi untuk gejala yang terjadi, pemberian oksigen, penggunaan antibiotik, terapi cairan, penggunaan vasopresor, dan tindakan medis (termasuk pemasangan ventilator) untuk menyelamatkan nyawa pasien. Belum terdapat obat khusus yang direkomendasikan untuk menekan replikasi SARS-CoV-2. Beberapa jenis obat yang potensial bermanfaat untuk SARS-CoV-2 antara lain: klorokuin atau hidroksiklorokuin, arbidol, ribavirin, favipiravir, lopinavir/ritonavir, remdesivir, oseltamivir, dan interferon. Namun sampai dengan tulisan ini dibuat, terdapat keterbatasan bukti penelitian dengan desain yang baik yang dapat digunakan untuk menarik kesimpulan terkait superioritas suatu jenis obat tertentu dibandingkan dengan alternatif yang lain. Dalam kondisi menunggu hasil penelitian dengan desain penelitian yang baik, penggunaan obat yang memiliki bukti efektivitas (walaupun belum baik) atau diduga efektif, perlu dioptimalkan untuk menyelamatkan nyawa pasien, khususnya mereka yang dalam kondisi parah.
