In the case of lethal defects, it is not possible to effectively help the child, despite using the most modern equipment or medicines in the treatment or the progress made by medicine. Parents, who decide to continue the pregnancy, although the fetus has a lethal defect that cannot be cured, may be covered by perinatal hospice care, which is comprehensive and consists in supporting the pregnant woman during the prenatal time, during delivery and after delivery and support of her family, giving full information to the parents about their child's illness. Childcare after birth is focused on protecting the infant from persistent therapy and providing him with appropriate conditions. Aim: To demonstrate the role of perinatal palliative care for pregnant women in whom the results of prenatal tests pointed to a severe developmental disorder in the fetus with a potentially lethal prognosis, and to present a pattern of behavior for their hospitalization in the perinatal hospice.

Patients and methods: Materials and methods: The retrospective analysis included documentation of 67 patients referred to the RAZEM (TOGETHER) Program in Wrocław in 2014-2018 due to abnormal results of (ultrasound and / or genetic) prenatal tests, which indicated a serious developmental disorder in the fetus with potentially lethal prognosis. Palliative care was continued in 51 patients, because 6 women decided to terminate their pregnancy during the diagnostic process (10.5%). The most common abnormalities in the fetuses were chromosomal aberrations, CNS defects and kidney defects. W ciągu roku większość dzieci może doświadczać 5-8 epizodów kaszlu trwających około tygodnia. Dłuższy niż 4 tygodnie czas trwania kaszlu, definiowany jako kaszel przewlekły, istotnie niepokoi rodziców, pogarsza jakość życia rodziny i stanowi przyczynę zwiększenia częstości konsultacji lekarskich oraz zdarzeń niepożądanych z powodu nieprawidłowego stosowania leków. Stanowi więc istotny problem nie tylko zdrowotny, ale też społeczny. W poniższej pracy został przedstawiony aktualny stan wiedzy dotyczący patofizjologii kaszlu przewlekłego, najnowszych możliwości diagnostycznych, sposobów monitorowania kaszlu i zaleceń postępowania terapeutycznego. Dla zaakcentowania odrębności patomechanizmów prowadzących do stanu kaszlu przewlekłego użyto nowego pojęcia cough hipersensitivity syndrom. Podkreślono fakt, że poszczególne metody monitorowania nasilenia kaszlu nie pozwalają na pełną charakterystykę kaszlu. Aktualnie zaleca się stosowanie jednocześnie kilku metod celem dokonania pełnej oceny. Przedstawiono nowe strategie terapeutyczne opierające się na regulacji przewodzenia bodźców drogą aferentną nerwu błędnego oraz na modyfikowaniu aktywności neurotransmiterów w pniu mózgu i śródmózgowiu. Ponadto w poniższej pracy przedstawiono algorytm postępowania diagnostyczno-terapeutycznego w przypadku kaszlu przewlekłego u dzieci, co pozwala przyspieszyć ustalenie właściwego rozpoznania, rozpoczęcie leczenia oraz poprawić jakość życia pacjentów z problemem kaszlu przewlekłego.","PeriodicalId":35058,"journal":{"name":"Medycyna wieku rozwojowego","volume":"22 1","pages":"329 - 340"},"PeriodicalIF":0.0,"publicationDate":"2018-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"69886823","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

U 44-46% dzieci współistnieje z innymi anomaliami rozwojowymi układu moczowego. Najczęściej rozpoznawane jest wodonercze, odpływ pęcherzowo-moczowodowy i zdwojenie moczowodu. Pięcioletni chłopiec został przyjęty do Kliniki Nefrologii z powodu bólów brzucha i wymiotów. W rutynowym badaniu USG jamy brzusznej wykonanym w wieku 9 miesięcy u chłopca rozpoznano nerkę podkowiastą. Kontrolne badania USG wykazywały niewielkie poszerzenie miedniczki i kielichów w nerce lewej. Scyntygrafia dynamiczna nerek, z użyciem 99mTc-EC, wykazała w nerce podkowiastej wodonercze lewostronne z wysokim udziałem nerki w oczyszczaniu (55% ERPF), wydłużonym czasem tranzytu miąższowego i objawami częściowego utrudnienia wydalania na poziomie ujścia miedniczkowo-moczowodowego. W urografii rezonansu magnetycznego opisano powiększoną nerkę lewą do 105 mm, z odcinkowo zwężoną warstwą miąższową, miedniczka w wymiarze przedniotylnym 39 mm, kielichy poszerzone do 26-32 mm, wysokie odejście moczowodu z miedniczki, nerka prawa prawidłowa.
