Wierzba, Monika Cichoń-Kotek, Alicja Jańczewska","doi":"10.34763/devperiodmed.20192301.6066","DOIUrl":"https://doi.org/10.34763/devperiodmed.20192301.6066","url":null,"abstract":"Fetal alcohol spectrum disorders (FASD) is a group of disorders that can occur in children whose mothers consumed alcohol in pregnancy.Diagnosis of fetal alcohol syndrome is based on the appearance of growth deficiency, the presence of the three key features of facial dysmorphism (short palpebral fissures, thin upper lip, smooth or flattend philtrum) and/or disorders in the central nervous system (minimum 3) and prenatal exposure to alcohol (confirmed if possible).Early diagnosis of fetal alcohol syndrome - after birth or in infancy - is very often impossible or very difficult due to the incomplete manifestation of the key dysmorphic features. The latest reports offer the chance of diagnosing children in the neonatal period.The research focuses on the analysis of ethanol metabolites in the biological tissues in pregnant women or newborns. These unique ethanol metabolites include: fatty acid ethyl esters (FAEE) present in the meconium, blood, hair of the mother and the newborn, ethyl glucuronide in the placenta and meconium, urine, nails and hair, and phosphatidyl ethanol (PEth) found in the infant blood. There were 152 measurements of maximum and minimum serum gentamicin concentrations. Samples were determined based on the chemiluminescence technique on the Siemens Advia Centaur analyzer. The concentrations of gentamicin that were measured were compared with various therapeutic ranges used in the literature.\u0000\u0000\u0000RESULTS\u0000According to the standards adopted in the University Hospital in Wrocław, the maximum concentration was reached in 38.16% of all the chi/dren, white the minimum in 26.32%. In other chi/dren the concentrations were below or above the therapeutic range. According to the Neofax® guidelines, the intended maximum concentration was observed in 71.05% of the newborns, and the minimum in 32.89%. The minimum concentration of <2 mg/L was found in 93.42% of the newborns, white >2 mg/L was determined in 33.33%, despite a 48-hour dosing interval. These were premature ba bies (<28th week of gestational age) and 55.56% of them reached a maximum concentration of 5-12 mg/L.There was no significant correlation between maximum or minimum concentration and gestational age or body weight.\u0000\u0000\u0000CONCLUSIONS\u00001. The dosage of gentamicin in newborns according to the Neofax® recommendations does not ensure achieving the intended serum antibiotic concentrations. 2. We discuss the most prominent clinical features of androgen excess and correlate them with studies on androgen receptor (AR) alterations. For the first time we summarize the confirmed localizations of all known AR receptors in women. The knowledge of ARs may be the basis for AR-targeted therapies of androgenic disorders in women, including malignancy, as it has recently been demonstrated for triple-negative breast cancer. It is often detected accidentally during an ultrasound or an abdominal x-ray performed for other reasons. However, the first symptom of kidney stone disease can be severe pain called renal colic. Pain caused by a colic attack is characterized by sudden onset. In half of the cases it is associated with nausea or vomiting and can lead to hypotension and fainting. The exact location and radiation of the pain depends on the location of the stone in the urinary tract. The first most commonly performed study is abdominal ultrasound with estimation of the deposit size and evaluation of urinary tract obstruction. Alternative or complementary studies are: an abdominal x-ray where radiopaque deposits can be shown, or unenhanced helical computed tomography of the abdomen. The severity of pain depends on the individual pain threshold and on the change in hydrostatic pressure in the part of the urinary system above the obstruction. Prolonged deposition of the stone in one place causes the activation of autoregulatory mechanisms to lower the pressure of the upper urinary tract, which limits the pain. The basic treatment for renal colic is analgetic therapy. The most commonly used drugs are NSAIDs and opiates. Another important component of renal colic treatment are medications that facilitate urinary stone passage by reducing oedema or limiting urethral contractions, such as: calcium channel blockers, alpha blockers, phosphodiesterase inhibitors. Intensive hydration is not currently recommended. Patients who are unlikely to spontaneously excrete the stone are eligible for minimally invasive treatment. The risk of urolithiasis recurring is high, reaching up to 40% in 5 years and up to 50% in 10 years. However, it can be reduced by proper prevention. Chopin Clinical Voivodeship Hospital in Rzeszow. Ca/cifying Epithelia/ Odontogenic Tumour (CEOT) is a benign odontogenic tumour arising from the remnants of the dental lamina epithelium. The first three cases of this tumour were recognized by the Danish patho/ogist J.J. Pindborg in 1955. Since then Ca/cifying Epithelia/ Odontogenic Tumour has been commonly referred to as the Pindborg tumour. This type of neop/asm is relatively rare, since it occurs in approximately 0.4% of all odontogenic tumour cases. Due to a fairly common tendency of recurrence, estimated to be approximately 14% of all cases, the preferred o choice of treatment is radical surgical procedure and postoperative follow-up. Appropriate clinical and histopatho/ogical diagnosis is very important before applying the most suitable surgical treatment. One of them is the short index Health Literacy for School-Aged Children (HLSAC) implemented by the members of the Health Behaviour in School-aged Children (HBSC) network. So far, when researching the properties of this instrument, less attention has been devoted to the correlation of HLSAC with other tools related to health.\u0000\u0000\u0000THE AIM\u0000of the paper is to present selected psychometric features of the HLSAC index in a sample of Polish pupils, supplemented by the preliminary analysis of its association with the health locus of control.\u0000\u0000\u0000MATERIAL AND METHODS\u0000The information collected relates to 630 junior secondary school students surveyed in Poland in 2016 as part of the HLSAC questionnaire pilot study. The Multidimensional Health Locus of Control (MHLC) scale by K. Wallston was used as the additional module for Poland. It differentiates among three dimensions of health control: internal, external - dependent on other people, and external - dependent on random factors. It was checked which HLC dimension correlates most strongly with HL.\u0000\u0000\u0000RESULTS\u0000It was demonstrated that the HLSAC scale consisting of 10 questions has a very good reliability in the Polish version (Cronbach alfa = 0.851) and a one-factor structure. Confirmatory factor analysis supported a unidimensional model (RMSEA = 0.082; CFI=0.922; TLI=0.877). The HLSAC most strongly correlates with the internal health locus of control (r=0.376; p<0.001), slightly less with the influence of other people (r=0.153; p<0.001), while the correlation with the random factor health locus of control is insignificant (p=0.947). It was found that there is a weak, however significant, positive correlation between family affluence and HLSAC (p=0.041).\u0000\u0000\u0000CONCLUSIONS\u0000The analyses conducted demonstrated that the Polish version of HLSAC has good psychometric features. The relatively higher correlation between HLSAC and internal rather than external health locus of control was confirmed. The two preferred methods for respiratory support in neonates that contribute to limiting the risk of lung damage associated with mechanical ventilation include nCPAP and non-invasive ventilation. The increasing popularity of surfactant administration techniques is due to the fact they reduce the time of mechanical ventilation until this medication is administered. In some cases a short period of mechanical ventilation follows (INSURE: INtubation-SURfactant-Extubation). There are also methods that make it possible to completely avoid intubation and help maintain spontaneous breathing during surfactant administration (LISA: Less Invasive Surfactant Administration, MIST: Minimal Invasive Surfactant Therapy). Aim To analyze treatment outcomes in preterm infants who suffer from respiratory distress syndrome and require exogenous surfactant administration depending on the technique used: LISA vs INSURE. Material and methods The present retrospective analysis included 129 infants born at a gestational age of between 24 and 33 weeks who were hospitalized in the Neonatology Department in the years 2014-2016, were administered surfactant and remained on non-invasive ventilation. All the subjects received only proractant alfa. Both study groups: LISA (n=83) and INSURE (n=46) were analyzed in terms of respiratory distress treatment outcomes and the presence of complications of prematurity. Results There were no significant differences in patient characteristics between the two study groups (LISA vs INSURE: mean birth body weight was 1210g vs 1275 g, respectively; mean gestational age at birth was 30 weeks vs 29 6/7 weeks, respectively). The comparison of respiratory support method and FiO2 concentration within the first 72 hours after surfactant administration showed no significant differences between the groups. Similarly, respiratory outcomes did not significantly differ between the LISA and INSURE groups and were: the need for intubation ☒ 42.2% vs 32.6%, p=0.201, duration of mechanical ventilation – median days 0 vs 0, p=0.377, duration of nCPAP – median days 5 vs 5, p=0.379, duration of oxygen supplementation – median days 1 vs 1, p=0.555, and the incidence of bronchopulmonary dysplasia – 28.9% vs 23.9%, p=0.506. Also, the incidence of complications was similar in both study groups. Conclusions Our retrospective analysis of preliminary outcomes of surfactant administration involving the use of the LISA technique showed no statistically significant differences as compared with the INSURE method. Due to current WHO recommendations that encourage mothers to breastfeed exclusively up to 6 months of age, quick recognition of any obstacles in the suction mechanism and determining the possible reasons for problems should be a priority. A review of the literature was conducted concerning the diagnosis of ankyloglossia, possible methods of treatment and their efficacy in improving breastfeeding quality. The authors of the research cited claim that any surgica/ intervention should be performed only in cases of symptomatic ankylog/ossia interfering with sucking mechanisms. The most frequent surgical procedure performed in newborns with symptomatic ankyloglossia is frenulotomy. ft is a simple procedure with a low risk of complications. It is listed as one of the possible reasons behind problems with breastfeeding. Due to current WHO recommendations that encourage mothers to breastfeed exclusively up to 6 months of age, quick recognition of any obstacles in the suction mechanism and determining the possible reasons for problems should be a priority. A review of the literature was conducted concerning the diagnosis of ankyloglossia, possible methods of treatment and their efficacy in improving breastfeeding quality. The authors of the research cited claim that any surgical intervention should be performed only in cases of symptomatic ankyloglossia interfering with sucking mechanisms. The most frequent surgical procedure performed in newborns with symptomatic ankyloglossia is frenulotomy. It is a simple procedure with a low risk of complications.
