The FMS model was established by injecting 2-morphine phenol ethanol sulfate into the left anterior tibial muscle. After successful modeling, EA was applied to \"Zusanli\"(ST36) and \"Yanglingquan\"(GB34) on the left hind limb for 20 min, once every other day for a total of 7 times. von Frey test was used to detect the mechanical pain threshold (MPT) of both hind limbs. H.E. staining was used to observe morphological changes of the anterior tibial muscle. Immunofluorescence staining was used to determine the expressions of SP, MC, GFAP and 5-hydroxtryptamine (5-HT) in the lumbar SDHs. Suspension chip method was used to detect the contents of inflammatory factors in the lumbar spinal cord, left anterior tibial muscle and serum.</p><p><strong>Results: </strong>Compared with the blank control group, the difference value of MPT between the bilateral hind-paws, immunofluorescence-positive number of SP and GFAP in the SDHs, contents of TNF-α, macrophage inflammatory protein-1α (MIP-1α), monocyte chemoattractant protein-1 (MCP-1) in the lumbar spinal cord, and the contents of MIP-α, MCP-1, interleukin (IL)-1α, granulocyte-macrophage colony-stimulating factor (GM-CSF) and granulocyte-colony stimulating factor (G-CSF) in the left anterior tibial muscle, as well as TNF-α, IL-1β, GM-CSF and 5-HT contents in the serum were significantly increased (<i>P</i><0.05), while the immunofluorescence-positive number of 5-HT and contents of 5-HT and IL-10 in the lumbar spinal cord, and IL-10 content in the left anterior tibial muscle and serum were strikingly decreased (<i>P</i><0.05) in the model group. In comparison with the model group, the increase of the difference value of MPT between the bilateral hind-paws, immunofluorescence-positive number of SP and GFAP, contents of TNF-α, MIP-α and MCP-1 in the lumbar spinal cord, and the contents of MIP-α, MCP-1, IL-1α, GM-CSF and G-CSF in the left anterior tibial muscle, as well as the contents of TNF-α, IL-1β, GM-CSF and 5-HT in the serum, and the decrease of the immunofluorescence-positive number of 5-HT and contents of 5-HT and IL-10 in the lumbar spinal cord, and IL-10 content in the left anterior tibial muscle and serum were reversed in the EA group (<i>P</i><0.05). Immunofluorescence staining displayed that no apparent changes in the number of MC in the lumbar SDHs after modeling and EA, and MCs presented degranulation in both the model group and EA group. Nerve circuit damage is an important cause of functional impairment after stroke. The neural circuit in the brain is a system of interconnected neurons, and is the only way for information transmission in the human body. Neural plasticity and circuit reconstruction are thought to contribute to post-stroke recovery. Acupuncture can effectively improve neural function damage after stroke by remodeling the neural circuit. In the present paper, we summarized the mechanisms of acupuncture in promoting the restoration of motor, sensory, cognitive and other functions after stroke from: 1) increasing the excitability of motor cortex, enhancing the corresponding functional connectivity between the premotor cortex/adjacent supplementary motor area and the supramarginal gyrus (intercortical circuit); 2) enhancing the effective connection between the cerebellum and sensorimotor cortex (cortico-cerebellum circuit); 3) regulating the enhanced functional connectivity between the cortex and the ipsilateral basal ganglia and the weakened functional connectivity between the cortex and the contralateral basal ganglia (cortico-basal ganglia circuit); and 4) promoting the functional connectivity between the cortex and hippocampus, strengthening hippocampal nerve regeneration and regulating synaptic plasticity (cortico-hippocampal circuits). In summary, current studies have focused on the regulation of acupuncture on the neural circuit between the cerebral cortex and other brain regions and on <i>in vivo</i> imaging mechanisms. AR-induced olfactory dysfunction rats model was established using ovalbumin sensitization. Bilateral \"Yingxiang\" (LI20) acupoints were stimulated with EA (2 Hz/15 Hz, 1 mA) for 10 min, once daily for 14 days. After the intervention, AR symptom scores of each group of rats were evaluated. Olfactory function was assessed using the buried food pellet test. HE staining was performed to observe the morphological changes in olfactory mucosa tissue. ELISA was used to detect plasma IgE, interleukin-1β (IL-1β) and tumor necrosis factor-α (TNF-α) contents in plasma and nasal lavage fluid. Immunofluorescence staining was used to detect OMP and Caspase-3 positive expressions in olfactory mucosa. TUNEL staining was performed to detect olfactory epithelial cell apoptosis condition.</p><p><strong>Results: </strong>Compared with the control group, rats in the model group showed significantly thinner olfactory mucosal epithelium, reduced number and disorganized arrangement of olfactory receptor neurons (ORNs), increased inflammatory cell infiltration, significantly decreased olfactory function, significantly decreased OMP expression in olfactory mucosa (<i>P</i><0.01), significantly increased nasal symptom scores, plasma IgE contents, IL-1β and TNF-α contents in plasma and nasal lavage fluid, significantly increased Caspase-3 expression in olfactory mucosa (<i>P</i><0.01), and significantly increased occurrence of olfactory epithelial cell apoptosis (<i>P</i><0.01). Compared with the model group, rats in the EA group showed significantly thicker olfactory mucosal epithelium, increased number and more orderly arrangement of ORNs, reduced inflammatory cell infiltration, significantly improved olfactory function, significantly increased OMP expression in olfactory mucosa (<i>P</i><0.01, <i>P</i><0.05), significantly decreased nasal symptom scores, plasma IgE contents, IL-1β and TNF-α contents in plasma and nasal lavage fluid (<i>P</i><0.05, <i>P</i><0.01), significantly decreased Caspase-3 expression in olfactory mucosa (<i>P</i><0.01), and significantly decreased occurrences of olfactory epithelial cell apoptosis (<i>P</i><0.01).</p><p><strong>Conclusions: </strong>EA therapy can improve olfactory function of olfactory-malfunctioned AR rat. Through short-range fiber projections, it can integrate somatosensory information and modulate visceral functions. The insular cortex has a close connection with the autonomic nervous system and is a key hub for regulating the autonomic nervous system, making it a potential target for revealing the correlation between acupoints and internal organs. The unique \"visceral localization\" characteristics and the spatial distribution of neuronal functions in the insular cortex provide a neurological basis for elucidating the \"multi-effects of one acupoint\" or \"bidirectional regulation\" of the acupoints. The phosphatidylinositol 3-kinase/protein kinase B (PI3K/AKT) signaling pathway is mainly involved in the regulation of cell proliferation, apoptosis, autophagy and synaptic plasticity, and plays an important role in the occurrence and development of AD. Eight homologous control Zucker lean rats (Lepr&lt;sup&gt;+/fa&lt;/sup&gt;) were taken as the control group. Rats in the EA group received EA treatment at bilateral \"Sanyinjiao\"(SP6) and \"Zusanli\"(ST36), continuous wave applied with a frequency of 15 Hz and a current intensity of 1 mA, 20 minutes per session, once a day. Rats of the medication group were given pioglitazone solution by gavage at a dose of 10 mg/kg, while rats in the control and model groups were given the same dose of 0.9% sodium chloride solution by gavage. The above treatments were all carried out continuously for 5 days a week and rested for 2 days, for a total of 8 weeks. During the experiment, the fasting body weight (FBW) was measured weekly, and the fasting blood glucose (FBG) was detected 1 day before sample collection. After sample collection, the contents of fasting insulin (FINS), free fatty acids (FFA), low-density lipoprotein (LDL), total cholesterol (TC), triglyceride (TG), tumor necrosis factor - α (TNF-α), interleukin (IL)-6, IL-1β, and monocyte chemoattractant protein-1 (MCP-1) in the serum of rats were detected by ELISA, and the insulin resistance index (HOMA-IR) was calculated. The pathological changes of skeletal muscle were observed after oil red O staining. The ultrastructure of rat skeletal muscle was observed by transmission electron microscopy. The protein expression levels of phosphatidylinositol 3-kinase (PI3K), glucose transporter 4 (GLUT4), and the phosphorylation level of protein kinase B (Akt) in the skeletal muscle tissue of rats were detected by Western blot.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Compared with the control group, the FBW, FBG, serum contents of FINS, FFA, LDL, TG, TC, IL-1β, IL-6, TNF-α, MCP-1, and HOMA-IR in the model group were increased (&lt;i&gt;P&lt;/i&gt;&lt;0.05, &lt;i&gt;P&lt;/i&gt;&lt;0.01), the protein expressions of PI3K and GLUT4 in skeletal muscle and the phosphorylation level of Akt protein were decreased (&lt;i&gt;P&lt;/i&gt;&lt;0.01). Oil red O staining showed severe lipid accumulation in skeletal muscle, and transmission electron microscopy showed a large amount of ectopic lipid accumulation in skeletal muscle tissue, swelling and deformation of mitochondria, and fragmentation of some muscle fibers. Mixed methods systematic reviews of acupuncture (MMSR-A) adopts PICOS and SPIDER to formulate the questions; the research objects are expanded to qualitative and mixed studies. This mixed methods use the mixed methods appraisal tool (MMAT) for the methodology quality evaluation or the quality assessment tool of the Joanna Briggs Institue (JBI), and a variety of mixed analytic methods. {"title":"[Acupotomy at trigger points combined with meridian sinew manipulation in treatment of knee osteoarthritis: a randomized controlled trial].","authors":"Huang-Dong Wang, Hua-Yu Wang, Li-Yue Yang, Ji-Jun Liu, Tao Zhang, Jia Zheng","doi":"10.13702/j.1000-0607.20240549","DOIUrl":"10.13702/j.1000-0607.20240549","url":null,"abstract":"<p><strong>Objectives: </strong>To observe the clinical efficacy of acupotomy at trigger points combined with meridian sinew manipulation in treatment of early and mid-term knee osteoarthritis (KOA), and investigate whether its therapeutic mechanism is related to the regulation of inflammatory response.</p><p><strong>Methods: </strong>Seventy-four patients with early and mid-term KOA were randomly divided into the control group (37 cases, with 2 cases dropouts) and the observation group (37 cases, with 1 case dropout). The control group was treated with meridian sinew manipulation for 30 min each time, 5 times a week, for 4 consecutive weeks. The observation group was given acupotomy at trigger points on the basis of the treatment of control group, once a week, for 4 consecutive weeks. Western Ontario and McMaster University osteoarthritis index (WOMAC) scores, short-form McGill pain questionnaire (SF-MPQ) scores, the medical outcome 36-item short form health survey (SF-36) scores, serum inflammatory factors content before and after treatment were compared between the two groups. The clinical efficacy were assessed after the treatment.</p><p><strong>Results: </strong>Compared with those before treatment, the total scores and the scores of pain, stiffness and activity impairment of WOMAC, the pain rating index (PRI), visual analog scale (VAS) and present pain intensity (PPI) scores of SF-MPQ, and the contents of serum tumor necrosis factor (TNF)-α, interleukin (IL)-6 and IL-10 in both groups were significantly decreased after treatment (<i>P</i><0.05), while the scores of the physical component summary (PCS) and the mental component summary (MCS) of SF-36 were significantly increased (<i>P</i><0.05). Compared with the control group, the total scores, scores of pain, stiffness and activity impairment of WOMAC, the PRI, VAS and PPI scores of SF-MPQ, and the contents of serum TNF-α, IL-6 and IL-10 in the observation group were significantly decreased after treatment (<i>P</i><0.05), while the scores of PCS and MCS of SF-36 were significantly increased (<i>P</i><0.05). The PSD model was established by occlusion of the middle cerebral artery and reperfusion (MCAO/R), combined with chronic unpredictable mild stimulation (CUMS). EA was applied to \"Baihui\" (GV20), \"Yintang\" (GV24&lt;sup&gt;+&lt;/sup&gt;), bilateral \"Hegu\" (LI4), and \"Taichong\" (LR3) for 30 min, once daily for 14 d. Rats in the medication group were given paroxetine intragastatically (10 mg/kg) once a day for 14 d. Neurologic impairment was assessed by Zea-Longa score. Behavioral assessment of depression was conducted by sucrose preference test and open field test. The pathologic changes of prefrontal cortex were observed by HE staining. The ultrastructure and mitochondrial morphology of prefrontal neurons were observed by transmission electron microscopy (TEM). Iron ion content in prefrontal cortex was detected by iron ion kit. The contents of glutathione (GSH) and malonicdialdehyde (MDA) in prefrontal cortex were determined by ELISA. Western blot and qPCR were used to detect the protein and mRNA expression levels of glutathione peroxidase 4 (GPX4), solute carrier family 7 member 11 (SLC7A11), lipoxygenase (LOX), lysophosphatidylcholine acyltransferase 3 (LPCAT3), and acyl-CoA synthetase long chain family member 4 (ACSL4) in prefrontal cortex.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Compared with the sham operation group, the sucrose preference rate, the number of crossings in the central area, the total duration of staying in the central area, and the total distance of activity were all significantly decreased (&lt;i&gt;P&lt;/i&gt;&lt;0.05), the iron ion content, MDA content, and the protein and mRNA expression levels of LOX, LPCAT3, and ACSL4 were increased (&lt;i&gt;P&lt;/i&gt;&lt;0.05), while the GSH content, the protein and mRNA expression levels of GPX4 and SLC7A11 were decreased (&lt;i&gt;P&lt;/i&gt;&lt;0.05) in the model group. Following interventions, the above-mentioned indexes were all reversed in the EA and medication groups (&lt;i&gt;P&lt;/i&gt;&lt;0.05). There were no significant differences in the above indexes between the normal group and the sham operation group, the EA group and the medication group. The asthma mouse model was established by repeated intranasal instillation of HDM 50 μg (in PBS) on day 0, 7 and 14 (for immunization), and 25 μg (in PBS) on day 21, 22 and 23 (for sensitization). EA (1 mA, 2 Hz/15 Hz) was applied to \"Dazhui\" (GV14), \"Feishu\" (BL13), and \"Zusanli\" (ST36) for 30 min, once every other day for 5 times. Mice of the DEX group received intraperitoneal injection of dexamethasone (1 mg/kg) once every other day for 5 times after modeling. The mice's general conditions were recorded and their asthma behavioral scores (0 to 9 points) were assessed according to the severity of symptoms of nasal grabbing, itch-scratching, and asthmatic attack. The mice's pulmonary function including forced expiratory volume in 0.1 second (FEV&lt;sub&gt;0.1&lt;/sub&gt;), forced vital capacity (FVC), the ratio of FEV&lt;sub&gt;0.1&lt;/sub&gt;/FVC, and peak expiratory flow (PEF) were measured using a pulmonary function detector. Additionally, the histopathological changes, inflammation scores and airway wall thickness (total bronchial wall area [Wat] / bronchial basement membrane perimeter [Pbm]) in the lung tissues were assessed using H.E. staining, periodic acid-Schiff (PAS) staining, and Masson staining, respectively. The contents of immunoglobulin E (IgE), tumor necrosis factor-alpha (TNF-α) in serum, interleukin-1 beta (IL-1β), and interleukin-6 (IL-6) in bronchoalveolar lavage fluid, and malondialdehyde (MDA), glutathione (GSH), and iron in the serum and lung homogenates were detected using ELISA and bicinchoninic acid (BCA), separately. The protein and mRNA expression levels of Keap1, Nrf2, solute carrier family 7 member 11 (SLC7A11), glutathione peroxidase 4 (GPX4), acyl-CoA synthetase long-chain family member 4 (ACSL4), and transferrin receptor 1 (TFR1) in the lung tissue were detected by Western blot and PCR, separately.
