IDCS arises from antigen-presenting cells, which involve nodal and extranodal sites. The aim of the study is to evaluate a case of IDCS and its diagnostic challenges. A 21-year-old female with no comorbidities, a bone marrow aspirate and biopsy and an excision biopsy of the lymph node shows atypical mononuclear infiltrates that were immunohistochemically positive for S100-diffuse, CD68, LCA and Ki67. Here, we report a single case of IDCS with a tumour arising from the lymph node and bone marrow infiltration. IDCS is an aggressive disease with uncertain behaviour; strong suspicion is required to establish a diagnosis.","PeriodicalId":34670,"journal":{"name":"Apollo Medicine","volume":"44 19","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-08-08","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141929644","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present a case of AL-type amyloidosis and chronic kidney disease (CKD) who is presented with diffuse abdominal pain. A contrast-enhanced CT scan of the abdomen revealed moderate to large amounts of free fluid in the peritoneum, indicating splenic rupture causing haemoperitoneum. The patient underwent emergency exploratory laparotomy and splenectomy. After the operation, the patient was closely monitored in the ICU and was transferred to the ward on postoperative day 3. He was discharged home on day 6. These applications offer multiple benefits in patient education, clinical decision-making, interpersonal communication, research activities, data analysis and administrative affairs. The present scoping review aims to analyse the current role, pitfalls, challenges and future scope of these modalities in diverse fields of medical science. Literature search was made on 9 April 2024, on five databases (PubMed, Scopus, Web of Science, Embase and Google Scholar). A narrative approach was used for the synthesis of results. Our literature search yielded 1024 studies. After the de-duplication of manuscripts using Endnote, 342 articles were identified. After title and abstract screening, 74 articles were included for the next round of screening. Finally, 14 articles were selected for this scoping review.Diverse chatbot applications have been developed at a growing rate for use in medical science. There is a gradual shift towards employing machine learning-based strategies to develop chatbot programs. Chatbots have a significant potential to revolutionise diverse aspects of medicine including patient care, academic activities, and research endeavours. Artificial Intelligence can be highly effective in streamlining the routine functions and activities in medical care. Chatbots have the potential to meliorate the accessibility, efficiency and standard of medical care. However, the need for further validation through high-quality studies to ensure patient privacy, strategies to conform to patients’ data security, accuracy and precision of the technology and mitigating potential pitfalls in its routine utilisation globally are warranted.","PeriodicalId":34670,"journal":{"name":"Apollo Medicine","volume":"44 3","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-07-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141802534","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose of this study was to compare the short-term therapy of young adults with chronic neuropathic pain with gabapentin in order to assess the safety and effectiveness of pregabalin. In cooperation with the Department of Pharmacology at a tertiary care facility in India, the outpatient departments of neurology, orthopaedics, general medicine, and psychiatry undertook this prospective, comparative, randomised, open-label, single-centre cross-sectional study. One hundred and thirty-five patients in Group A received 75 mg of pregabalin and were then given 150 mg of pregabalin twice a day. One hundred thirty-five participants in Group B received 300 mg of gabapentin. The Numeric Pain Rating Scale (NPRS) was used to quantify pain intensity at baseline, 1 month, and 2 months later in order to compare the negative effects of pregabalin to those of gabapentin. When comparing the two groups’ NPRS scores at baseline, 1 month, and 2 months, Group A (pregabalin) had a comparatively lower NPRS score. At baseline, Group A had a mean ± SD score of 8.5 ± 1.25, when compared to Group B with 8.7 ± 1.20. After 2 months, Group A had a mean ± SD score of 2.5 ± 1.50, when compared to Group B with 4.5 ± 1.80, respectively, which was statistically significant with P < .001. This study suggests that gabapentin and pregabalin are equally effective at reducing pain in persons with neuropathic pain. In terms of NPRS score, pregabalin performs better than gabapentin.","PeriodicalId":34670,"journal":{"name":"Apollo Medicine","volume":" 19","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-06-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141367232","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study is directed toward early identification of deafness in neonates born to gestational diabetic mothers that help in the early rehabilitation of the child. Around 50 infants born to mothers with gestational diabetes were taken and screened after obtaining consent from parents and clearance from the ethics committee. Neonates born to gestational diabetic mothers in Father Muller Medical College were included. Critically ill neonates, neonates with hyperbilirubinemia, family history of deafness, use of ototoxic drugs, and low APGAR score babies (<6) were excluded. Detailed family history and information of the patients were obtained. The occupation of both parents, family income, and consanguinity was noted. It was also observed if the antenatal mother has attended ANC clinic regularly. Results: A longitudinal descriptive study was conducted on 50 infants born to gestational diabetic mothers. Hearing loss was assessed using distortion product otoacoustic emission. The majority of gestational diabetic mothers were homemakers followed by salesgirls. The other occupations also included Anganwadi workers, beedi workers, manual laborers, tailors, and farmers. The list also included cooks, hotel receptionist, office assistant, and floor manager. Most of the fathers were drivers ( n = 10) followed by farmers. The list also included fishermen, manual laborers, painters, salesmen, security personnel, waiters, tailors, teachers, office assistants as well as storekeepers. Our study assessed if there was a correlation between hearing loss in infants and gestational diabetes. Hence, in our study, we found that there may not be any relationship between hearing loss and diabetes in pregnancy.","PeriodicalId":34670,"journal":{"name":"Apollo Medicine","volume":"8 34","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141101045","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

As airway management is a crucial factor in the causation of POST, various methods, especially non-pharmacological, are used to reduce the incidence of postoperative sore throat with the use of a small-sized endotracheal tube with low intracuff pressures. The tracheal tube is applied with other agents that may help in reducing the symptoms such as steroids, lozenges, lignocaine, ketamine gargles, inhalation of steroids (also included in pharmacological methods used for reducing the incidence of postoperative sore throat). The aim of this study was to evaluate the efficacy of prophylactic low-dose nebulised dexamethasone for decreasing the incidence of post-extubation sore throat and to evaluate the complications of low-dose dexamethasone nebulisation in patients undergoing surgery under general anaesthesia. The study was done in the Anaesthesiology Department of Maharishi Markandeshwar Institute of Medical Sciences and Research (MMIMSR) Mullana, Ambala, after institutional ethical committee approval and written, informed consent was obtained from the patients from March 2021 to March 2022 with a group size of Group S – 25 patients and Group D – 25 patients. The results showed that patients in the dexamethasone group had a lower POST rate compared to the control group. We concluded that a low dose of 4 mg dexamethasone was effective in reducing the incidence and severity of POST in patients after anaesthesia, and no side effects compared to 4 mg normal saline.","PeriodicalId":34670,"journal":{"name":"Apollo Medicine","volume":"109 10","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-23","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"141105778","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
