We report a case of posterior interosseous nerve palsy in a child with dysplasia. After a physical examination and complementary diagnostic tests that revealed posterior interosseous nerve compression neuropathy, surgical neurolysis og the posterior interosseous nerve was performed at level of the the arcade of Frohse. The satisfactory evolution of the patient confirmed our diagnosis. Attention must be paid to the onset of signs or symptoms of nerve compression in these patients. Resumen La displasia epifisaria múltiple es una condición que se caracteriza por el retraso en la aparición de las fisis, y que puede provocar dismorfias articulares. Presentamos un caso de paresia del nervio interóseo posterior en una niña con displasia. Tras la realización de un examen físico y pruebas diagnósticas complementarias que evidenciaron neuropatía compresiva del nervio interóseo posterior, se procedió a realizar la neurólisis del interóseo posterior a nivel de la arcada de Frohse. La evolución satisfactoria de la paciente apoyó nuestro diagnóstico. Delgado","doi":"10.1055/s-0040-1721045","DOIUrl":"https://doi.org/10.1055/s-0040-1721045","url":null,"abstract":"Abstract Despite being the second most common osteoarthritis of the wrist, little is known about scaphotrapeziotrapezoid osteoarthritis. Not all patients with this type of osteoarthritis have symptoms, and, even in those symptomatic patients, the intensity of the complaint is not proportional to the severity of the degeneration. In symptomatic patients, when the conservative treatment fails, grading classifications to help define the surgical treatment are solely based on the joint characteristics, and fail to assess the rest of the carpus. In general, most carpal degenerative processes show the same evolution pattern, which varies according to the initial injury, along with the type of deformity and the inherent instabilities. Thus, we hypothesize that the degenerative evolution due to primary osteoarthritis of the scaphotrapeziotrapezoid joint also follows a logical evolutionary sequence, similar to the one that occurs in cases of scapholunate advanced collapse (SLAC), scaphoid nonunion advanced collapse (SNAC), scaphoid malunion advanced collapse (SMAC), and scaphoid chondrocalcinosis advanced collapse (SCAC), thus resulting in scaphotrapeziotrapezoid osteoarthritis advanced collapse (SOAC). We have divided the SOAC into three groups, and considered that their treatment shall be guided by the evolutionary stage of the disease, and not by the surgeon's preferred technique. Resumen A pesar de constituir la segunda causa más común de artrosis de la muñeca, poco se conoce sobre la artrosis escafotrapeciotrapezoidea. No todos los pacientes con este tipo de artrosis presentan síntomas, y, incluso en los pacientes sintomáticos, la intensidad de las quejas no es proporcional a la gravedad de la degeneración. En los pacientes sintomáticos, cuando fracasa el tratamiento conservador, las clasificaciones de los grados para ayudar a definir el tratamiento quirúrgico se basan únicamente en las características de la articulación, y no evalúan el resto del carpo. Proper, pertinent diagnosis and treatment are required to avoid salvage procedures. Arthroscopic techniques have emerged as an important tool for the treatment of certain stages of the disease. We present a review of the literature and propose a treatment algorithm according to the stage of the disease, including arthroscopic techniques. We detail the surgical technique of arthroscopic scaphocapitate arthrodesis, and the functional outcomes obtained by our group. Resumen La enfermedad de Kienböck es una condición clínico-radiológica que puede afectar de manera permanente la funcionalidad de la muñeca y la calidad de vida del paciente. Se requiere de un diagnóstico y manejo oportuno para evitar la necesidad de cirugías de salvataje. Las técnicas artroscópicas han aparecido como importante herramienta para el manejo en ciertas etapas de la enfermedad. Realizamos una revisión de la literatura y proponemos un esquema de manejo según estadío de la enfermedad incluyendo técnicas artroscópicas. Vázquez","doi":"10.1055/s-0040-1719014","DOIUrl":"https://doi.org/10.1055/s-0040-1719014","url":null,"abstract":"Abstract Objectives The present study aims to describe a new percutaneous technique for the reconstruction of the ulnar collateral ligament of the metacarpophalangeal joint of the thumb with a tendon graft, and to evaluate the functional outcomes and stability six months postoperatively. Material and methods The percutaneous ligamentoplasty technique was performed with a palmaris longus tendon graft and tenodesis screws in 6 patients with ulnar instability of the thumb (> 30° degrees of radial deviation) for more than 6 weeks. Range of motion, grip strength, and pinch strength (lateral/key pinch, tip pinch, and palmar pinch) were assessed in comparison to the contralateral limb, as well as the Quick Disabilities of the Arm, Shoulder and Hand (DASH) score. Results At 6 months, the joint movement was of 100%, the grip strength was of 94.8%, the key pinch was of 95%, the palmar pinch was of 85.7%, and the tip pinch was of 93.5% compared to the contralateral limb. The internal tilt was of 14° compared to 9° at the contralateral limb. The Quick DASH score decreased from 24.2 to 2.6 points. Conclusion We devised this versatile surgical procedure to obtain early mobility with the advantages related to the percutaneous techniques. Resumen Objetivo El objetivo de este trabajo es presentar una nueva técnica percutánea de reconstrucción del ligamento colateral ulnar de la articulación metacarpofalángica del pulgar con injerto tendinoso, y evaluar los resultados funcionales y la estabilidad a los seis meses postoperatorios. Material y métodos Se realizó la técnica de ligamentoplastia percutánea con injerto de tendón del músculo palmaris longus y tornillos de tenodesis en 6 pacientes que presentaban inestabilidad ulnar de pulgar (> 30° de desviación radial) con más de 6 semanas de evolución. Se valoró el rango de movilidad, la fuerza de agarre (prensión), y la fuerza de pinzas (llave, palmar, y tres puntos) en comparación al miembro contralateral, así como la escala de Quick Disabilities of the Arm, Shoulder and Hand (DASH). Resultados A los 6 meses, el movimiento articular fue del 100%, la fuerza de agarre fue del 94.8%, la pinza de llave, del 95%, la pinza palmar, del 85,7%, y la pinza 3 puntos, del 93.5% en comparación al miembro contralateral. El bostezo interno fue de 14° en comparación con 9° en el miembro contralateral. El Quick DASH disminuyó de 24.2 a 2.6 puntos. Secondary stabilizers include the distal oblique band (DOB), which is inconsistently found. When TFCC repair has failed or cannot be performed, DOB reconstruction is a therapeutic option. Even though distal radioulnar ligamentoplasty remains the technique of choice, recent papers show similar outcomes from both methods. We present two cases of successful DOB repair with the extensor carpi radialis longus (ECRL) hemitendon. Resumen El principal elemento estabilizador de la articulación radiocubital distal (ARCD) es el complejo fibrocartílago triangular (CFCT), y existen además estabilizadores secundarios, entre los que se encuentra de forma inconstante la banda oblicua distal (BOD). Cuando la reparación del CFCT ha fallado o no es posible realizarla, la reconstrucción de la BOD es una opción terapéutica; aunque la ligamentoplastia radiocubital distal sigue siendo la técnica de elección, están apareciendo artículos que manifiestan resultados similares entre ambas técnicas. The purpose of the present study was to evaluate our experience with this procedure and to assess bone growth and digit function as well foot morbidity. Material and Methods We retrospectively evaluated the clinical records for all children with symbrachydactyly submitted to free nonvascularized toe phalangeal transfer between 2002 and 2017. A total of 8 patients were included. We summoned the patients to an appointment to clinically assess the range of motion, the stability, and the alignment of the neo-joint. We radiographically measured the final length and the expected percentage of growth of the transferred phalanx. We also evaluated the foot for comorbidities. Results The mean age at the time of the first surgery was 19 months (range: 8–42 months). A total of 20 phalanges were harvested: 16 total proximal phalanges, 2 middle phalanges, 1 subtotal proximal phalanx, and 1 accessory thumb phalanx. The distal part of one proximal phalanx was trimmed because the skin pocket was too tight. Two patients underwent a secondary procedure to release the syndactyly. One transfer required revision surgery due to distal tip necrosis and exposition of the transferred phalanx. In the present series, the overall clinical and radiographic outcomes were compatible to those reported in other studies. Discussion The main limitation of the nonvascularized toe phalanx transfer is the preexisting soft tissue envelope of the finger and the limited growth potential of the transferred bone. Conclusion Irrespective of the amount of growth achieved in the transferred phalanx, the actual transfer and growth attained should not be viewed as the end result, but rather as a means of providing a stable and functional joint. Resumen Introducción La transferencia no vascularizada y libre de la falange del pie es una opción quirúrgica establecida para la reconstrucción de dedos hipoplásicos. El objetivo de este estudio fue analizar nuestra experiencia con este procedimiento y evaluar el crecimiento óseo y la función digital, así como la morbilidad del pie. Material y Métodos Analizamos retrospectivamente el proceso clínico de los niños (n = 8) con simbraquidactilia que se sometieron a una transferencia no vascularizada y libre de la falange del pie entre 2002 y 2017. Hicimos una consulta para evaluar clínicamente el rango de movilidad, la estabilidad, y la alineación de la nueva articulación. Medimos la longitud final y el porcentaje de crecimiento esperado de la falange transferida. Evaluamos el pie para la comorbilidad. Resultados La edad media al momento de la cirugía fue de 19 meses (rango: 8–42 meses). It is the second most common arm fracture, only surpassed by fractures of the distal radius. Despite being so common, these fractures can be difficult to diagnose and treat due to the anatomic and physiological particularities of the bone, including its precarious vascularization, its complex three-dimensional structure, and its ligament connections, which greatly contribute to the risk of complications such as malunion, pseudoarthrosis and avascular necrosis. Although there are many published studies on the treatment of these injuries, there is still controversy over what is the most suitable one for certain fracture types. The present article is a comprehensive and updated review of the literature. Combining strategies for clinical and radiological diagnosis, we propose a complete algorithm for the diagnosis of scaphoid fractures based on the varying availability of resources, and we also describe the most appropriate therapeutic approach for the different types of acute fractures of this bone. Resumen El escafoides es el hueso del carpo que con mayor frecuencia se fractura; comprende hasta el 70% de las fracturas carpianas, y el 11% de las de la mano. Estas fracturas constituyen las segundas en frecuencia de todo el miembro superior, sólo superadas por las fracturas de la extremidad distal del radio. A pesar de ser tan frecuentes, pueden ser difíciles de diagnosticar y de tratar debido a las particularidades anatómicas y fisiológicas del hueso, incluyendo su precaria vascularización, su compleja estructura tridimensional, y sus conexiones ligamentosas, que contribuyen al riesgo de complicaciones como la unión en mala posición, la pseudoartrosis, y la necrosis avascular. Aunque existan numerosos estudios publicados sobre el tratamiento de estas lesiones, aún existe controversia sobre cuál es el más adecuado para determinados tipos de fractura. En este artículo se hace una revisión completa y actualizada de la literatura. The aim of the present study is to analyze the radiological and functional results after a protocolized surgical treatment. Material and Methods A retrospective review of case series of patients with diagnosis of nonunion of the distal radius surgically treated from 2010 to 2016. The diagnosis of nonunion was made at 6 months without signs of consolidation. According to computed tomography (CT) scan images, we propose a classification of this complication as type I: no deformity or mild deformity, with subchondral metaphyseal bone stock beneath lunate facet greater than 10 mm, with no degenerative changes at the distal radioulnar joint (DRUJ); type II: moderate deformity or degenerative changes in the DRUJ, distinguishing between IIA and IIB based on the subchondral metaphyseal bone stock; and type III: degenerative changes at the midcarpal joint. In all cases, a reconstructive technique consisting of volar locking plate osteosynthesis with autologous tailored iliac crest graft was performed, hence none of the cases were classified as type III. Results In total, six cases were included. Mean follow-up: 58 months (range: 30–108 months). Consolidation was achieved in a mean period of 5 months (range: 3–9 months). Average active range of motion: flexion: 54° (range: 10°–80°); extension: 40° (range: 10°–85°); supination: 82.5° (range: 75°–85°); pronation 80° (range: 55–85°); radial deviation: 15° (range: 5°–20°); and ulnar deviation 15° (range: 5–20°). Mean strength: 24.15 Kg (6.9–35 Kg). The average score on the Visual Analog Scale (VAS) at rest was of 0.6 (0–2), and with movement, it was of 1.8 (0–4). The mean postoperative score on the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire was of 18.48 (range: 0–50.8). Average radial height: 9.65 mm (range: 7–12 mm); average radial angulation: 17° (10°–20°); volar tilt: 8.36° (range: 0.2°–21°); average ulnar variance: 1.52 mm (range: 0–3.5 mm). Complications: persistent nonunion, superficial wound infection, acquired residual ulnar club hand deformity, radiocarpal arthritis, and midcarpal arthritis. Nevertheless, no salvage techniques were needed or requested during the follow-up, with all patients in an active work situation or living an active life in complete normality. Conclusions The protocolized surgical treatment of pseudoarthrosis of the the distal radius, based on our proposed classification, offers good functional outcomes in the long-term follow-up. One Of Them Suffered An Assault and the other a high-energy accident. Both cases were misdiagnosed. The first case presented an injury of the dorsoradial capsule of the metacarpophalangeal (MCP) joint and the second case presented an injury of the adductor pollicis (AP) which is an unusual finding regarding this kind of deformity. The surgical treatment failed up to two times in the first case hile in the second case it failed once. Resumen Se presentan dos casos de deformidad en boutonnière tras traumatismo en pacientes sin antecedentes de enfermedad reumatológica. Uno de ellos había sufrido una agresión, y el otro, un accidente de alta energía. Ambos fueron diagnosticados tardíamente de forma clínica. En el primer caso, se encontró una lesión de la cápsula dorsoradial de la articulación metacarpofalángica (MCF), mientras que en el segundo se encontró una lesión del aductor del pulgar (AP), hallazgo novedoso en este tipo de deformidad.
