{"title":"Urothelial bladder cancer in young age group: Presentation, clinical behavior and outcome","authors":"Pishtewan Hashim Saeed, Bayar Abdullah Ahmed","doi":"10.22317/imj.v6i4.1205","DOIUrl":"https://doi.org/10.22317/imj.v6i4.1205","url":null,"abstract":"Objectives: The main aim of our study is to review the clinical presentation, histopathological features, staging and grading, as well as the recurrence, and progression (for 1 year) of UBC in patients less than 40 years age. \u0000Methods: We retrospectively reviewed records of 42 newly diagnosed patients with UBC between May 2016 to May 2020 at the Rizgary Teaching hospital, and Zheen International hospital in Erbil city, Iraq. \u0000Results: The mean age ± SD of the sample were 29.05 ± 7.05 years, ranging from 17 to 39 years. The median was 28.5 years. Patients were divided into three groups, Group I (6 patients) were aged ≤ 20 years, Group II (17 patients) were aged between 20-29 years old, and Group III (19 patients) were aged between 30-39 years old. The majority (78.6%) of the cases were males. Gross hematuria (73.8%) is the most common presenting symptom. The proportion of smoker patients was 61.9%. Significantly higher proportions of patients with lower grade and stage and small tumor size were detected that comparable with previous studies that support the bladder cancer in young-aged group with lower grade and stage. {"title":"Effects of single versus multiple tracts on Percutaneous Nephrolithotomy outcomes","authors":"Shakir Saleem Balindi, Khidhur Saleh Khidhur","doi":"10.22317/imj.v6i4.1209","DOIUrl":"https://doi.org/10.22317/imj.v6i4.1209","url":null,"abstract":"Objectives: The study designed to compare outcomes and morbidities in patient undergoes multi tracts percutaneous nephrolithotomy (PCNL) versus single tract PCNL. \u0000Methods: From October 2019 to September 2020, 50 patients were prospectively investigated at Azadi Teaching Hospital and Vajeen Private Hospital in Duhok, Iraq. Group A included 25 patients who received single tract PCNL while group B had 25 patients who underwent multiple tract PCNL. Complex renal stones are treated with single or multiple tract PCNL depending on stone configurations and urinary system architecture utilising Guy's stone score. Laboratory and imaging data were correlated with stone size, stone-free rate, mean haemoglobin decrease, operation time, hospital stay, and postoperative problems. \u0000Results: 64% of cases are male and 36% are female. The mean stone size in Group A was 3.3 cm (ranges 2.0 - 4.9 cm), but in Group B it was 5.1 cm (ranges 3.3 - 7.4 cm), which is substantially bigger. Group A's mean operational time was 64.4 minutes (ranges from 30-115 minutes), which is statistically significant compared to multiple tracts' 80.2 minutes (ranges from 45-120minutes). The decline in mean hemoglobin between the two groups is statistically significant. Group A and B had 8% and 12% blood transfusions, which is not significant. Group A had a mean hospital stay of 28.2 hours (20-48 hours), whereas group B had 40.8 hours (ranges 24-96 hours). Suliman","doi":"10.22317/imj.v6i4.1213","DOIUrl":"https://doi.org/10.22317/imj.v6i4.1213","url":null,"abstract":"Objectives: The aim of this in vitro study was to assess the physical and mechanical properties of resin cements to ceramic substrate using different curing modes by evaluating the degree of conversion and bond strength of these cements. \u0000Methods: Hundred twenty square lithium disilicate ceramic discs specimens of three thicknesses (0.5, 1 and 1.5 mm) and 10x10mm diameter were bonded to 120 bovine teeth using two resin cements (one light cure and one dual cure) and using two curing modes (ramp and continuous). As a result, 12 experimental groups were established for bond strength testing. degree of conversion of luting cements was evaluated using Fourier transform infrared spectroscopy (FTIR). \u0000Results: using the continuous mode compared to the ramp after controlling the type of resin and material thickness showed no significant difference(P>0.001) in SBS and DC. the effect of changing the material thickness between 0.5mm, 1mm and 1.5mm after controlling the resin type and mode of light curing, showed significant difference (p<0.001) from 1mm to 1.5mm using light-cured resin and continuous curing mode for SBS. while for the DC these correlations between different material thicknesses and both light cure type of resin and continuous light-curing mode was significant (p<0.001), it was also significant with dual cure cement. Lucero-Prisno, Dina Shehata","doi":"10.22317/imj.v6i4.1186","DOIUrl":"https://doi.org/10.22317/imj.v6i4.1186","url":null,"abstract":"Objective: This study aims to assess the ability and the safety of fasting among patients who underwent PPCI within two specified periods.\u0000Methods: This study was a Retrospective Cohort Study with a convenience sample size of 200 consecutive patients who were divided into two groups based on the duration of their last primary PCI for an attack of acute myocardial infarction (AMI) and the start of the month of Ramadan. The patients were admitted to the Causality Department of the Surgical Specialty Hospital-Cardiac Center, Erbil/Iraq. Group I included patients undergone PPCI 6 weeks or less before the start of Ramadan while group II included patients undergone PPCI for more than 6 weeks before the start of the fasting month of Ramadan. Furthermore, each of these two groups was subdivided based on their ability to complete the Ramadan Intermittent Fasting (RIF) with no recurring or worsening of symptoms into those who fasted the whole Ramadan month, those who did not fast, those who could not continue fasting, and those who passed away. At the end of the RIF, patients’ symptoms were recorded along with their status according to the New York Heart Association (NYHA).\u0000Results: In a comparison of the proportion of fasting and non-fasting patients among the two groups, 14% in group I were able to complete the 30 days of intermittent fasting during the Ramadan month while this percentage in group II was 54% (P-value=0.001). Among our findings, there were significant associations between post-PCI symptoms (exertional shortness of breath and palpitations) and RIF (P-values=0.001and P-values=0.004, respectively). We also aimed to analyze some prognostic clinical parameters (age, stage, grade). \u0000Methods: This cross sectional study was carried out on paraffin embedded surgical specimens of endometrial tissue. Applying the immune histochemical techniques by using the GLUT-1 as a primary antibody, statistical analysis was done and the correlation with different clinical and pathological parameters were assessed. \u0000Results: 98 cases of endomertial tissue, 17.4% disordered proliferative endometrium, 22.4% endometrial hyperplasia without atypia, 18.4% endometrial hyperplasia with atypia, 41.8% endometrioid adenocarcinoma, 56.1% were GLUT-1 positive. Significant correlation was found between GLUT-1 expression and increasing degree of atypia as it was negative in benign proliferative and hyperplasia without atypia meanwhile positive in hyperplasia with atypia and endometrioid carcinoma. Colony morphology, gram staining, and VITEK 2 compact were used for bacterial identification, antibiotic profile and ESBL screened phenotypically by antibiotic profile results and double disk synergy test and confirmed by combined disk test methods. Conventional PCR was used to detect classⅠ Integron by using a specific primer and the result was analyzed, sequenced and uploaded into NCBI. \u0000Results: Among four hundred samples the prevalence rate of Enterobacteriaceae was 67.03 which includes 86 E. coli, 32 Klebsiella pneumoniae, and two isolates of Proteus mirabilis, according to the antibiotic profile, the most sensitive antibiotic among all three isolates were Imipenem, meropenem, and nitrofurantoin, while most resistance antibiotic were nalidixic acid and third generation cephalosporin. The prevalence rate of ESBL-producing Enterobacteriaceae was 56.6% by the confirmatory test which was a combined disk test, and the prevalence rate of classⅠintegron was 54.2%. \u0000Conclusion: A significant majority of the isolates under study had integrons gene and were ESBL positive. Al-Tu’ma","doi":"10.22317/imj.v6i3.1199","DOIUrl":"https://doi.org/10.22317/imj.v6i3.1199","url":null,"abstract":"Objectives: This work aimed to study the association between KIM-1, Calprotectin and Cystatin C and other biomarkers and their roles for early detection of the severity of Covid-19 associated with kidney injury complications that lead to death in severe Covid- 19 patients. \u0000Methods: This study included 91 samples patients with Covid-19 divided into two group, 46 patients of them with severe Covid-19 and 45 patients of them with moderate infected Covid-19 include (56 male, 35 female) with a matched age ranged between (22-88) years patients. The study was conducted from Al-hayat unit, Al-Hussein Teaching Hospital, Al-Hussein Medical City during the period from (Oct., 2020 to July., 2021). The biomarkers determined include: ferritin, C-reactive protein (CRP), total lactate dehydrogenase (LDH) activity, creatinine, urea, cystatin C (Cys C), calprotectin (CLP), and kidney injury molecule-1 (KIM-1) beside complete blood count test (CBC) which were determined by using various biochemical techniques including ELISA. Results: The results of data which include 46 patients with severe Covid-19 group and another 45 patients with moderate Covid-19 infection, there is significant data obtained for ferritin, LDH and RDW-CV when compare between severe and moderate group of Covid-19 patients (p<0.05) while other parameter showed a non-significant increased result. There is positive correlation between KIM-1 and Cys C in severe and moderate group (p< 0.01) and with creatinine (p<0.01) and urea (p<0.05) in severe Covid-19 group, and significant correlation between Cys C and creatinine in severe group (p<0.05). \u0000Conclusion: There is a significant correlation between KIM-1 and CysC and serum creatinine with Cys. C and KIM-1. Also, the obtained results illustrated that CLP, Cys C and KIM-1were non-significant increase in Covid-19 patients groups, but ferritin and LDH had increment significantly when compare between severe and moderate group and can be served as parameter predictor and increased mortality risk for Covid-19 patients. It is a deformity in metatarsophalangeal joint of big toe because of laxity of joint capsule and ligaments resulting in failure of stabilization of the affected joint. High heels, tight and narrow tiptoe box pointed shoes are potential apparent causes of this pathology. Initial treatment remains conservative with large toe box shoes, pain control medicines and surgery as a last choice. When indicated, surgical treatment is controversial because of the greater than 100 described surgical procedures and a historically high prevalence of recurrence. Platelet rich plasma (PRP) is an autologous treatment and pain management method came out in last twenty years, now it is so popular in orthopaedic, rheumatology and sport medicine practices because of it’s effectiveness and cheapness and safety. {"title":"Misconceptions of COVID-19 pandemic among a sample of general public in Baghdad Iraq 2021: A Cross sectional survey","authors":"Hawraa Basim Jawad, Asmahan Abdul Kadhim Qasim","doi":"10.22317/imj.v6i3.1193","DOIUrl":"https://doi.org/10.22317/imj.v6i3.1193","url":null,"abstract":"Objectives: to determine the misconceptions regarding COVID -19 pandemic among adults attending the three primary healthcare centers in Baghdad, Iraq. \u0000Methods: Three primary health care centers were conveniently chosen to recruit adults attending them (using systematic sampling technique of every fifth attendee) and interview them to answer a special questionnaire form concerned with globally and locally spreading myths and misconceptions regarding COVID-19 pandemic and consisted of two part one for reporting demographic characteristics and the other consisted of 19 questions to assess people perceptions towards COVID-19 pandemic. Each question had two scores either zero for wrong response or one for correct one. Scores below 10 were considered bad perception and ≥10 were considered good perceptions. \u0000Results: The study included 400 participants; 235 (58.8%) were females with a female: male ratio of 1.4:1, their age ranged from 18-82 years with a mean of 40.2 ± 14.6 years SD, more than half of them (224; 56.0%) were ≤40 years of age, 272 (68.0%) were either governmental employee or working in private sector, 214 (53.4%) were with college education and higher and 300 (75.0%) were ever married. These participants were submitted to The Sniffin’ Sticks test in order to identify those with persistent olfactory dysfunction who were treated either by olfactory training combined with a 10-day course of oral corticosteroids, or by olfactory training alone. \u0000Results: Cross sectional comparative study for 50 patients have history of COVID 19 infection, mean age of patients [47 ± 10] years. [56%] of patients at age group ≥ 45 years, [44%] of patients are females and [56%] are males.
