Ivaniv","doi":"10.24969/hvt.2022.334","DOIUrl":"https://doi.org/10.24969/hvt.2022.334","url":null,"abstract":"Transthoracic echocardiography (TTE) showed normal left ventricular systolic function (ejection fraction 65%), left ventricular hypertrophy (interventricular septum 1.5 cm, posterior wall 1,5 cm), left atrial dilatation (4.8 cm), right ventricular dilatation (3.2 cm), normal left ventricular end-diastolic diameter (5.2 cm) and a cystic-like structure (aneurysm) in the right ventricle adjacent to the interventricular septum (see Fig. 1, 2 – p. XX), located from the base of the septum to its mid-ventricular portion. In color Doppler imaging, turbulent flow was seen from the aorta into the aneurysmatic sac and from its cavity into the right ventricle (RV) (see Fig. 1, 3 pXX). Continuous-wave Doppler revealed continuous high-velocity flow from the aorta into the aneurysm and from its cavity into the RV during both phases of the cardiac cycle (systole and diastole) (see Fig. 4 – page XX). The estimated Qp/Qs ratio was 1.54. Systolic pulmonary artery pressure was 60 mm Hg. Aortic valve was tricuspid with mild regurgitation. Right sinus of Valsalva aneurysm (SVA) with interventricular septum dissection and rupture into the RV was suspected. Contrast-enhanced multidetector computed tomography (MDCT) confirmed the diagnosis (Fig. 1, 2). Syphilis and other infections were excluded during differentiation between congenital and acquired sinus of Valsalva aneurysm. The patient had a fall from height 2 years earlier, which could be a trigger for rupture of his congenital malformation. Coronary angiography showed normal coronary arteries. The anomalous origin of the circumflex (LCX) of the right coronary artery (RCA) is one of the common anomalies. We describe a case of CX orifice arising from right coronary sinus in a ``shotgun`` manner with orifice of RCA and successful percutaneous coronary intervention (PCI).\u0000\u0000Case Report: In this case, the anomalous incidental finding was the arising of the CX artery in “shotgun” shape with the RCA (separate ostium) in a clinic scenario of anterior ST-segment elevation myocardial infarction wherein the patient received fibrinolytic therapy with alteplase, presenting reperfusion criteria (&qt;50% ST-segment resolution at 60 minutes and relief angina) and then underwent to successful PCI.\u0000\u0000Conclusion: The ectopic origin of the CX is a well-recognized variant, which is considered the most common coronary anomaly. Although this anomaly is classified as benign and asymptomatic, a few cases of sudden death, myocardial infarction, and angina pectoris in the absence of atherosclerotic lesion have been reported. We shared successful PCI in such coronary anomaly in a patients with ST elevation anterior myocardial infarction. MB is formed by a segment of a coronary artery taking an intra-myocardial course. As of date, huge knowledge gap exists in the literature in terms of MB and its associated complications.\u0000\u0000Methods: This single-centre, observational, non-randomised study was conducted over a period of 20 months to identify the rare anomalies such as MB using coronary angiograms.\u0000\u0000Results: Eight of 400 cases screened have been found to have MB bridging in mid left anterior descending artery (LAD) segment. The male to female ratio was 7:1 all within an age range of 24 years to 45 years. Most of the patients were asymptomatic with no history of drug usage (6/8) and family history (3/8) of coronary disorders. In symptomatic cases, the most common presenting symptoms were observed to be typical chest pain, dyspnea, nausea, light-headedness, or headaches, diaphoresis, and tachycardia. In most of the patients, MB showed systolic shortening with resultant flow limitation on coronary angiography. The segments were reported to be 1 mm to 10 mm in size and traversed 1 cm to 3 cm length. All the patients were treated symptomatically and were kept on follow-up. No mortality was observed in any case with MB.\u0000\u0000Conclusion: Even though MB is a benign condition, but its vast array of related complications was reported to be life threatening and require prompt diagnosis and treatment. The global case fatality rate ranged from 6-7% in United States, Switzerland, Ireland, 12-14% in Sweden, Spain, Netherlands, United Kingdom, Italy, and 15-16% in France and Belgium among hospitalized and non-hospitalized COVID-19 patients, the prevalence of underlying cardiovascular diseases was 9%. The prevalence of cardiovascular diseases among hospitalized COVID-19 patients and those requiring intensive care unit admission were 23% and 29% respectively.
