{"title":"Clinical manifestations and treatment outcomes of pulmonary aspergilloma","authors":"Kiran Ganta, Popuri Madan","doi":"10.24969/hvt.2023.432","DOIUrl":"https://doi.org/10.24969/hvt.2023.432","url":null,"abstract":"Objective: In developing countries like India where tuberculosis is highly prevalent, the incidence of pulmonary aspergilloma is also high. Mortality and morbidity associated with surgical treatment of aspergilloma in previous studies has discouraged surgeons from taking up surgery as the preferred treatment. This study focuses on the clinical outcome of patients with pulmonary aspergilloma and its surgical management. Methods: Fifty-one patients who underwent thoracotomy for pulmonary aspergilloma from 2015 to 2017 were studied retrospectively by reviewing their medical records. Results: The most common clinical presentation of pulmonary aspergilloma was hemoptysis, which occurred in 76.0% of patients. Tuberculosis was the most common pre-existing disease (72%). High resolution computed tomography of chest, showed the typical \"air-crescent\" sign in all patients. Systemic antifungal therapy neither palliated the clinical symptoms nor eradicated the aspergilloma, and transarterial embolisation was also unsuccessful. Surgery offered the only chance of cure for both unilateral and bilateral disease. Procedures varied from segmentectomy to pneumonectomy with most (75%) undergoing lobectomy. There was no death following surgery and postoperative complications occurred in 5 patients. Postoperatively, most of the patients were symptom-free. To formulate single stage safe strategy for redo repair of aortic coarctation and simultaneous coronary artery bypass grafting (CABG). Case presentation: The present case report describes a 55-year-old male, who underwent a redo safe single -stage surgical repair for both pathologies. CoA was tackled by ventral aortic repair followed by coronary artery bypass grafting under cardiopulmonary bypass. The vascular prosthesis was anastomosed to descending thoracic aorta in end-to-side fashion, and it was located posterior to the inferior vena cava through the oblique sinus but inferior to the superior vena cava and curved around the right atrium, anastomosed to ascending aorta. Conclusion: Ventral aortic repair through a midline approach is our preferred technique for redo surgical repair of CoA. Patients with Mondor's disease, usually middle-aged women, have a cord-like, subcutaneous tender induration with or without infiltration of the skin and subcutaneous tissue that tends to resolve on its own within three to four weeks. However, these cases are fairly rare and the lack of clear diagnostic criteria makes them often misdiagnosed. In 50–60% of cases, no cause is identified (primary idiopathic disease), and in 40–50% of cases, some factors may be present, such as pregnancy or breast cancer. We aimed to demonstrate case series of MD of the breast, as a replenishment of the clinical base of this pathology. Methods: All patients underwent clinical examinations and breast sonography with triplex Doppler. Results: We described the clinical and sonographic picture of thrombophlebitis of the superficial veins of the mammary glands in 5 patients with an idiopathic course, during pregnancy, and in combination with breast cancer to indicate clear diagnostic criteria. Conclusion: Thus, the clinical cases presented in the article demonstrate different clinical features of Mondor's disease with a single outcome. Valvular lesions are relatively common in developing countries and these can mount to significant risk during pregnancy if not investigated and managed with a multi-disciplinary approach Case presentation: We report case of a pregnant patient who was diagnosed to have a cardiac lesion intra-operatively during cesarean section when the patient’s condition deteriorated on table. Patient was diagnosed to have bicuspid aortic valve with severe aortic stenosis and severe mitral regurgitation. Patient was taken for emergency surgery and operated for double valve replacement (DVR).
