The aim of our study is to investigate left atrial mechanical function in healthy subjects and patients with atrial fibrillation paroxysms less than 48 hours with low risk of thromboembolic complications at 1 and 28 days after restoration of sinus rhythm.\u0000\u0000Methods: Thirty-six healthy controls and patients with paroxysms of non-valvular atrial fibrillation less than 48 hours (n=25) with low-risk CHA2DS2VASc  score were included in the study. The mechanical function of the left atrium  (LA) was investigated by the 2D speckle tracking program. Transthoracic echocardiography was performed on the 1st and 28th days after restoration of sinus rhythm by pharmacological cardioversion.\u0000\u0000Results: Despite the absence of differences in left ventricular systolic function and LA size between the groups, there was a significant decrease in LA deformation and strain rate on day 1 immediately after sinus rhythm recovery in the patients. In the pump phase, the decrease was about 3%, reservoir phase – 10%, and conduit phase – 7% compared to the control group. The strain rate also decreased in the patient group: -1.5 s-1 versus control: -2.14 s-1 (p<0.0001). On the 28th  day after restoration of sinus rhythm, there were significant changes in reservoir and conduit functions in patients group in comparison with control group. Global longitudinal LA strain (reservoir phase) decreased to 7%, conduit phase decreased to 11% compared with the control group. In patients group we can see the dynamics during 28 days: pump phase and strain rate were significantly improved, however reservoir and conduit did not significantly change and were lower than in healthy.\u0000\u0000Conclusion: Mechanical function of the LA in patients with non-valvular atrial fibrillation and low risk of thromboembolic complications, studied with speckle tracking on the first day was significantly impaired compared with the control group (reservoir phase and strain rate). Pişkin","doi":"10.24969/hvt.2022.351","DOIUrl":"https://doi.org/10.24969/hvt.2022.351","url":null,"abstract":"In recent years, coronary computed tomography angiography (CCTA) has made significant advances in both imaging technology and clinical validation of CCTAderived interpretations. During this period, imaging protocols were optimized according to image quality, diagnostic accuracy and radiation dose. Indeed, interpretation standards are as important as image quality. In general, standardized reporting helps reduce variability among image interpreters. Because of standardized reporting, it is always expected to be beneficial to link the final opinion in the CCTA report with recommendations for subsequent patient management. The first Coronary Artery Disease Reporting and Data System (CAD-RADS) was launched in 2016 to standardize the reporting system for patients with suspected or known coronary artery disease undergoing CCTA in an outpatient, inpatient, or emergency setting, and was created for the purpose to guide the next possible pathways in patient management (1). The main goal of CAD-RADS was to create standardization of report terminology for CCTA results in order to improve communication between image interpreter and referring physicians in a clear and consistent fashion and to better guide clinical decisionmaking. The proposed CAD-RADS classification was applied on a per-patient basis and represents the highest-grade coronary artery lesion documented by CCTA. The implementation of the first CAD-RADS system included application in two different clinical settings (patients presenting with stable chest pain and patients presenting with acute chest pain), description of the most severe coronary artery luminal stenosis (for vessels ≥1.5 mm in diameter), and three modifiers (stent, graft, and vulnerability). In July 2022, a new expert consensus document on Coronary Artery Disease-Reporting and Data System (CAD-RADS 2.0-2022) was published by the Society of Cardiovascular Computed Tomography in collaboration with the American College of Cardiology, the American College of Radiology and the North America Society of Cardiovascular Imaging (2). New additions for the updated expert consensus recommendations include an emphasis on the reporting of ischemia findings and coronary plaque burden on CCTA. While medical management and mechanical circulatory support have undergone significant advancement in recent years, orthotopic heart transplantation (OHT) remains the most definitive therapy for refractory HF. OHT has seen steady improvement in patient survival and quality of life (QoL) since its inception, with one-year mortality now under 8%. However, a significant number of HF patients are unable to receive OHT due to scarcity of donor hearts. The United Network for Organ Sharing has recently revised its organ allocation criteria in an effort to provide more equitable access to OHT. Despite these changes, there are many potential donor hearts that are inevitably rejected. Arbitrary regulations from the centers for Medicare and Medicaid services and fear of repercussions if one-year mortality falls below established values has led to a current state of excessive risk aversion for which organs are accepted for OHT. Furthermore, non-standardized utilization of extended criteria donors and donation after circulatory death, exacerbate the organ shortage. Data-driven systems can improve donor-recipient matching, better predict patient QoL post-OHT, and decrease needless organ waste through more uniform application of acceptance criteria. To describe cases of infectious endocarditis (IE) in in a cohort of adults with IE in a Brazilian center and to review data from the recent literature on the subject.\u0000\u0000Methods: Cases of definite IE by the modified Duke criteria in adults were retrieved in a contemporary cohort of adults with IE from January 2006 to December 2020.  Literature review on the topic was carried out and case reports on PPIE from 2014 to 2020 were compiled.\u0000\u0000Results: Two cases of PPIE were found in 401 adult patients with IE, with a prevalence of 0.5% in the cohort, of 2/139 (1.4%) among women and 2/83 (2.4%) of women of reproductive age (18 -49 years) in this same cohort. The search for published case reports from 2014 to 2020 resulted in 61 episodes of PPIE; in these, 7 pregnant women (11.4%), 10 fetuses (16.4%) and 1 premature newborn (1.6%) died. There was a previous valve predisposition in 12 (19.7%) cases and intravenous drug use in 14(23%). Left-sided valves were the most frequently affected in 41 (67.2%) of the cases. The most common isolated infectious agent was methicillin sensitive Staphylococcus aureus in 18/61 (29.5%) patients, although as a group, oral viridans streptococci accounted for 16/61 (26.2%). Valve surgery was done for 70% of patients.\u0000\u0000Conclusions: Intravenous drug use remained to be the main risk factor for IE. Mortality remained high for mothers and their children, despite the fact surgery was done more frequently in the recent cases.
