Results and conclusion: A total of 80 patients were enrolled in this study and placed in four groups of 20 patients each in Groups A,B,C,D. SNOT-20 questionnaire was basis of symptomatology before and after the intended topical therapy for 8 weeks and objective outcome by diagnostic nasal endoscopy before start and again at culmination of therapy. It is associated with the presence of bacteremia due to PA, in immunocompromised patients.1 It is characterized by producing ulcers surrounded by an erythematous halo, and as it progresses, a black eschar is also added.2 A differential diagnosis should be made with acute invasive fungal rhinosinusitis, which also occurs in immunocompromised patients. The initial symptoms are usually not very specific, including fever, nasal obstruction and rhinorrhea. R. seeberi has been variously classified as a prokaryote cyanobacterium of the genus Microcystis, a eukaryote pathogen belonging to the class Mesomycetozoea, and a fungus.1 Yet unresolved enigmas in Rhinosporidiosis include the mode of infection mechanisms of spread, mechanisms of immunity, some aspects of histopathology e.g. the significance of trans epidermal elimination of sporangia, the cause of the variation in cell infiltration patterns in Rhinosporidial tissues and their correlations with immune status, and the absence of the Splendore-Hoeppli reaction which is wellmarked in invasive, classical mycoses.2–4 It is a chronic disease, with frequent recurrence after surgery, and occasional dissemination from the initial focus which is most commonly seen in upper respiratory sites. It occurs universally, although it is endemic in south Asia, notably southern India and in Sri Lanka. Complications associated with it still poses a major problem in the developing countries. The anatomical proximity of the middle ear cleft and mastoid air cells to the extracranial and intracranial compartments places structures located in these areas at a high risk of developing infectious complications. Complications are usually seen more in the rural population than in urban population. The incidence of chronic suppurative otitis media related complications has significantly reduced since the introduction of antibiotics, but this problem has not been completely eliminated. The purpose of the auditory ossicles (or ossicular chain) is to transmit sound via a chain reaction of vibrations that connects the eardrum to the inner ear and cochlea. The auditory chain reaction starts when sound reaches the tympanic membrane. The vibrational pressure is passed to the malleus, an articulated bone that flexes at one of the two incudomalleolar joints. The vibration is then passed to the incus which flexes at another incudomalleolar joint before transferring the impulses to the stapes. The role of auditory ossicles is complete when the vibrational input is passed to the cochlea through the oval window. The auditory ossicles serve an important role in hearing in that they transition sound waves from the air to the fluid-filled core the cochlea. Vascular malformations are seen in about 1% of the population. But, many of them do not present themselves for treatment.1 Progression of AVM is commonly induced by puberty, trauma and pregnancy. Those induced by trauma usually involve a single vessel whereas congenital form of AVM usually involves multiple vessels.3 Most AVMs are present at birth, but become clinically significant later1.
