Tympanometry is a simple diagnostic instrument that measures middle ear (ME) pressure, compliance, ear canal volume, and the width of tympanogram. While WNL ranges are based on average values from the population, they often do not account for individual differences, leading to possible misdiagnoses. Conditions such as AV, caused by asymmetric ME pressure, can show symptoms even when tympanometry results are within normal ranges. The editorial suggests a more thorough diagnostic approach that includes detailed patient histories, dynamic testing methodologies, and additional hearing and balance tests to improve diagnostic accuracy and patient outcomes. Symmetry in tympanometry data is crucial, and the Valsalva maneuver should be avoided due to potential harm. However, laryngeal involvement can also be potentially serious, with the potential for upper airway obstruction and other complications. Treatment typically involves systemic corticosteroids, which are often effective in managing symptoms. In some cases, more invasive procedures such as tracheotomy or mechanical ventilation may be necessary. Aim: Emphasize the involvement of the larynx in systemic lupus erythematosus and the crucial role of clinicians in recognizing its symptoms to consider a potential diagnosis. Case report: We report two cases of isolated dysphonia revealing systemic lupus erythematosus in two young adult females who had no other clinical signs of systemic lupus erythematosus. Conclusion: Due to the potential seriousness of laryngeal involvement in systemic lupus erythematosus, it is important for clinicians to maintain a high degree of suspicion for this condition in patients with systemic lupus erythematosus, particularly in cases where there are unexplained respiratory or throat symptoms. PM can disrupt the nasal epithelial barrier, leading to vulnerability to respiratory disease. Nasal saline washing can help support nasal functioning by removing trapped PM. We aimed to determine which nasal saline administration technique provided the best intranasal saline deposition and to assess the effectiveness of intranasal saline cleansing solutions for removing PM-simulating dust. Methods: We conducted 3 in vitro studies using a nasal cast coated with an artificial mucus. Study 1 evaluated the deposition patterns of 3 nasal sprays administered with different techniques. A lateral image was taken after each administration to quantify the exposure area. Studies 2 and 3, in which PM-simulating dust was added to the nasal cast, evaluated the effectiveness of 4 intranasal saline sprays administered with the line-of-sight (LoS) method (head tilted sideways 45°, spray angle 0° from vertical) for washing away PM-simulating dust. The percentage of PM removed was quantified from pre- and post-washing images and from a high-accuracy liquid particle counter analysis of cast run-out. Results: Study 1 demonstrated that the LoS method provided the best intranasal saline deposition. Studies 2 and 3 showed that intranasal saline administration with this method effectively recovered and removed the PM-simulating dust from the mucus-coated cast. Conclusion: These results support the benefit of nasal saline washing with LoS administration, suggesting that this method should be recommended for nasal spray use to effectively remove PM. Future investigations are warranted to explore the benefits of nasal washing in a variety of clinical settings. The symptoms from most acquired conditions, however, are the number one complaint of patients aged 65 years and older.2 This is further demonstrated by the number one fear of older adults is loss of independence and ability to age in place. This is strongly associated with fear of falling. Given the exponential growth of this older cohort group worldwide, this is a significant and growing segment of the population. This older population is also confronted with numerous medical co-morbidities complicating their management with the increased prevalence of falls. It is estimated that dizziness is the 3rd most common complaint heard in physicians’ offices across all age groups, preceded only by headache and lower back pain. It has been observed by clinicians for over 100 years, that there is a sizable segment of this population of patients with or without a precedent vertigo episode who report difficulty coping and ambulating comfortably within a visually dynamic world. The purpose of this article is to provide the reader with an understanding of these conditions. This will include definitions of the conditions and a review of their symptoms and management options. Method: This is a comparative study of 30 mothers of children (age between 8-12) using cochlear implant (G-1) and 30 mothers of children (age between 8-12) using hearing aids (G-2) was included for the study. A questionnaire was developed based on ICF-CY version, consisting of 46 questions distributed across 9 domains. The questionnaire form in the Hindi language was provided to the participants (mothers) with enough amount of time to complete it. For mothers who were not able to self-administer the tool, the researcher helped by interviewing them. Due to COVID- 19 pandemic and ensuing lockdown most parents (mothers) were unwilling to report to the clinic. Owing to this, to ensure safety of parents and children, data was obtained through virtual mode. Results: Across all domains, the comparison between Group A and Group B indicates no statistically significant difference except for two domains (general tasks and behaviours, communication). When the total third-party disability scores were analysed, it is seen that there is significant difference in third-party disability scores of the two groups. Conclusion: Findings enhances mothers of children with hearing aids report greater third-party disability as compared to mothers of children using unilateral cochlear implant. Rarely, it could affect the nasal septum, causing nasal septal perforation. Methods: We reviewed the case of a 52-year-old male patient known to have PG with ongoing nasal septal perforation and reviewed his blood tests, computed tomography scan findings and histologic results. Results: A diagnosis of nasal septal perforation due to PG was confirmed after exclusion of other common aetiologies. This was further supported by the presence of extensive ulceration of the nasal squamous mucosa with inflamed granulation tissue and abscess-like areas. These findings were consistent with the diagnosis of PG based on the histology report. We aimed to sensitize head and neck surgeons to this complication. Observation: a 61-years old man was referred to our surgery department for management of T3N0M0 laryngeal cancer after failure of preservative treatment. The patient had received 2 cycles of chemotherapy and was judged a bad responder. A total laryngectomy with bilateral neck dissection was indicated. The port catheter was kept in place for possible further chemotherapy treatment. During neck dissection the catheter fractured and migrated. Post operative CT scan showed the distal part of the catheter bulging in the right cardiac chambers. The patient was transferred to an interventional cardiology unit where the foreign body was successfully retrieved using femoral venous access. Conclusion: port catheters are a real potential danger because of the risk of fracture and migration of the distal part.
