Invasive fungal infections can masquerade as other granulomatous diseases like Tuberculosis, which is highly prevalent in India. We herewith report one such case of a 6-year-old boy with swelling over the right orbit diagnosed as Extra-Pulmonary Tuberculosis of the right orbit and started on ATT (anti-tubercular treatment) for the same. However repeat analysis of the biopsy sample with appropriate staining techniques and high degree of suspicion was diagnosed to have invasive fungal infection, which responded well with use of IV and Oral antifungals. Clinicians should always keep invasive fungal infections as a differential diagnosis with any immunocompetent or immunocompromised child presenting with granulomatous inflammation and proper staining techniques used on the biopsy sample. G","doi":"10.54136/erwej-0203-10023","DOIUrl":"https://doi.org/10.54136/erwej-0203-10023","url":null,"abstract":"Leiomyosarcoma is a rare tumor that accounts for about 2 to 5% of uterine malignancies originating from the smooth muscles of the uterus. Women can be affected at any age group; however, the majority get diagnosed at their perimenopausal age. The early-stage diseases are managed by hysterectomy and complete surgical resection. In the present case report, we discuss a 41-year-old female with a tumor mass of approximately 32-week gestation at the time of examination. The patient had a regular menstrual cycle with no abnormal uterine bleeding except for abdominal pain for three months. A large, lobulated mass (25 x 18 x14 cm) with solid, cystic, and hemorrhagic components was removed surgically (total abdominal hysterectomy) from the uterus's fundus and spalingo-oophorectomy. The patient developed severe hypotension and tachycardia post-surgery and required a longer recovery time. We referred the patient to the medical oncologist for further treatment. In our patient, the tumor rapidly increased, and metastasis was noted in the peritoneal fluid. A further follow-up will be needed to understand the course of the disease and its outcome. Kidney dysfunction is reported in many patients. We report a finding of an uncommon form of urine crystals in a RT-PCR confirmed case of COVID-19 patient. RT-PCR confirmed case of COVID-19 patient, no clinical history of renal calculi reported until day 7 of the treatment. Treatment mainly comprised of favipiravir, vitamin C and zinc. On seventh day of treatment the patient became asymptomatic but the crystals were observed in the urine. The loading dose of favipiravir in COVID-19 positive patient considered as the reason for the urine crystals. In view of nephrotoxic nature of the disease as well as the drugs used in its therapy, it is important to look for subtle signs as well as inexpensive tests which can give vital information, to pick these complications early. Although this observation was made on a single patient, contributions from multiple centres can help understand this further, with keen observation on urine microscopy of COVID-19 patients. On evaluation her vitals were stable, pedal edema was present, cardiovascular, respiratory, abdominal system revealed no significant abnormality. Neurological examination revealed absent ankle reflexes bilaterally. Plantars were flexor bilaterally, altered sensation and decreased proprioception and joint sensation bilateral lower limbs. She was evaluated for above mentioned complaints and examination findings. Nerve conduction study revealed severe sensory motor polyneuropathy predominantly involving bilateral lower limbs. MRI Brain and whole spine screening were within normal limits. Autoimmune workup was positive for antinuclear antibodies, anti-dsDNA, anti-SmD1, anti-histone, anti RibPO,c-ANCA. Bilateral lower limb arteriovenous color doppler was suggestive of subcutaneous edema over ankle and foot with no DVT. According to SLICC (systemic lupus international collaborating clinic criteria) was diagnosed with SLE. {"title":"Efficacy of Combination of Iron Pyrophosphate and Folic Acid in Management of Iron Deficiency- A Retrospective Study","authors":"S. If not treated, it leads to weakness, fatigue, menorrhagia, amenorrhea in females and can cause serious consequences such as heart problems and complications in pregnant ladies such as premature births and low birth weight babies. Thus, it is necessary to prescribe iron supplementation in iron-deficient women to avoid complications in the later stage. Objective: The objective of this study was to evaluate the efficacy of the combination of ferric pyrophosphate and folic acid in reproductive-aged women who suffered from iron deficiency. Methods: Electronic medical records of 100 reproductive-aged women deficient in iron were analyzed. The baseline hemoglobin and iron parameters were noted. They were prescribed a 30 mg tablet combination of ferric pyrophosphate and folic acid, and the parameters were noted at 4 weeks, 8 weeks, and 12 weeks. The data was statistically analyzed by SPSS version 26.0. Results: The hemoglobin (hemoglobin, total erythrocytes, mean corpuscular volume, red cell distribution width, haematocrit) and iron parameters (serum iron, ferritin, transferrin saturation, serum transferrin) showed significant improvement in week 4, which continued till week 12. The RDW levels showed much improvement after week 4 till week 12. As the acute febrile illnesses are spreading, there is an increase in atypical cardiac manifestations ranging from asymptomatic bradycardia to life threatening manifestations like myocarditis and pericardial effusion. Objective: To determine cardiac manifestations of AFI amongst intensive care unit patients in rural populations. Methods: Twenty patients of AFI were studied between January 2021 to June 2021. Echocardiography (ECG), and cardiac enzymes were evaluated. Patients were from either gender from 15 to 60 years and were willing to sign consent. patients admitted with fever and on examination were found abnormal cardiac functions like hypotension, arrhythmia, hypertension, cardiac enzyme changes and reduced ejection fraction on 2D Echo. Patients with preexisting heart disease, cardioactive drugs affecting the heart rate were not involved in this study. Results: In total, 20 patients having cardiac manifestations with AFI were studied, out of which 9 cases presented with bradycardia, 3 cases presented with asymptomatic pericardial effusion, and 8 cases presented with ECG abnormalities such as narrow QRS complex, ST, and T changes. Conclusion: Cardiac Manifestations in adults with AFI were common, ranging from elevated cardiac biomarkers to myocarditis.
