In this report, an unusual presentation of IPH is described. 11-year-old boy presented with clinical features suggestive of hemolytic anemia. However, on detailed evaluation, he was diagnosed to have IPH and confirmed by the presence of hemosiderinladen macrophages in bronchoalveolar lavage fluid (BALF). PSP often goes under diagnosed, hence making it essential for physicians to be well-known of this disorder. We present a case of PSP of a 65- year-old female, which came primarily with reiterative falls due to postural instability and speech disturbances. Objective: To evaluates the effectiveness of luliconazole in real-world settings. Methods: This is a retrospective, EMR based study conducted on 145 patients with tinea infections treated with luliconazole 1% cream for four weeks, and the effectiveness was assessed at the end treatment. Results: Of 145 patients, 115 (79.31%) had Tinea cruris, and the treatment outcome for 138 (95.17%) was recorded as recovered significantly (p-value <0.05). Fifty-one (96.23%) females and 87 (94.57%) males recovered from the infections during treatment period. A 100% recovery rate observed among patients aged 46-60 years. Of 88 patients below 30 years, 82 (93.18%) recovered, while 44 (97.78%) patients recovered in the age group of 31-45 years. In both the venous and arterial compartments, variations in pressure and flow will necessarily affect each other. Flow and pressure within and between the arterial and venous compartments are influenced by hemodynamic on a beat-to-beat basis. Hemodynamic values that are treated in one compartment are dependent on the state of the other compartments. There are several curved proximal isovelocity surface areas as the streamlines of flow approach the stenotic orifice. A stenotic valve's spatial flow velocity profile depends on its anatomy, inlet geometry, and degree of flow acceleration. A patient with Mitral Stenosis (MS) experiences a pressure gradient from the left atrium to the left ventricle that propels the flow abruptly across the stenotic orifice. An arc-shaped flow profile is created, and the atrioventricular valve will therefore have a hemi elliptical velocity profile. At the level of narrowed stenotic orifices, high-velocity jets form in laminar flow when it reaches the vena contracta, the narrowest cross-sectional area downstream from the anatomic orifice, the flow profile in cross-section is relatively flat at the jet's origin and remains flat until the jet reaches the anatomic orifice. Distally from the stenotic region, the blood flow becomes disorganized with multiple velocities and directions. Stenotic mitral valves exhibit thickened leaflets with a reduced excursion. Methods: A retrospective study was carried out on 153 adult patients with melasma. The patients were divided into two treatment groups. Group A (n=102) received topical silymarin, petrowhite and soy extract (Isoflavones) and group B (n=51) received a combination cream of arbutin, octylmethoxycinnamate, and vitamin E. The treatment duration varied from 30 to 180 days depending on the response and the response was evaluated by a change in MASI score from baseline. Treatment-related side effects were monitored. Results: The mean MASI score was significantly reduced after treatment with silymarin, petrowhite and soy extract (Isoflavones) (10.73 ± 8.00 vs 4.74 ± 5.53, p<0.001). Similarly, in the group B, the mean MASI score was significantly reduced after treatment (9.65 ±6.09 Vs. 4.36 ±4.01, p<0.001). It was also observed that patients with side effects had higher mean MASI score in both treatment groups (Group A 9.30 ± 7.69 vs. 3.76± 4.43, p<0.001) (Group B 8.40 ± 4.64 vs. 3.50 ± 3.33, p=0.003) when compared with patients without side effects. Conclusion: Though both treatment modalities seem to be effective in treating melasma, silymarin, petrowhite and soy extract (Isoflavones) seemed to be moderately superior compared to combination cream (considering the differences in the mean values before and after treatment). This study focuses to determine the efficacy of bilastine in combination with other antihistamines or corticosteroids in the treatment and symptomatic relief from Urticaria in Indian patients. Objective: A retrospective study to evaluate the efficacy of bilastine in the management of Urticaria in an Indian population. Methods: A retrospective study was carried out on 99 patients (7 children and 92 adults) with urticaria. Bilastine, 20 mg once daily, was given in patients with other antihistamines such as levocetirizine, fexofenadine, and corticosteroids. The treatment outcome was assessed as recovered, unchanged, worsened, change in therapy, or lost to follow-up. Results: Bilastine was prescribed at 20 mg dose orally either once daily (91 patients) or twice daily (2 patients). About six patients were given bilastine in syrup formulation at a dose of 5 mL (2.5 mg/mL), OD. Out of the 99 patients, most (77/99- 77.78%) reported recovery during the follow-up visit. Seven patients were still on treatment, six patients still had rashes on exposure to the sun, one patient had rashes while swimming, three patients had no recovery, and four were lost to follow-up. Methodology: A retrospective study was carried out on 3,458 T2DM patients reportedly with CAD, G1 (n=33), and without CAD, G2 (n=3125), who had given the ABI test. ABI was defined as normal (1.0 - 1.4) and abnormal (<1.0 and >1.4). The logistic regression analysis was used to identify the determinants of CAD positive outcomes in T2DM patients. Results: Overall, patient percentage-wise CAD 10% (333), abnormal ABI 26% (913), neuropathy 37% (1278), nephropathy, 8% (293), and retinopathy <1% (24). The ABI abnormality and CAD positives were higher in patients with nephropathy (32 and 20%) than in neuropathy (28 and 13%). Age-wise, 52 and 62% of neuropathy and nephropathy patients, respectively, were >60 years and had shown increased ABI abnormality from age 50 years onwards. The logistic model had shown nephropathy patients, [OR-1.8 [1.26-2.58], - p-0.001] in association with abnormal ABI, [OR-1.27 [95% CI, 0.99–1.63], p- 0.057] predicted to have CAD positive outcomes. While neuropathy, OR-0.69, [0.54 - 0.90], and hypertension, OR-0.43 [0.33 - 0.57] were not contributing towards CAD in our study population. Conclusion: Type 2 diabetes patients with abnormal ABI were predicted to have a 27% increased odds ratio of CAD outcome and 80% in the presence of nephropathy. It is considered one of the most common movement disorders with various treatment options reported in clinical practice but uncertainty about the most robust one. The International Parkinson and Movement Disorder Society recently reviewed the clinical studies of essential tremors and updated the treatment guidelines. Mild to moderate tremor severity can sometimes be managed with occupational treatment such as speech therapy or adaptation. In contrast, the first-line pharmacological treatments include symptomatic treatment with propranolol, primidone, and topiramate. Botulinum toxin is preferred for selected cases, whereas invasive treatments should be considered a severe essential tremor treatment option. Focused ultrasound thalamotomy is attracting a lot of attention as a new therapy for essential tremors. Misdiagnosis among tremor syndromes is another point of concern and impacts both clinical care and research. A new neurophysiological measure, known as the tremor stability index, is introduced recently to attain diagnostic accuracy between Parkinson's disease tremor and essential tremor. Also, ET-Plus is a newly suggested term for the classification of tremor in the recent Consensus statement, which acknowledges the presence of additional neurological signs in patients with ET. Doctors take several considerations into account while prescribing antiepileptic drugs (AEDs) to young women to balance the risk of seizures with other concerns. Objective: To understand how doctors perceive various AEDs regarding their safety, efficacy, and other attributes for women of childbearing age. Materials and Methods: This study consisted of a survey designed to understand the usage and perception of 12 antiepileptic drugs in the Indian market. The survey respondents of this study were neurologists, neurosurgeons, and psychiatrists from all over India, from private or government setups, and with different experience and patient load levels. Results: In the study, 93% of all survey respondents indicated that they avoid one or more AEDs in women of childbearing age, making it a significant consideration. This consideration is a specialty split for the second- generation AEDs, Oxcarbazepine, and Levetiracetam. Approximately three times more Neurosurgeons avoid Oxcarbazepine as compared to neurologists and psychiatrists who use this drug. In the case of Levetiracetam, mainly only psychiatrists avoid it for women of reproductive age. Apart from the personal experience of doctors, factors other than teratogenic risks, such as changes in reproductive hormone levels, may have contributed to the observed difference in perception. The circulating thyroid hormones have a significant effect on the metabolism of glucose, lipid, and protein. In the case of T2DM, glycaemic control can worsen due to this effect. In T2DM patients, hyperglycemia can occur due to worsening diabetes mellitus because of hyperthyroidism and thyrotoxicosis and can increase the risk of complications of diabetes. T2DM also affects the levels of thyroid stimulating hormone by reducing it. It also impairs triiodothyronine (T3) conversion from thyroxine (T4) in the peripheral tissues. If T2DM is poorly managed, it can lead to insulin resistance and hyperinsulinemia. It is important to carry out more studies to clarify the relationship between thyroid disorders and T2DM. Objective: To understand the interdependent relationship between TD, T2DM, and hypertension. Material and methods: Retrospective, single-center, observational study conducted on anonymized patient data captured from electronic medical records (EMR). The demographic and clinical profile of T2DM patients were noted along with the thyroid test lab values available for the diabetic patients in the records. Results: A high prevalence of thyroid dysfunction was found in T2DM diabetics than found in the normal population. In the present study, hypothyroidism was found to be more in T2DM patients as compared to hypothyroidism. Conclusion: An increased prevalence of TD is being seen in T2DM patients and can worsen each other's function.
