But that belief that one has to undergo pain during labor is slowly fading away, as we are entering an era where women are seeking options for painless labor, and delivery. Objective: To evaluate the spine of women in labor opting for epidural analgesia. To correlate the abnormal position of the maternal spine to the progress of labor and outcomes. Methods: The current prospective observational study performed between 2021-2023 had enrolled 200 women who were given epidural analgesia irrespective of stage of labor. Women who had a history of spine surgeries or congenital spinal deformities were excluded. Results: Of the 200 women included in the study, 80 women showed a deviation of spine mostly to the right from the midline or significant lordosis, irrespective of babies’ weight and maternal height. Out of the 80 women, 70 women ended up in an emergency cesarean section. Of the 70 women who were taken up for emergency c-sections, the major indication was nonprogress of the second stage of labor. Ten patients had a vaginal delivery, of which 3 had assisted instrumental delivery due to a prolonged second stage and poor maternal efforts. In the present case a 35-year-old female presented with abdominal fullness and pain in the upper abdomen for the past two months which was not associated with the consumption of water or food, nausea, or vomiting. When we investigated it was seen that she had severe anemia with pancytopenia with no malarial parasite on peripheral smear, liver renal and liver function tests were within the normal range. The diagnosis of Banti’s syndrome was made. However, the patient was then vitally stable and was started on propranolol and was advised for splenectomy due to refractory anemia. Splenic tuberculosis is typically a component of miliary TB, ranks third after lung and liver, and is more prevalent in people with weakened immune systems. An extremely uncommon variant of abdominal tuberculosis is splenic TB in immune-competent people with no extra splenic involvement. In individuals without pulmonary disease and without symptoms, splenic TB diagnosis is challenging. There are frequently no diagnostic standards for isolated TB. A definitive diagnosis is rarely reached by preoperative testing. When other potential causes of fever and splenomegaly have been ruled out in endemic areas, splenic TB should be suspected. The typical treatment for splenic TB without abscess is medical management (ATT). For splenectomy, a single TB splenic abscess in a healthy patient is recommended. Here we report the case of a 33-year-old patient, who complained of left-sided stomach pain that had been bothering him for four years but was unrelated to radiation or food consumption. It has been linked to several high-grade fever bouts and frequent hospitalizations for the same during the past three years. This case report outlines a case of melioidotic osteomyelitis presenting as a suspected sarcoma. The diagnosis of melioidotic osteomyelitis was done against the diagnosis of sarcoma only after radiological evaluation, microbiological evidence of Burkholeria pseudomallei and osteomyelitis confirmed on histology. It is an extremely rare mimic of primary sarcoma of bone and the presentation described in the present case report appears to be unique.
