{"title":"Prognostic Value of Coronary Artery Calcium in the Intermediate-Risk Population of Developing Coronary Artery Disease","authors":"K. Kagita, Madhuri Posina","doi":"10.54136/erwej-0304-10063","DOIUrl":"https://doi.org/10.54136/erwej-0304-10063","url":null,"abstract":"Background: The present study was designed to estimate the coronary artery calcium score (CACS) and its association with the incidence of major adverse cardiovascular events (MACE) in asymptomatic patients who are at risk of coronary artery disease (CAD). Objective: To estimate the CACS and its association with the incidence of MACE in asymptomatic patients. Methods: In this prospective cross-sectional observational study, 108 consecutive patients were enrolled. Demographic details and clinical data including lipid profile, systolic blood pressure, electrocardiography, 2D echocardiography, and routine blood investigations were reported. CACS was derived from computed tomography using a 256-slice scanner with a rotation time of 270 milliseconds. MACE was recorded at a one-year follow-up. Results: The mean age was 54.55 ± 7.7 years with male predominance (62%). CACS categories 0, 1-99, 10-399, 400-999, and more than 1000 constituted 43.5%, 28.7%, 18.52%, 8.33%, and 0.93% respectively. The correlation between the groups of positive and negative CACS and the presence or absence of standard risk factors was found to be statistically significant in diabetes mellitus (P=<0.001), hypertension (P=<0.001), and history of CAD in the family (P = 0.036). Although the association between smokers and calcium was statistically insignificant, it had clinical significance (P= 0.294). Out of 108 patients, MACE was observed in 16 (14.81%) patients with positive CACS at the 1-year follow-up. Conclusion:CACS measurement is often regarded as the primary non-invasive approach for risk stratification, MACE estimation, and promptly identifying high-risk asymptomatic individuals. Cardiac myxomas frequently originate from the left atrium and predominantly affect women. We present a case of a 65-year-old male who was admitted for hernia surgery and underwent preoperative evaluation. On evaluation, he was found to have a diastolic murmur on auscultation and had a history of exercise intolerance, and a transthoracic echocardiogram was done. It revealed a large mass in the left atrium attached to the interatrial septum suggestive of left atrial myxoma. He then underwent surgical resection of the tumor. It is caused by the presence of the endometrial tissue exterior of the uterus. A 45-year-old woman presented with recurrent episodes of dysuria. Computed tomography (CT) scan showed ill-defined mildly enhancing soft tissue density within the vesicouterine region showing loss of fat planes with the posterior wall of the urinary bladder in the region of the trigone s/o bladder endometriosis. The patient was planned for cystoscopy and cystoscopy resection of bladder endometriosis. Endometrioma was resected using a resectoscope. Bilateral ureteric reflexes were elicited, and the rest of the bladder mucosa was normal. Patients with a perineal hernia may be asymptomatic or complain of urinary or bowel disturbance or pain. Our patient was a 63-year-old symptomatic female with a history of vaginal hysterectomy and laparoscopic sacrospinous fixation. She presented with heaviness in the lower abdomen and increased stooling frequency. She was found to have a perineal hernia up to the right ischioanal fossa, with grade four vaginal vault prolapse. Laparoscopic sacrocolpopexy with levator ani plication with perineal hernia repair was performed. We used a T-shaped mesh composed of monofilament polypropylene with a combination of synthetic sutures to plicate the levator ani bilaterally and repair the vault prolapse. The postoperative course has been favorable. As of 12 months postoperative, there are no fresh complaints. The use of mesh (e.g., polypropylene) is important for levator ani repair for reinforcement. Laparoscopic repair is generally preferred, as it provides better access with a lesser risk of complication. Almost all cases of perineal hernia in literature have been after colorectal surgery and with no specific protocol for management. Its burden in resource-poor countries prompts the development of investigative modalities that offers accuracy and affordability. Objective: To determine that serum CK-MB and LDH estimation could be markers of Asphyxia neonatorum, and the elevated levels directly correlate with the grade of severity of Hypoxic Ischemic Encephalopathy (HIE) as defined by Levine. Methods: In this single-center, non-interventional study, we included 100 asphyxiated neonates from level III NICU in the case group, and 100 apparently healthy neonates from the post-natal ward admitted over 18 months at RCSM Government Medical College and CPR Hospital, Kolhapur. 100 healthy neonates were the control group. Results: The median 8-hr serum CK-MB in the case group was 74 U/L and in the control group as 63.5 U/L with P<0.001. Whereas the median serum LDH level in the case group was 597 U/L and the control group was 383.5 U/L with P<0.001. Raised LDH (cut off 580 U/L) had 100% specificity, while CK-MB (cut off 92.6 U/L) had 100% specificity for asphyxia. Amongst the neonates with HIE (n=60), 56% had raised CK-MB (cut off 92.6) and 80% had raised LDH (cut off 580 U/L). 100% of neonates with HIE Grade III (n=9) had elevated CK-MB and LDH levels. Myomectomy is one of the most common gynecological procedures and can be performed as vaginal, abdominal, and laparoscopic procedures. In the present case, a patient who was elderly primi at 33 weeks came with a complaint of diffuse pain abdomen in an emergency. She had a history of infertility 2 years back. During the surgery, uterus repair was done, and 4 units of packed cells and 2 units of fresh frozen plasma (FFP) were transfused. She was discharged in stable condition. Macrostomia is a form of facial cleft and can be associated with other congenital abnormalities or syndromes. In the present case, a 1.8 kg, late preterm male baby was delivered by emergency cesarean section. A unilateral macrostomia with a cleft palate was present. An abnormal right ear with a deformed ear lobule was present and the left ear was normal. In this case, surgery is the treatment of choice.
