Macrostomia is a form of facial cleft and can be associated with other congenital abnormalities or syndromes. In the present case, a 1.8 kg, late preterm male baby was delivered by emergency cesarean section. A unilateral macrostomia with a cleft palate was present. An abnormal right ear with a deformed ear lobule was present and the left ear was normal. In this case, surgery is the treatment of choice.
