Along with anteroposterior mal-relation, there is often the rotation of upper first molars in Class II cases. Rotated molars not only occupy more space than normally positioned molars but also increase the challenge for orthodontic treatment.\u0000Aims and objectives: This study aimed to determine the prevalence and extent of maxillary molar rotation in Class II division 1 and 2 malocclusion cases.\u0000Material and Methods: A total of 142 pretreatment maxillary casts of Class II malocclusion (71 division 1 and 71 division 2) cases were collected from the Department of Orthodontics of People's Dental College and Hospital. All the casts were scanned at a 1 200 dpi resolution with teeth touching the glass of the scanner (brother MFC-L2700DW model). Scanned pictures were then printed without any change in magnification. Measurement was made on those printed copies of casts. To determine the rotation and its severity a method developed by Ricketts (Line of Ricketts) was used. Descriptive analysis was carried out in Microsoft Excel 2013.\u0000Result: In Class II division 1 cases, 45 (63.4%) had right molar rotation and 47 (66.2%) had left molar rotation. In Class II division 2 cases, 33 (46.5%) had right molar rotation while 34 (47.9%) had left molar rotation. Similarly, the mean rotations of the right and left molars in division 1 were 6.24 mm and 6.77 mm respectively and those in division 2 were 4.82 mm and 4.94 mm respectively.\u0000Conclusion: The prevalence and extent of rotation of maxillary molars in Class II divisions 1 and 2 malocclusion cases were assessed in this study. Mesiopalatal rotation and left molar rotation were observed more commonly in both divisions.

Several studies evaluated the safe zone for mini-implants placement, but only a very few previous studies have taken different skeletal patterns into account when assessing measurements.\u0000Objective: The purpose of this cross-sectional, comparative study was to compare the inter-radicular distance and buccal cortical bone thickness in Class I and Class II skeletal malocclusion patterns.\u0000Materials and Methods: A total of 62 CBCT images of patients with Class I and Class II skeletal malocclusion were obtained from the records of the department of Oral medicine and Radiology, Kathmandu University Teaching Hospital. The inter-radicular distance and buccal cortical bone thickness were measured at four different heights (2, 4, 6 and 8 mm) from the CEJ towards the apex. These measurements were measured between different skeletal pattern and gender with independent t-test. The intergroup comparison at different height from CEJ was done with ANOVAfollowed by Tukey's post-hoc test to see the difference within the category.\u0000Result: There was a statistically significant difference observed in the inter-radicular distance between the maxillary first and second premolars at a height of 6 mm between Class I and Class II malocclusion patterns (p = 0.03). There were differences observed in the inter-radicular distance of the mandible at a different height based on skeletal malocclusion pattern, which was not statistically significant (p > 0.05). The buccal cortical bone thickness between the maxillary central and lateral incisors at the height of 2 mm from CEJ between Class I and Class II skeletal malocclusion patterns was statistically significant (p = 0.01). The buccal cortical bone thickness of the mandible at different heights based on skeletal malocclusion pattern there were differences observed which were not statistically significant (p > 0.05).\u0000Conclusion: The inter-radicular distance and buccal cortical bone thickness could be influenced by different skeletal patterns and tend to increase from the CEJ to the apex in both Class I and Class II skeletal patterns.

The aim of this study was to describe various skeletal, dentoalveolar, and soft tissue components of skeletal Class II malocclusion in Nepalese orthodontic patients.\u0000Materials and Method: This cross-sectional study was carried out at the Universal College of Medical Sciences (UCMS) in the Department of Orthodontics and Dentofacial Orthopedics. Lateral cephalograms of 70 patients with skeletal class II malocclusion of age greater than 1 8 years, and ANB > 4°, were traced. Various skeletal, dentoalveolar, and softtissue measurements were taken. Descriptive statistics were performed followed by an independent sample t-test to find the difference in mean of various cephalometric parameters between males and females.\u0000Result: Among 70 subjects, 54 samples had class II div 1 malocclusion and 16 had class II div 2 malocclusion. Mandibular retrognathism was seen in 37 subjects having skeletal Class II Div 1 malocclusion and in 9 subjects with Class II Div 2 malocclusion. The mean value of SNA was 82.91 ° ± 2.87°, SNB was 76.81 ° ± 2.97°, and ANB was 6.11 ° ± 1.22° in Class II Div 1 subjects. In subjects with Class II Div 2, the mean value of SNA was 82.31 ° ± 3.19°, SNB was 76° ± 4.08°, and ANB was 5.44° ± 1.21 °. Mean value of U1 to NA degree and linear measurement was Class II div 1 was 28.30° ± 6.86° degree and 5.56 ± 2.76 mm respectively. Similarly, mean value of LI to NA degree and linear measurement was 31 .93° ± 5.96° and 6.16 ± 2.37 mm respectively. Mean and standard deviation value of upper lip to S-line and lower lip to S-line was 2.85 ± 2.1 3 mm and 3.54 ± 2.47 mm respectively in Class II Div 1 subjects. Similarly, in Class II Div 2 it was 1.50 ± 2.16 mm and 0.13 ± 2.66 mm respectively. Mean and standard deviation value of nasolabial angle was 93.69° ± 12.91° in Class II div 1 and 96.38° ± 12.26° degree in Class II Div 2. The mean values of most cephalometric parameters did not differ statistically between male and female subjects with Class II div 1 malocclusion.\u0000Conclusion: In subjects with skeletal Class II mean value of SNA was within normal range whereas SNB was decreased. Retrognathia and a small mandible in relation to the cranial base with a well-positioned maxilla were observed in the majority of subjects. Mean U1 to NA angular and linear measurement was increased in Class II Div 1, whereas they were decreased in Class II Div 2.

The scales used for outcome assessment are usually the same for both and can be classified as generic or condition-specific. Generic scales are used in general for a variety of clinical conditions in dentistry, whereas condition-specific scales are used for specific problems like malocclusion. Due to the peculiar nature of orthodontic problems that do not fall into the definition of a disease, generic scales are of little use and condition-specific ones are recommended. The present article aims to provide a summary of the available condition-specific patient-reported outcome measures in orthodontics and perform a critical analysis of the same.

The present study aimed to evaluate the smile attractiveness in subjects treated with extraction or non-extraction orthodontic treatment modality.\u0000Materials and Methods: Frontal smiling photographs of 100 orthodontically treated subjects with an age range of 16-25 years were taken and divided into two groups each having 50 subjects. Group I (ANB 0° to 4° ) was subdivided into cases treated with extraction (group la) and without extraction (group lb). Similarly, Group II (ANB > 4°) was subdivided into cases treated with extraction (group lla) and without extraction (group lib). Smile esthetics and esthetic scores were assessed on frontal smile photographs by using different smile variables objectively and with the help of raters (10 laypersons,10 dental graduates, and 10 orthodontists) subjectively.\u0000Results: In extraction cases, the esthetic scores of dental practitioners and orthodontists were higher than in cases of non-extraction. Between extraction and non-extraction cases, laypeople had no esthetic preference. In Skeletal class I, smile variables such as the arch form index, smile arc and maxillary incisor show had a higher value in extraction cases however, the buccal corridor was more in non-extraction cases. In Skeletal Class II, smile variables such as the maxillary incisor show and arch form index were more in extraction cases and exposure of mandibular teeth was more in non-extraction cases. Judgement given by the raters was mainly based on maxillary incisor show, number of teeth displayed, and buccal corridor ratio. In orthodontic patients treated with and without the extraction, statistically non-significant variations in smile esthetics and esthetic scores were found.\u0000Conclusion: Thus, a decision regarding the extraction of teeth in orthodontic patients should not be solely based on smile esthetics but other factors which determine extraction should be considered.

There are number of clinical dilemma where both the specialties need robust guideline for clinical decision making. The effect of orthodontic force is expressed in different way in different patients and teeth which need to be understood by clinicians. On the contrary, the tooth movement of endodontically involved teeth demands careful evaluation of the tooth and its surrounding tissue, quality of root treatment, presence or absence of peri-apical pathologies, previous history of trauma etc. Management of external apical root resorption following orthodontic therapy might be challenging some time. Orthodontic extrusion of fractured teeth is one of the viable prosthetic rehabilitation options where orthodontic, endodontic and prosthodontics specialties are involved. So this review article will focus on reviewing current level of evidence on orthodontic- endodontic interface.

Lingual orthodontic appliances are preferred over labial by patients because of its invisibility. The aim of this research is to compare the Von Mises stress distribution and displacement of palatal implants in the lingual orthodontic system among four different combinations of palatal implant and lever arm.\u0000Materials and Method: Four Finite element models were constructed for the bilaterally extracted first premolar maxillary arch. In all these models 0.018” slot lingual brackets were placed at the center of the clinical crown. A similar retraction force (150gm) was applied with the help of NiTi closed coil spring for all the models but the length of the lever arm vary as well as the palatal implant position also varies in these models. Finite element analysis was then performed to compare the Von Mises stress distribution, and displacement of the palatal implant using ANSYS 12.1 software.\u0000Result: In this study, displacement was the same (0.0005 mm) for all four models. Highest amount von mises stress was observed in Model 3 (3.4798 MPa) as a comparison to Model 1 (2.5442 MPa), Model 2 (2.5018 MPa), and Model 4 (3.3854 MPa). The stress value for the palatal implant was within the acceptable fatigue limit of the titanium of 193 MPa therefore all four models combination was safe for the clinical application.\u0000Conclusion: Double palatal implant systems were more effective in comparison to the single palatal implant system in lingual orthodontics. In this study, we found that the displacement of the palatal implants was not affected by the length of the lever-arm and the amount of stress was decreased when we increase the length of the lever-arm.
