Attia, Mohamed El-Layeh","doi":"10.21608/mjd.2021.200347","DOIUrl":"https://doi.org/10.21608/mjd.2021.200347","url":null,"abstract":"Objectives: the purpose of this in-vitro study was to investigate the effect of different surface treatment and various etching times on the shear bond strength of machinable glass ceramic using 2 resin cements. Materials and Methods: A total of 64 ceramic samples (10x12x3.5 mm) were fabricated from (IPS e.max CAD) using CAD/CAM technology. Ceramic samples were divided into two main groups (n=32) according to surface treatment: Hydrofluoric acid followed by Monobond N application or Monobond Etch and prime. According to the etching time, either 60 or 120 sec each main group was divided into two subgroups (n=16). Composite resin discs were fabricated (4mm diameter x 3mm thickness) dimensions. Each subgroup was further subdivided into two divisions (n=8) according to type of luting agent used, either adhesive resin cement (Multilink N) or self-adhesive resin cement (G-CEM LinkAce). Bonded specimens were stored in water bath at 37°C for 5 months then exposed to 10,000 thermal cycles. Universal testing machine was used to record the shear bond strength values for each specimen. Failure analysis was performed using a stereomicroscope (10x) and a scanning electron microscope (SEM). Results: SBS values ranged between 11.9 and 15.1 MPa in groups treated with HF pluse Monobond N and between 13.9 and 15.8 MPa in groups treated with MEP. ANOVAs test showed statistically significant difference between the two etching agents used for surface treatment as well as luting agent used for the bonding. However, etching time factor had no statistically significant difference. Moreover, the interaction between the factors had no statistically significant difference P (≤0.05). Most of specimens exhibited cohesive and mixed failure mode. Conclusions: MEP showed comparable SBS results to the combination of HF and Monobond N after artificial aging. Etching time had no effect on the SBS results. Subjects and methods: Thirteen patients (9 females, 4 males; mean age 16.55±2.54 years) who required both maxillary first premolar extraction and bilateral maxillary canine distalization were selected in this study. Each Patient was randomly assigned to either the experimental side that used a mechanical vibration device (a modified Gillette Fusion ProGlide ) or the control side that did not receive a vibration. Miniscrews were used in each side to support anchorage and closed-coil nickel-titanium springs with 150 g of force were used to retract the canines. Vibration of 133 Hz, was applied on the buccal surface of maxillary canine 10 mins twice daily for 12 week at the vibration side. The outcome was the rate of canine retraction measured using 3D digital models superimposed from the baseline (T0) to 4 weeks, 8 weeks and 12week (T1, T2 and T3). All date were analyzed and evaluated by Student t-test. Results: Eleven subjects remained in the trial at start of canine retraction. The mean rate of tooth movement (TM) was 0.956±0.10 mm/month for control sides versus 0.912±0.12mm/month for experimental side, which was not statistically significant difference at any of the timepoints. Regarding to (TM) mean values of control and vibration sides at T1 was (0.59±0.13 mm Vs 0.55±0.14 mm), at T2 (0.92±0.19 mm Vs 0.98±0.23 mm) and at T3 (1.32±0.16 mm versus 1.20±0.18 mm) respectively. Medical and dental history were taken from the patients. Evaluation of clinical parameters was performed 1. Plaque index. 2. Clinical attachment level. 3 pocket probing depth 4 Bleeding index. The microbiological evaluation was performed by obtaining samples of gingival crevicular fluid. All patients were treated with full scaling and root planing. Then the patients were divided into the two groups: Group 1): consisted of fifteen patients. Application of Photodynamic therapy using diode laser combined with indocyanine photosensitizer. Group 2): consisted of fifteen patients, treated by scaling and root planning. Clinical and microbiological evaluation was performed at the baseline and after six weeks Results: All clinical parameters (PI, SBI, PPD and CAL), total bacterial count in addition to P.gingivalis & P.intermedia counts were revealed statistically significant difference after six weeks of treatment among both groups. However on comparing post-treatment values of P.gingivalis & P.intermedia counts in between groups demonstrated statistically significant difference between groups 1&2. Methods: two hundred mandibular first molars with mesio-buccal canals having angles of curvature ranging from 20° to 40° were divided into four equal groups each of 50 according to the viscosity of lubricant: group I (Castor oil 100%), group II (Castor oil 90%, group III (Castor oil 80%) and group IV(EDTA 17%). Then each groups divided into five subgroups (n=10). The root canals were prepared using 2Shape rotary instruments which examined before and after root canal preparation under electron microscope at different magnifications (10× and 50×) for any fracture or deformity. Collected data were then statistically compared to determine differences between the tested root canal lubricants. . Results: After using tested lubricant solutions, the Castor oil 80% showed the lowest fracture rate on 2Shape rotary instruments while EDTA 17% showed the highest rate. Castor oil 100% have the highest incidence rate of unwinding in comparison with other groups while castor oil 80% have the lowest rate of unwinding. Treatment quality and integrity of the remaining tooth structure should be conserved cautiously. Although many new restorative materials and techniques have become available in the last years, some basic concepts in restoring ETT remain the same. Aim of the work: The aim of our review is to highlight different designs used for reconstruction of ETT. Methods: we searched The PubMed database for study reporting reconstruction of ETT. Results: We found several designs used for the restoration of ETT. For example, fiber post, endocrown, sharonlay. Each design should be selected according to the status of the remaining tooth structure. The most popular design was fiber post followed by endocrown. Sharonlay is a novel design was introduced in the 2015 reconstruction of ETT however, the available research on it were few, and the restoration was fabricated from metal. Materials and Methods: In this study 20 Caries free human maxillary premolars were used with a standard onlay cavity preparation with anatomical occlusal preparation on the palatal cusp. Preparations were scanned, designed and sent for milling the ceramic onlays using CORiTECH 250i milling machine. Half of onlays were milled from Vita Enamic blocks and the other half from Vita Suprinity blocks. Resin cementation weas done for all the onlays. The onlays were subjected to 5000 cycles of thermocycling. Marginal gaps (μm) were measured using digital microscope before and after thermocycling. Three-way ANOVA was used to study the effect of tested variable and its interaction on marginal gap (μm). Results: Zirconia reinforced lithium silicate ceramic (Vita Suprinity) had higher marginal gap than Polymer infiltrated ceramic (Vita Enamic) before and after thermocycling (40.52μm) (30.29μm) and (71.86μm) (62.10μm) respectively. Also, thermocycling had increased the marginal gaps for all onlays sample. Thus, providing the celiac children with gluten, sodium lauryl sulphate (SLS), fluoride, and parabenfree toothpaste without inducing inflammatory or immune reaction. Such toothpaste is not available in the Egyptian market. A gluten-free toothpaste was formulated and produced for this study. Methodology: Twenty children diagnosed with celiac disease, aged from 6 to 15 years and on a strict gluten-free diet were recruited from the gastroenterology clinic at New Children University Hospital (Abo El Rish Hospital). Each participant was provided with a tooth brush and an experimental toothpaste. The parents were asked to use the glutenfree toothpaste 3 times daily for their children and report every time if there is any inflammation or any reaction associated with using the toothpaste. All children were recalled weekly for follow-up during the first month and at 3 & 6 months after using the toothpaste for assessment. Results: During different follow-up intervals (starting from one week until 6 months), all the cases were free from any ulcers and/or inflammation with a 100% success rate during toothpaste use. Conclusion: The toothpaste did not cause any undesirable side effects during use. Cellulose gum is the common vernacular for carboxymethylcellulose (CMC), which is derived from the cell walls of woody plants, generally trees or cotton (Pelak, 2015). The most frequently used detergent at present in most toothpastes is sodium lauryl sulfate. Sodium lauryl sulphate may be irritant to the skin and mucosa. It may also damage the mucosal mucin layer by denaturing its glycoproteins. The epithelium will then be more exposed for irritants resulting in aphthous ulceration in some patients. It has also been claimed that there is a connection between the use of toothpaste or mouthwash containing SLS and an increased frequency of recurrent aphthous ulcers (RAU) in some patients. A product without SLS may thus be recommended for patients with RAU (Iqbal et al., 2011). RAU is a symptom of celiac disease (Pray, 2011). Materials and methods: A total of 64 lithium disilicateIPS e.max CADceramicspecimens with dimension(12X10X3) were fabricated using milling technology, thespecimens were equally divided into 4 main groups (n=16) according to the method of surface treatment as follows. In Group I: polished, Group II: Acidulated phosphate fluoride (APF) application, Group III: Hydrofluoric acid (HF) application, Group IV:Monobond Etch & Prime (MEP) application. Each main group was further equally subdivided into 2 subgroups (n=8) according to methods of chemical bonding. Subgroup (A):Conventional silane.Subgroup (B):Multipurpose primer.Composite resin cylinder (Master fill) were fabricated and cemented to the treated specimens by multistep adhesive resin cement. All bonded specimens were stored in water bath for 5 months and subjected to 10000 thermal cycles. Shear bond strength (SBS) test was performed afterwards. Scanning Electron Microscope (SEM) was used for specimens examination .Results: The statistical analysis were done using two-way ANOVA and serial one-way ANOVAs followed by Post Hoc Tukey-HSD test at α =(0.05). Application of multipurpose primersignificantly increased SBS (P=0.001). The highest SBS mean value (14.8 ± 2.2 MPa) was obtained for APF followed by Monobond Etch & Prime with conventional silane (14.7 ± 2.8 MPa)On the other hand,APF with conventional silane showed the lowest SBS mean value(4.6 ± 2.0MPa). There was statistically significant difference between polished and APF considering conventional silane and multipurpose primer, and no statistically significant difference was reported between HF and Etch & Prim for conventional silane and multipurpose primer. Materials and methods: Thirty sound human maxillary incisors (n=30) that were extracted due to periodontal diseases and collected from Oral Surgery clinic at Faculty of Dentistry, Mansoura University, according to the regulation of the ethics committee The teeth were randomly divided into three groups (n=10) ; Group A: control group,Group B:CPPACP group and Group C :Tricalcium silicate group with 10 samples in each group(n=10). The specimens of control group were cut into two halves, one of them was used as negative sound control and the other section was subjected to demineralization process. All specimens of other remaining groups were subjected to the demineralization process, then subdivided into two subgroups by cutting the tooth longitudinally through the center of the tooth by carborundum bur. One half was left without any surface treatment, act as positive control group and the other received remineralizing treatment (test group). All specimens were measured by laser induced fluorescence device(Diagnodent pen, KAVO model 2095, Biberach, Germany) and prepared for elemental and morphological analysis by drying to ensure that the teeth moisture free. The tested groups were tested after 3 months,6months and 9months.They stored in artificial saliva that prepared in faculty of pharmacy Mansoura university Results:The results of Kolmogorov–Smirnov and Shapiro Wilk tests revealed that all data following normal distribution. Therefore, a parametric Two-way analysis of variance test (ANOVA) was conducted. The outcomes of ANOVA test revealed that “type of remineralizing agent” and “storage time ”significantly affected laser fluorescence and EDX mean values (p˂0.05) . For laser induced fluorescence results ,at 3 and 9 months there was a significant difference between the two materials but at 6 months ,there was no statistically significant difference. For EDX outcome (calcium and phosphorus),no statistically significant difference between the two remineralizing agents at 3 and 6 months but, there was a significant difference at 9 months. These results showed that thetricalcium silicate showed the highest remineralizing potential , while CPP-ACP showed the least . Conclusion:Under the limitation of this study, the outcomes reveled that tricalcium silicate performed better than CPP-ACP as remineralizing agent. . This in vivo study is designed to provide data regarding retention force and chewing efficiency at insertion, after 3 and 6 months of function with the use of bar clip (group1)&telescopic attachments(group 2) in mandibular overdenture treatment. Six healthy male completely edentulous patients will be selected from the outpatient clinic of Prosthodontic Department, Faculty of dentistry, Mansoura University, Egypt. Each patient will receive three dental implants in the 2 in the canine region of the mandible bilateraaly and one in the midline and two suprastructure modalities (bar and telescopic attachments ). The retention force of the attachments at insertion , after 3 and six months was measured in a standardized way. The amount of retention that was related to the attachment were evaluated. The difference in retention force at insertion , after 3 and 6 months observed for bar-clip attachments. Decrease of bar-clip retention from at insertion to 3& 6 months at bar attachment (from 20.07 to 9.1 and 6.5 , respectively). Slight differences in retention force at insertion, after 3and 6 months were observed for telescopic attachments.(mean retention 8.5).chewing efficiency at insertion , 3 and 6 months was measured in s standardize way. The mean chewing efficiency illustrates statistically significant higher mean among bar attachment than telescopic attachment at insertion , after 3 and 6 months. Chewing efficiency illustrates increased mean by increasing number of cycles. Purpose:The aim of this within patient study was to evaluate the effect of linear distribution of implant supported mandibular over denture using bar and telescopic attachments on the retention and chewing efficiency. Methods:Six male patients ofSix healthy male patients ranged between 48-60 years old were included in the study. For each patientrecieveda conventional complete denture before implant surgery was performed in the midline and two canine areas. After healing period, the conventional denture was converted into implant supported overdentureattached to three implants and checked forretention at insertion and 3 and 6 months later by forcemeter and masticatory efficiency was evaluated at insertion 3 and 6 months later by two-colored chewing gum.Repeated Measures ANOVA test was used to compare more than 2 studied periods with post Hoc test Tukey test. Two & three Way ANOVA tests were used to assess the effect of combination of 2 or 3 independent factors on continuous parametric outcome (retention and chewing efficiency ). Results: Mean retention illustrates statistically significant higher value among Bar attachment than telescopic at insertion (20.07 and 8.5) , while there is statistically significant lower value among bar than telescopic attachment(6.5 and 8.83).
