Ben-Ameh","doi":"10.46912/jbrcp.v3.i1.2020.139","DOIUrl":"https://doi.org/10.46912/jbrcp.v3.i1.2020.139","url":null,"abstract":"Rhino-orbito-cerebral mucormycosis (ROCM) is an uncommon acute aggressive fungal infection occurring in several immunocompromised states including diabetes which is the most common predisposing factor. In this case report we present the clinical finding, therapeutic challenges and outcome in a 44 years old male type II diabetic patient. Patient first presented to the ortorhinolaryngologist with complaints of nasal discharge, postnasal drip, cough, hyposmia, facial pains, and intermittent nasal obstruction. He had bilateral intranasal antrostomy, upon X-ray confirmation of homogenious opacity of the left maxillary antrum and mucosal thickening of the right antrum. Fungal study of the specimen revealed mucormycosis. Six months later he presented with perception of light (PL) vision in the right eye and non-perception of light (NPL) in the left eye. There was mild lid edema in the right eye. There was marked proptosis associated with external ophthalmoplegia suggestive of orbital apex syndrome. The conjunctiva was keratinized and the cornea necrotic and opaque. The left eye enucleation and nasal exploration were done. The patient objected to taking amphotericn B, the drug of choice for this condition because of his previous adverse reaction experience to the drug. Posacornazole, the second line drug could not be assessed, posing a great therapeutic challenge in the effective management of this case. Except where good diagnostic imaging procedure is performed, they could be mistaken for massive abdominal ascites. Here we report 2 cases which were diagnosed at a medical missions facility during a free medical outreach program in March 2017. The patients presented with gross abdominal distention. Ultrasound scan revealed cystic ovarian tumors in both cases with no suspicious finding for malignancy. Salpingo-ophorectomies were carried out at laparatomies through midline incision extending from below the epigastric to just above the suprapubic. Patients were treated with postoperative antibiotics and analgesics and discharged home after 7 days on admission. They were followed up for 2 years and both recovered well without recurrence. It causes ill-health among millions of people each year and ranks alongside the Human immunodeficiency virus (HIV) as a leading cause of death worldwide. The aim of this study is to assess the outcome of tuberculosis treatment in a Rural Primary Health Care Centre, Benue State, North Central Nigeria. A three-year retrospective study was employed to review 150 tuberculosis cases treated from January 2014 - December 2016. Data were analysed using Statistical Package for Social Sciences (SPSS) version 20 and presented as tables and charts. The relationship between independent and outcome variables were tested with Chi-square test and P-value was set at 0.05. Of the 150 Tuberculosis patients treated, 84(56.0%) were males while 66(44.0%) were females. More than two-thirds (77.3%) of the patients had pulmonary tuberculosis. The successful treatment rate was 60.0% (32.7% treatment completed and 27.3% cured). Over twelve percent defaulted, 10.7% were lost to follow up, 6.7% were transferred out and 0.7% died. In this study, the successful treatment outcome was lower than the National target .The relationship between the disease sites; HIV status, Cotrimoxazole prophylaxis and the treatment outcome were statistically significant. With challenges in the use of the standard anthropometric parameters and higher mortality in these infants than in older children, it is pertinent to document the burden, pattern and risk factors for SAM in U6m. This study aimed to determine the prevalence of SAM in U6m, the pattern and the determinants of malnutrition using weight-for-length (WFL) Z score and mid-upper arm circumference (MUAC), and also to determine the usability of MUAC and WFL in detection of SAM in U6m. Infants six weeks to <6months were recruited. Socio-demographic and nutritional data were collected using researcher administered questionnaire. WFL z scores <-3 and MUAC <11.5cm were used to define SAM. Of the 233 infants aged<6months, mean age was 90.0±38.3days. Females accounted for 52.8%. Nineteen, (8.5%), were born with low birth weight and 11.7% were preterm deliveries. Males had higher mean length for age and weight for age than females but MUAC showed no significant difference. The prevalence of SAM was 2.6% by MUAC and WFL parameters but MUAC identified more SAM subjects in those <3months while WFL identified more in older children. Concordance between MUAC and WFL was poor. Both MUAC and WFL showed more female children than males with MAM. Lower social economic status was significantly associated with SAM while birth weight, birth order, maternal nutritional status and time of first feed did not affect prevalence of SAM in any of the parameters. The burden and pattern of diabetic eye diseases among persons living with diabetes in the commercial city of Aba, Nigeria is not known. This study, therefore, set out to determine this. This was a 6-year retrospective study in which all diabetic patients referred to or presenting at the Ophthalmology clinic of Abia State University Teaching Hospital (ABSUTH) Aba, Nigeria between January 2013 and December 2018 were included. The patients were examined by ophthalmologists and optometrists. Visual acuity, refraction, tonometry, slit-lamp examination of the anterior segment and fundus examination were carried out on each patient. Data was analyzed using SPSS version 20.0 and p<0.05 was considered statistically significant. Participants in the study were 56 (44.4%) males and 70 (55.6%) females. Diabetic ocular complications occurred in 2.4% of the total Ophthalmology clinic visits in ABSUTH, Aba. The commonest diabetic eye disease was cataract which was present in 50% of the participants, No diabetic patient presented with blindness and the commonest co morbid disease in the participants was systemic hypertension. This occurs as a result of a number of factors, for instance, drug abuse may facilitate the full expression of a latent psychiatric disorder; mental disorder may lead to SUD (drugs used for self- medication; or both SUD and mental disorders are caused by the same underlying brain deficit such as genetic vulnerability, neurotransmitter abnormality, structural or functional abnormality, and so on. After obtaining ethical clearance, the case notes of all patients who were admitted in the Centre for Addiction Treatment and Research, (CATR) Vom, Plateau state throughout the first quarter of year 2019 were traced. A systematic random sample of every third consecutive patient was selected beginning with the first patient admitted and relevant data were collected and analyzed. A total of fourty- eight (48) in-patients were analyzed. Ninety -four percent (94%) of the patients were males, the mean age of 23.6 + 5 years with 46% being below 25 years of age. Fourty-six percent (46%) had cannabis as their primary drug followed by alcohol (32%) and opioids (28%). Only 1 % had a history of injecting drug use. Twenty-nine percent (29%) of the SUD patients had co-occurring depression, nine percent (8%) had anxiety disorder, and five percent (4%) had Post Traumatic Stress Disorder (PTSD) in addition to their substance use disorder. G6PD deficiency is a common inherited enzyme defect associated with severe neonatal hyperbilirubinaemia that can result in permanent neurologic damage or death. This study was aimed at estimating the level of G6PD activity among icteric neonates to assess its usefulness in the evaluation of icteric neonates in Jos. One hundred and fifty icteric neonates (92 males and 58 females) whose parents consented were consecutively enrolled as they presented at the Special Care Baby Units (SCBU) of the Jos University Teaching Hospital (JUTH), Bingham University Teaching Hospital (BhUTH), and the Plateau State Specialist Hospital (PSSH), Jos. These subjects had their G6PD activity levels assayed using the Pointe Quantitative Diagnostic Kit (USA) while other relevant clinical information was obtained using a questionnaire. G6PD activity of the icteric neonates ranged between 0.54 and 24.18 IU/gHb with a mean of 8.02 ± 4.87 IU/gHb. Sixty-one (40.7 %), comprising 45 males and 16 female neonates were G6PD deficient with mean G6PD activity of 3.79 ± 1.37 IU/gHb while eighty-nine (59.3%) were G6PD normal with a mean G6PD activity of 10.92 ± 4.24 IU/gHb. G6PD activity in icteric neonates in Jos varies widely with a relatively high proportion of these neonates being G6PD deficient. We report the case of a 38year old farmer who had recurrent fronto-ethmoidal mucocoele and proptosis of the left eye. She presented with a history of hypertension and no other significant findings on examination. Computer tomography scan of the brain showed a left fronto-orbital uniform contrast-enhancing extra-axial lesion with thickened peripheral capsule and an associated left fronto-orbital skull defect. Cerebrospinal fluid analysis did not show any sign of infection. Intra-operative biopsy sample showed cheesy material which on culture grew Aspergillus species that was identified further using molecular methods. Antifungal agents were used to treat the patient. The weight and height are the most used anthropometric measurements in paediatric practice for the calculation of many parameters. The objective is to determine the reliability of weight and height estimates among children with sickle cell anaemia compared with measured weight and height. A random sample of SCA children aged eight months to 12 years was studied. Height and weight were measured using standard methods, while BMI was estimated using kg/m2. These were compared with estimated weight, height and BMI. Five subjects were adjudged obese using estimated BMI: three of these five subjects were identified as obese using actual BMI. Regression analysis showed that there appears to be more positive linear relationship for weight and length/height among children aged 1– 6 years. The measured mean weight and BMI were significantly lower than the estimated mean values. The overall mean of estimated length/height did not differ significantly from overall mean of the actual length/height. In comparison with the actual BMI, the estimated BMI did not identify any of the subjects with either thinness or overweight while children with obesity were over-estimated. Scaling up of new and under-used vaccines to 90% coverage could save more than 600,000 Nigerian children. Healthcare givers knowledge of vaccine used for immunization is essential to increase the vaccine uptake rates. This study assesses the knowledge of routine, underutilized and future vaccines among health workers. A cross sectional descriptive study was done among health care workers at a tertiary health facility in Nigeria. Using a pre-tested semi structured interviewer administered questionnaire, 220 respondents were selected by cluster sampling technique. The health workers’ knowledge of these vaccines was assessed using a scale developed for the study. Data collected were analyzed using SPSS version 22.The mean age of the respondents was 31.9 + 5.7 years . Doctors and nurses formed more than half of the respondents, 51.8% (n=114). About three quarters of respondents 72.3% (n=159) had good knowledge of vaccines used in routine immunizations. Knowledge of under-utilized and future vaccine was low with less than a fifth 18.6% (n=41) and one tenth 9.1% (n=20) having good knowledge scores respectively. Similarly, poor perception scores of future vaccines were observed in 90.1% (n=218) of the respondents. Only marital status was associated with knowledge of underutilized vaccines (p<0.05) and no association was observed between other sociodemographic variables and knowledge of these vaccines (P>0.05). The awareness level of health care workers on routine immunization is high. Their knowledge and perception of under-utilized and future vaccines were however low.
