Therefore, this study is aimed at reviewing a five-year profile of women admitted at a rehabilitation center in Jos, Nigeria for substance use and mental illness. This is a retrospective cross-sectional study carried out on 183 females admitted between 2012 and 2017 at a rehabilitation center in Jos, Nigeria. Data collection was carried out from documents note of these female participants. Data were analyzed using SPSS version 22. The mean age of the participants was 38.515.3 years. Almost half of the participants were within the age bracket of 20 – 29 years and married (45.9%). Depressive disorder (36.1%) was the commonest mental disorder in the studied participants. Among those abusing substances, multiple substance use (12.6%) was the most prevalent with alcohol use disorder (9.3%) the main diagnosis among those with substance use disorder. According to the result almost half of the studied participants were young, married and educated females. Depression and multiple substance abuse with alcohol the main drug of abuse was a cause for concern in this population. It is transmitted via faecal-oral route and causes life-threatening, cholera-like diarrhoea in immunocompromised individuals such as HIV/AIDS patients. This study aimed to determine the prevalence of cryptosporidiosis and associated risk factors among HIV/AIDS patients with diarrhoea. This was a cross-sectional study of 100 HIV/AIDS patients with diarrhoea in a tertiary health institution in Jos, North-central Nigeria between April and November 2019. Fifteen millilitres (15ml) of stool samples were collected from each patient after signing a consent form and filling a well-structured questionnaire. The oocytes of Cryptosporidium parvum were identified in the stool samples using modified Ziehl-Neelsen stain and polymerase chain reaction (PCR). The results obtained were computed using SPSS version 21. The mean age (standard deviation) of the study participants was 37.0 (± SD9.6), with a minimum age of 20years and a maximum age of 63 years. The study comprises of 53(53.0%) males and 47(47.0%) females. Fifty-four (54) of the participants were on antiretroviral (ARV) drugs while 46 were ARV drug naïve. The prevalence of cryptosporidiosis among the study population was 13.0%. Cryptosporidiosis was found in 10(21.7%) of the 46 ARV drugs naïve participants and in 3(5.6%) of the 54 participants on antiretroviral therapy. This was statistically significant at p = 0.016. There was also a significant relationship (p = 0.012) between the prevalence of cryptosporidiosis and the level of CD4+ T-lymphocytes count of the study participants. The infection was more among participants with CD4+ T-lymphocytes count less than 200 cells/μl. Having adequate knowledge and proper attitudes toward hepatitis B virus infection are crucial for its prevention. This study assessed the knowledge, attitude, and hepatitis B virus vaccination status of health care workers. A descriptive cross-sectional study among 120 healthcare workers that attended the World Hepatitis B-Day Celebration in June 2018 was undertaken. Data were obtained with a self-administered questionnaire on socio-demographic characteristics, knowledge, attitude towards HBV infection, and practice of hepatitis B vaccination. Commercial enzyme-linked immunosorbent assay kits were used to determine the prevalence of hepatitis B surface antigen. Data were analyzed using computer software SPSS version 22. The prevalence of HBsAg among the subjects was 4.5%. Only 53(47.7%) of the respondents had good knowledge of hepatitis B virus infection. The majority of respondents 91(82.0%) demonstrated a positive attitude towards hepatitis B virus infection and vaccination. Over 30% of respondents were aware of their hepatitis B virus infection status, and 29(26.1%) of them had received the hepatitis B virus vaccine. The major reason for the poor uptake of hepatitis B virus vaccination was not knowing where to get the vaccine in 40(57.1%) of them. Good knowledge of HBV infection had a statistically significant association with the age of respondents and their years of experience (p<0.05). It is recommended that a healthcare worker should be provided with more education and information on hepatitis B virus infection and vaccination. Several organizations in the state collaborated to organize the event and used various media to spread the key messages of the day. Inadequate resources however limited full commemoration. {"title":"The American Heart Association Classification of Blood Pressure and the Determinants of Hypertension among Medical Practitioners in Bayelsa State: A Cross-Sectional Study","authors":"T. Objective was to assess prevalence of hypertension using 2017 American Heart Association/American College of Cardiology (AHA/ACC) guideline and it's determinants among Medical Practitioners in Bayelsa State, Nigeria. Two hundred and forty-four apparently healthy medical doctors were recruited. A structured self-administered questionnaire was used to gather data on CVD risk factors. Anthropometric and blood pressure measurements were taken. Association between hypertension and sociodemographic features, anthropometric measures, smoking, alcohol, fruit and salt intake, exercise was explored with chi-square for proportions. Predictors of hypertension were identified by two-step binary logistic regression. A third of participants were women (29.9%), most were below age 30 years (40.2%) and married (54.9%). One fifth was consultant/professor cadre (18.9%) and a third had worked ≥11 years as medical practitioners. Almost 2 in every 3 of the participants (63.1%) were considered hypertensive by the AHA 2017 classification. However, using a cut off of ≥140/90mmHg used by other guidelines gave a prevalence of 25%. Only 13.5% had been diagnosed hypertensive prior to this study. The most important predictor of occurrence of hypertension was age, although marital status, salt intake, work cadre and duration of practice were also significantly associated with the occurrence of hypertension. The use of the 2017 ACC/AHA hypertension guidelines for diagnosis of hypertension with a blood pressure cut off ≥ 130/80 mmHg resulted in a marked increase in the prevalence of hypertension in medical doctors compared to other guidelines that use a cut off value of 140/90mmHg (63.1% versus 25%). Increasing age is a significant predictor of hypertension in medical doctors. Echocardiography can detect changes in cardiac geometry/function before overt CVD symptoms. This study aimed to evaluate left ventricular (LV) geometry and function in normotensive/hypertensive patients with T2DM without overt cardiac symptoms. A cross-sectional study in which fifty normotensives and fifty hypertensive adults with DM without overt cardiac symptoms were enrolled from the cardiology/diabetes clinics of Jos University Teaching Hospital (JUTH) in a simple random manner. Relevant history, physical examination and biochemical investigations were performed. 12-lead electrocardiography and echocardiograph assessment of LV geometry and function were also performed. Data was analyzed using Epi-info 7 statistical software; p value < 0.05 was considered significant. There were 27 females and 29 females in both groups. The prevalence of abnormal LV geometry was 36.0%, 95% CI 33.2-38.8% and 58.0%, 95% CI 55.2-60.8% in the normotensive and hypertensive groups respectively, P=0.028. Similarly, the prevalence of LV dysfunction was 38.0%, 95%CI 35.2-40.8% and 62.0%, 95%CI 59.2-64.8% respectively, P=0.017. The independent predictors of LV dysfunction were found to be duration of diabetes (OR 7.74, 95%CI 4.46-10.46), duration of hypertension ≥5years (OR 4.15, 95%CI 4.01-9.27), smoking (OR 4.34, 95%CI 1.32-6.23), body mass index ≥25 (OR 5.53, 95%CI 1.38-2.09) and glycosylated haemoglobin ≥7 (OR 7.11, 95%CI 2.15-0.81). There is high prevalence of LV dysfunction/abnormal LV geometry in T2DM patients without overt cardiac symptoms; co-morbid hypertension worsens these abnormalities. In the 42 weeks that it has lasted, about 46 million cases were confirmed with about 1.2 million deaths globally out of which 63, 173 and 1, 151 respectively occurred in Nigeria. Globally including our country and hospital, it has put a major strain on healthcare services with major disruptions in provision and uptake of these services and in people's lifestyle. Presently some adverse health consequences of these disruptions are known but many may yet to be discovered. The purpose of this study is to assess the impact of COVID-19 outbreak on the delivery of fine needle aspiration cytology (FNAC) services by our institution, a university teaching hospital. The hospital offers the full complement of specialist care including fine needle aspiration cytology, FNAC. Oral interviews of practitioners who provide this service were used to gather information for this study. Analysis of findings shows that a combination of patient factors, hospital and healthcare workers' factors and government policy all in response to the pandemic has led to a profound disruption of the fine needle aspiration cytology (FNAC) service in our hospital. With highly limited testing capacity and lack of PPE in our center, the trend is likely to persist through the pandemic. The acquisition of the oral colonizers may be altered by factors such as pregnancy events and outcomes, mode of delivery. This pilot study aimed to determine the Candida species that colonize the oral cavity of neonates and some maternal/neonatal factors that affect neonatal oral colonization. This was a cross sectional study in three tertiary health care facility in Jos, North-Central Nigeria. Oral swab samples were collected from the neonates and processed using both phenotypic and molecular techniques. The rate of neonatal oral colonization was 6.9%. The four Candida species isolated and characterized were Candida tropicalis as confirmed by sequencing and phylogenetic analysis. Candida tropicalis is one of the major neonatal oral colonizers. In this comparative randomised study, patients of American Society of Anesthesiologists’ (ASA) class I and II with age range of 18-60 years scheduled for short elective surgeries were randomly assigned into two equal groups. Patients were premedicated with fentanyl 1ug.kg-1. Anaesthesia was induced with either 2.5mg.kg-1 propofol or a sequence of 2mg.kg-1 lignocaine and 5mg.kg-1 thiopental given by a trained assistance. Anaesthesia was maintained with 2% isoflurane and 100% oxygen. Haemodynamic variables [Systolic Blood Pressure (SBP), Diastolic Blood Pressure (DBP), Mean Arterial Pressure (MAP) and Heart Rate (HR)] were measured non-invasively in three periods; before drug administration, immediately after drug administration prior to insertion of LMA and finally after LMA insertion.In group A, the baseline heart rate (92.3±11) was compared to the post-induction heart rate (100.7±09) with p=0.765 and post-insertion heart rate (98.0±13) with p=0.767. Although, there was a rise in the heart rate from the baseline after induction and insertion of LMA, this was not statistically significant. Also, baseline SBP (120.7±09) was compared to the post-induction SBP (102.5±07) with p=0.001 and post-insertion SBP (102.59±07) with p=0.001. This was statistically significant. The baseline DBP (77.9±08) was compared to the post-induction DBP (67.0±12) with p=0.004 and post-insertion DBP (62.5±09) with p=0.001.This was statistically significant. The baseline MAP (92.6±01) was compared to the post-induction MAP (79.7±01) with p=0.008 and post-insertion MAP (76.2±07) with p=0.001. This was also statistically significant. In group B, the baseline heart rate (93.2±12) was compared to post-induction heart rate (99.2±11) with p=0.520 and post-insertion heart rate (94.8±12) with p=0.989. This was not statistically significant. The baseline SBP (120.7±13) was compared to the post-induction SBP (115.9±12) with p=0.139 and post-insertion SBP (117.5±13) with p=0.318. This was not statistically significant. The baseline DBP (80.6±14) was compared to the post-induction DBP (75.2±11) with p=0.636 and post-insertion DBP (76.6±13) with p=0.712. This was also not statistically significant. The baseline MAP (94.3±01) was compared to the post-induction MAP (87.5±01) with p=0.779 and post-insertion MAP (88.3±01) with p=0.882. This was not statistically significant. We present a case report of a 54-year-old woman who presented with fever, anaemia and peripheral lymphadenopathy of four weeks duration, and was managed for disseminated tuberculosis initially. Full blood count and bone marrow aspiration cytology constituted part of her initial investigations and were found to be in keeping with acute lymphoblastic leukaemia (ALL-L3). Patient had supportive treatment and managed with Cyclophosphamide, Vincristine (Oncovin), Cytosine Arabinoside and Prednisolone (COAP Regimen), achieved complete clinical and haematologic remission, and has remained disease free after consolidation and maintenance therapy. No evidence of haematologic or clinical relapse at five years and currently in her ninth-year post diagnosis and treatment.
