Kaneko","doi":"10.4030/JJCS.37.239","DOIUrl":"https://doi.org/10.4030/JJCS.37.239","url":null,"abstract":"A 70-year-old woman receiving highly active antiretroviral therapy (HAART) for human immunodeficiency virus (HIV) infection presented at our clinic after she noticed a 1-cm mass in her left breast. A palpable tumor was diagnosed as squamous cell carcinoma with fine needle biopsy, but imaging revealed that the main lesion was surrounded by multiple tumors. We performed a left total mastectomy. Histopathology of the surgical specimen revealed an invasive squamous cell carcinoma surrounded by multiple ductal adenomas. There are few reports of neoplastic lesions in the breasts of patients with HIV. We report an illustrative case and discuss our findings in light of recent reports. and the development of other including diabetes, kidney and heart problems, and malignant disease. Clinical examination showed abdominal distension, decreased bowel sounds, and tenderness in the right upper abdomen with rebound tenderness. Unenhanced abdominal computed tomography showed intra-abdominal free air, ascites, and air collection within the bowel wall. The possibility of perforated peritonitis associated with the unexplained free air and ascitic fluid could not be eliminated, and therefore, emergency laparotomy was performed. During the surgery, a large amount of free air escaped when the abdomen was opened. Multiple gas-filled subserosal vesicles were found to be scattered throughout the surface of the bowel wall and the mesentery and almost entirely in the small intestine. There was no evidence suggestive of gastrointestinal tract perforation. Therefore, pneumatosis cystoides intestinalis (PCI) was diagnosed. Under a diagnosis of acute appendicitis and generalized peritonitis, we attempted the emergency operation. Because operative findings revealed the severe inflammation of the vermiform appendix spreading to the surrounding tissue, appendectomy and abdominal cavity drainage were performed. A definitive diagnosis of acute appendicitis with generalized peritonitis was made on the basis of the surgical and pathological findings. Moreover, a definitive diagnosis of appendicular schwannoma was made by immunohistochemical staining.
