The patient presented with nausea and upper abdominal pain in January 2008. An Upper gastrointestinal endoscopy showed a bezoar and ulcer in remnant stomach, and he admitted. After hospitalization he vomited. Abdominal computed tomography showed 40mm size low density mass causing obstruction of jejunum around a Treitz ligament. He was sent to our hospital for medi-cal treatment. We diagnosed the ileus by fall of a gastric bezoar. At urgent operation, the 70 × 40mm size benzoar was removed through an enterotomy. {"title":"A Case of Perforated Appendicitis Caused by a Small Appendiceal Calculus Found During Laparoscopic Surgery","authors":"H. Murakami, S. Fujitake","doi":"10.4030/JJCS.38.846","DOIUrl":"https://doi.org/10.4030/JJCS.38.846","url":null,"abstract":"Appendicolithiasis is a relatively rare condition, but it has been reported that appendicitis accompa-nied by an appendiceal calculus tends to be severe. We report a case of perforated appendicitis with a rapid course caused by an appendiceal calculus found during laparoscopic surgery. A 43-year-old woman presented with right lower abdominal pain and epigastralgia at our hospital. Blood analysis indicated a slight inflammatory response, and plain abdominal computed tomography (CT) revealed bilateral renal calculi and in the Pouch of Douglasʼ but no swollen appendix. The patient was suspected to have pelveoperitonitis and was admitted to the Department of Gynecology. However, her abdominal pain and inflammatory response became worse, and therefore, she was re-ferred to the Department of Surgery. The appendix did not appear swollen on CT, but appendicitis was not excluded because of an inflammatory change observed near the cecum, we performed laparoscopic appendectomy, and found a 5-mm diameter perforation at the appendiceal root and a 5-mm diameter calculus falling through the perforated site of the appendix. The patient underwent an ap-pendectomy and abdominal drainage, and was discharged from our hospital 9 days after surgery. {"title":"Triple Negative and Younger Cases in Breast Cancer Showed Higher Ki-67 Proliferation Index, Nuclear Grade and Histological Grade","authors":"Koichiro Sato, M. Yamaguchi, T. We evaluated the relationship between outcome of triple negative and younger cases and the proliferation potency of breast cancer cells. 94 cases of operated breast cancer were classified by subtype according to 2011 St. Gallen consensus meeting. Each subtype group and younger patients (=or<45) were investigated Ki-67 proliferation index indicated proliferation potency of cancer, nuclear grade (NG) and histological grade(HG). Out of 94 cases 48 were classified as Luminal A, 30 as Luminal B, 8 as HER2 and 8 as TN. The average proliferation index of Ki-67 was 6.4 ± 3.7 in Luminal A, 24.6 ±11.2 in Luminal B, 20.2 ± 6.6 in HER2, and 62.6 ± 15.7 in TN. There was a significant higher proliferation index of Ki-67 in the TN group. There were significantly higher NG and HG in the TN group compare with Luminal A and B. Mean Ki-67 proliferation index, NG and HG in cases which were 45 years old and less was significantly higher than those in cases which were more than 46 years old. An enhanced abdominal CT scan showed the acute appendicitis with abdominal abscess. Emergency appendectomy was performed. The excised appendix was swelling and its wall revealed gangrenous lesions with partial perforation. Pathological findings showed gangrenous appendicitis with numerous old ova of schistoma japonicum in the submucosal layer of appendix. patient with schistosoma japonicum 1978 the eradication of Miyairi shellfish, the intermediate host, and few cases with old ova of schistosoma japonicum reported. However, of schistosoma japonicum have recently been increasing. Suspicious active cases are needed for the treatment with anthelmintic administration. A total of 5 patients (4 men, one woman) who underwent SSG for anastmotic stricture were evaluated retrospectively. A median age was 63 (range, 58-75) years. One patient was performed preoperative chemoradiation therapy (pre-CRT). Partial and subtotal ISR were performed in 4 and one patient, respectively. All patients were observed for a median of 1,034 days (range, 559-3,010 days). Anastomotic stricture developed in 5 of 75 patients (6.7%). A median of operative duration and bleeding volume were 79.5 (25-136) min-utes and 10 (0-143) ml, respectively. Although reoperation was needed in one patient with pre-CRT, stricture was improved in all patients. Regarding the continence function of 4 patients, Kirwan grading scale showed ≥ Grade 2 in all patients. A procedure, SSG for anastmotic stricture developed after ISR was safety and effective.
